Background: Left bundle branch area pacing (LBBAP) has emerged as a physiological alternative pacing strategy to biventricular pacing (BIVP) in cardiac resynchronization therapy (CRT). We aimed to assess the impact of LBBAP vs. BIVP on all-cause mortality and heart failure (HF)-related hospitalization in patients undergoing CRT.

Article Synopsis
  • High driving pressure (DP) during mechanical ventilation can negatively impact outcomes in pediatric patients with acute respiratory distress syndrome (PARDS), particularly regarding 28-day mortality rates.
  • A study of 184 intubated children showed that those with a DP greater than 15 cm H2O at 72 hours had a higher risk for mortality and longer ventilation duration.
  • Key findings included that lower tidal volumes were linked to decreased mortality risk, while higher plateau pressures in non-survivors highlighted the importance of managing ventilation parameters effectively.*

  • Antiphospholipid syndrome (APS) involves blood clotting issues and pregnancy complications linked to antiphospholipid antibodies and is a common acquired disorder affecting clotting.
  • While strokes can occur in APS patients, there is limited data on the use of thrombolytic treatment (clot-busting drugs) and its safety during such events.
  • A clinical case is presented where thrombolysis was successfully used on an APS stroke patient without complications, highlighting the neurological symptoms associated with APS and stroke.

Early-life external exposome in children 2-5 years old in Colombia.

Environ Res

July 2024

School of Medicine, Universidad Pontificia Bolivariana, Medellín, 050034, Colombia; Department of Medical Microbiology and Infectious Diseases, University of Manitoba, Winnipeg, MB, R3E 0J9, Canada.

Exposome studies are advancing in high-income countries to understand how multiple environmental exposures impact health. However, there is a significant research gap in low- and middle-income and tropical countries. We aimed to describe the spatiotemporal variation of the external exposome, its correlation structure between and within exposure groups, and its dimensionality.

  • Cardiotoxicity from lithium, though rare, can be severe and present as various heart issues like high-degree AV blocks and QT prolongation.
  • A patient with severe sinus dysfunction due to cardiogenic shock was hospitalized after experiencing chest pain, dizziness, and weakness for a week, along with elevated lithium and TSH levels and acute kidney injury.
  • After being treated in the ICU for extreme sinus bradycardia, the patient recovered fully with supportive care and did not need long-term cardiac devices.

Intracardiac Echocardiography-guided Left Atrial Appendage Occlusion.

Arrhythm Electrophysiol Rev

March 2024

Cardiac Arrhythmia Service, Brigham and Women's Hospital, Harvard Medical School Boston, MA, US.

Left atrial appendage occlusion (LAAO) is a suitable alternative to oral anticoagulant therapy to prevent stroke in patients with AF. Most procedures are performed under transoesophageal echocardiography (TOE) guidance, which facilitates transseptal puncture, reduces the risk of procedurerelated complications and provides an additional method for device selection and real-time monitoring during device deployment. However, TOE has significant shortcomings, including the need for general anaesthesia/deep sedation as well as a significant risk of procedure-related adverse events.

Background: The Racing and Crowded Thoughts Questionnaire (RCTQ-13) is the most widely used specific scale for the measurement of racing thoughts, but there is currently no Spanish version that allow the evaluation in Spanish-speaking patients. The objective of this study is to translate, adapt, and validate the RCTQ-13 in a Colombian population with affective disorders.

Methods: The questionnaire was translated and back-translated, and corrections were implemented following a pilot test to improve comprehensibility.

Cardiovascular Complications in Patients with Heart Failure and COVID-19: CARDIO COVID 19-20 Registry.

J Cardiovasc Dev Dis

January 2024

Consejo Interamericano de Falla Cardiaca e Hipertensión Pulmonar (CIFACAH) de la Sociedad Interamericana de Cardiología (SIAC), Mexico City 01000, Mexico.

Since early 2020, different studies have shown an increased prevalence of COVID-19 and poorer prognosis in older adults with cardiovascular comorbidities. This study aimed to assess the impact of heart failure (HF) on cardiovascular complications, intensive care unit (ICU) admissions, and in-hospital mortality in patients hospitalized with COVID-19. The CARDIO COVID 19-20 registry includes 3260 hospitalized patients with a COVID-19 serological diagnosis between May 2020 and June 2021 from Latin American countries.

[The current state of renal sympathetic denervation in hypertension].

Arch Cardiol Mex

February 2024

Departamento de Cardiología, Servicio de Hemodinamia y Cardiología Intervencionista, Hospital San Vicente Fundación Rionegro, Colombia.

This review provides an overview of the efficacy and safety of renal sympathetic denervation as a therapeutic approach for resistant hypertension. While the initial enthusiasm was sparked by the results of early clinical trials, it was dampened by the findings of the Symplicity HTN-3 study. However, recent advances in catheter technology and more refined patient selection criteria have yielded more promising results.

Atrial fibrillation (AF) is the most common arrhythmia in adults. Prevention of the ischaemic risk with oral anticoagulants (OACs) is widely recommended, and current clinical guidelines recommend direct oral anticoagulants (DOACs) as preference therapy for stroke prevention. However, there are currently no clinical practice guidelines or recommendation documents on the optimal management of OACs in patients with AF that specifically address and adapt to the Central American and Caribbean context.

Oral Chagas Disease in Colombia-Confirmed and Suspected Routes of Transmission.

Trop Med Infect Dis

January 2024

Grupo Biología y Control de Enfermedades Infecciosas, Universidad de Antioquia, Medellín 050010, Colombia.

Chagas disease (CD) remains endemic throughout many regions of Colombia despite implementing decades of vector control strategies in several departments. Some regions have had a significant decrease in vectorial transmission, but the oral ingestion of through consumption of contaminated food and drink products is increasingly described. This form of transmission has important public health relevance in Colombia due to an increase in reported acute CD cases and clinical manifestations that often lead to significant morbidity and mortality.

Article Synopsis
  • Intracardiac echocardiography (ICE) is being evaluated against transesophageal echocardiography (TEE) for guiding left atrial appendage occlusion (LAAO) procedures, focusing on procedural characteristics and outcomes.
  • A review of twelve studies involving 5,637 patients shows that ICE and TEE have comparable success rates and adverse event occurrences, but ICE significantly reduces in-room time by about 28.6 minutes.
  • Although both methods result in similar rates of periprocedural leaks, ICE guidance is linked to a higher occurrence of residual interatrial septal defects compared to TEE.
