Introduction: transfusion-related complications are a major concern for surgeons performing total hip and knee arthroplasty (THA and TKA). Several strategies have been implemented to reduce transfusion rates, including the use of tranexamic acid (TXA), whose optimal dosage remains a matter of debate.
Objective: to evaluate the efficacy and safety of a single pre-surgical dose of TXA in reducing blood loss in THA and TKA.

Aortic valve calcification results from degenerative processes associated with several pathologies. These processes are influenced by age, chronic inflammation, and high concentrations of phosphate ions in the plasma, which contribute to induce mineralization in the aortic valve and deterioration of cardiovascular health. Environmental factors, such as wood smoke that emits harmful and carcinogenic pollutants, carbon monoxide (CO), and nitrogen oxide (NO), as well as other reactive compounds may also be implicated.

Splenic artery pseudoaneurysms represent one of the most feared vascular complications of chronic pancreatitis. Sectional imaging studies such as computed tomography represent the first-line diagnostic tool for this pathology, being found as an incidental finding in patients with risk factors. We report the case of a splenic artery pseudoaneurysm in a 55-year-old patient diagnosed with chronic pancreatitis.

Background: The trend over time and across different geographical areas of outcomes and management with noninvasive ventilation or invasive mechanical ventilation in patients admitted for acute exacerbations of chronic obstructive pulmonary disease and treated with ventilatory support is unknown. The purpose of this study was to describe outcomes and identify variables associated with survival for patients admitted to an intensive care unit (ICU) with acute exacerbation of chronic obstructive pulmonary disease [aeCOPD] who received noninvasive or invasive mechanical ventilation worldwide.
Methods: Retrospective, multi-national, and multicenter studies, including four observational cohort studies, were carried out in 1998, 2004, 2010, and 2016 for the VENTILAGROUP following the same methodology.

Almost 20% of patients affected by COVID-19 develop dermatological symptoms after recovery. This condition is termed as post-COVID-19 syndrome and is characterized by a state of hyperinflammation, as well as deregulations in the humoral response of CD8 T-cells. Since there is no specific treatment for these injuries, the treatment of choice depends on the symptoms; thus, it is essential to provide a description of the type and nature of the injuries presented.

Spontaneous laryngeal rupture is an unusual event, typically caused by a sudden increase in barometric pressure and biomechanical forces within the laryngotracheal complex. Triggers often include actions such as coughing, sneezing, or violent retching. Given the delicate structures involved in this region, careful assessment is essential to ensure proper management and prevent complications.

Bouveret's syndrome is a rare form of bowel obstruction caused by the impaction of a large gallstone through a cholecystoduodenal fistula, leading to gastric outlet obstruction. This article aims to highlight the clinical presentation and management of this syndrome, given its low incidence and high mortality rate of 12% to 30%. We present the case of an 83-year-old patient with a history of diabetes and hypertension who presented with abdominal pain, distension, and vomiting.

Introduction: The coronavirus disease 2019 (COVID-19) pandemic is a global health concern and has persisted through the emergence of variants that have caused subsequent waves of COVID-19 due to the high dispersion and contagiousness of the virus. The aim of this work was to analyze the epidemiology of the cases of reinfection by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) variants during the third and fourth wave of the COVID-19 pandemic at the Hospital Juárez de México (HJM).
Methodology: A prospective study of the cases of SARS-CoV-2 reinfection, variants detected, symptoms, and associated comorbidities was carried out on 1,347 patients who attended the HJM from September 2021 to July 2022.

BACKGROUND Compartment syndrome of the forearm has been associated with a variety of etiologies, including fractures, snake bites, complications of certain infections, and, very rarely, spider bites. Loxoscelism is the venom-related clinical manifestation of the bite of spiders of the genus Loxosceles, also called brown or fiddler (violinist) spiders. It manifests locally/regionally with pain, erythema, and edema, with subsequent necrotic plaque formation at the site of the bite.

Introduction: Obesity is a global health issue that significantly increases morbidity and mortality when the Body Mass Index (BMI) reaches values ≥ 50. While metabolic and bariatric surgery (MBS) is the most effective treatment for severe obesity, it carries risks. Robotic surgery is promising but not extensively studied in Mexico, which presents an opportunity for research at a National Hospital with an academic program.

In plastic surgery, the reconstructive ladder is a systematic approach used to guide the planning and execution of reconstructive procedures. The use of autologous tissues is preferred in the breast reconstruction process due to the multiple advantages they offer. The latissimus dorsi musculocutaneous flap has been the therapeutic option of choice in these cases, replacing other surgical techniques because of the high adaptability and low complication rates.

Background: SARS-CoV-2 infection has become a major international issue, not only from a medical point of view, but also social, economic and political. Most of the available information comes from the United States, Europe, and China, where the population and the socioeconomic status are very different from Latin American countries. This study evaluates the effect of regional socioeconomic characteristics on mortality due SARS-CoV-2 infection in patients with immune-mediated rheumatic diseases (IMRD) from Argentina, Mexico and Brazil.
