September 2024

Medica Scientia Innovation Research (MEDSIR) - Oncoclínicas&Co, Jersey City, NJ, USA; International Breast Cancer Center (IBCC), Pangaea Oncology, Quiron Group, Barcelona, Spain; Universidad Europea de Madrid, Faculty of Biomedical and Health Sciences, Department of Medicine, Madrid, Spain. Electronic address:

Article Synopsis
  • Leptomeningeal disease (LMD) significantly impacts survival and quality of life, and trastuzumab deruxtecan (T-DXd) has demonstrated effectiveness in treating advanced HER2-positive and HER2-low breast cancer in patients with a history of brain metastases or LMD.
  • In a phase 2 trial involving seven patients with confirmed LMD, T-DXd was administered, revealing a median overall survival (OS) of 13.3 months and a progression-free survival (PFS) of 8.9 months, with many patients achieving disease stabilization.
  • The results suggest T-DXd has promising activity in this challenging patient population, indicating a need for further research on its efficacy in treating LMD.

ESMO Open

September 2024

Medica Scientia Innovation Research (MEDSIR) - Oncoclínicas & Co, Jersey City, USA, Sao Paulo, Brazil; Hospital Arnau de Vilanova, FISABIO, Valencia; Universidad Católica de Valencia, Valencia, Spain. Electronic address:

Background: Trastuzumab deruxtecan (T-DXd) is approved for human epidermal growth factor receptor 2 (HER2)-positive and HER2-low advanced breast cancer (ABC). T-DXd has shown encouraging intracranial activity in HER2-positive ABC patients with stable or active brain metastases (BMs); however, its efficacy in patients with HER2-low ABC with BMs is not well established yet.

Methods: DEBBRAH is a single-arm, five-cohort, phase II study evaluating T-DXd in patients with central nervous system involvement from HER2-positive and HER2-low ABC.

Nanoparticles engineered to combat cancer and other life-threatening diseases may significantly improve patient outcomes. However, inefficient nanoparticle delivery to tumors limits their use and necessitates the development of complex delivery approaches. Here, we examine this issue by harnessing the tumor-homing abilities of human mesenchymal stem cells (MSCs) to deliver a decoupled theranostic complex of rare earth-doped nanoparticles (dNPs) and photosensitizer chlorin e6 (Ce6) to tumors.

The main objectives of the pre-anaesthesia consultation are to establish the patient's anaesthesia and surgical risk, evaluate and optimize their health status, provide the patient with information and preoperative recommendations, and fulfil the legally established bureaucratic obligations. The incorporation of information technologies - e-Health - has maximised the efficiency of pre-anaesthesia assessments and provided patients with an added benefit. The SEDAR Task Force has developed a digital framework as an alternative to the conventional pre-anaesthesia assessment process, and has put forward a series of policies and technical recommendations for the incorporation of different types of pre-anaesthesia teleconsultation services in hospital anaesthesiology departments.

  • - Metastatic renal cell carcinoma (mRCC) with sarcomatoid features (sRCC) is aggressive and has a poor prognosis; this study aimed to investigate its clinical features and outcomes compared to non-sarcomatoid patients.
  • - The study analyzed data from 1362 mRCC patients, finding that the median overall survival for sRCC patients was 26.8 months, significantly lower than the 35.3 months for non-sRCC patients.
  • - Both first-line treatment strategies, immunotherapy combined with immunotherapy (IO + IO) and immunotherapy with a tyrosine kinase inhibitor (IO + TKI), showed similar survival outcomes, confirming that

Emergencias

June 2024

Departamento de Medicina Interna, Clínica Universidad de Navarra, Madrid, España. CIBER Enfermedades Respiratorias (CIBERES), Madrid, España. Interdisciplinar Teragnosis and Radiosomics (INTRA) Research Group, Universidad de Navarra, España.

Objective: To estimate the prevalence of inappropriate use of prophylaxis to prevent venous thromboembolism (VTE) in patients with medical diseases admitted to hospital from the emergency department. To explore variables associated with inappropriate thromboprophylaxis.

Methods: Prospective multicenter cohort study in 15 hospital emergency departments.

This study generated evidence to guide anticoagulation in patients with VTE after vaccination for COVID-19. We provided data on the low recurrence rate after cessation of anticoagulant therapy and the findings for this study offer timely insights into the management of a potentially vaccine-related adverse event.

  • Surgical rhinoplasty has a high revision rate, and nonsurgical rhinoplasty (NSR) can address complications from previous surgeries, potentially avoiding the need for secondary operations.
  • A systematic review of 20 articles analyzed data from over 2,000 patients, focusing on the reasons for NSR, the types of fillers used, and patient satisfaction.
  • Hyaluronic acid was the most commonly used filler, and while patient satisfaction was generally high and major complications were low, there was still a noted risk of skin necrosis with NSR procedures.

Chulalongkorn Centre of Excellence for Parkinson's Disease and Related Disorders, Department of Medicine, Faculty of Medicine, Chulalongkorn University and King Chulalongkorn Memorial Hospital, Thai Red Cross Society, Bangkok, Thailand.

Article Synopsis
  • Exercise improves physical function, cognition, and quality of life for people with Parkinson's (PwP), but participation levels vary among individuals.
  • A survey of 2,976 PwP found that 40.6% exercised regularly, with higher activity linked to group workouts, weightlifting, and use of muscle-building equipment.
  • Factors promoting exercise included education level, receiving advice from doctors, being diagnosed at a younger age, while issues like falls and bothersome symptoms deterred exercise participation.

Introduction: Pulsed-field ablation (PFA) is a novel nonthermal energy that shows unique features that can be of use beyond pulmonary vein ablation, like tissue selectivity or proximity rather than contact dependency.

Methods And Results: We report three cases of right focal atrial tachycardias arising from the superior cavoatrial junction and the crista terminalis, in close relationship with the phrenic nerve, effectively ablated using a commercially available PFA catheter designed for pulmonary vein isolation without collateral damage.

Conclusion: PFA can be useful for treating right atrial tachycardias involving sites near the phrenic nerve, avoiding the need for complex nerve-sparing strategies.

The Hodgkin and Reed - Sternberg (HRS) cells in classical Hodgkin Lymphoma (cHL) actively modify the immune tumor microenvironment (TME) attracting immunosuppressive cells and expressing inhibitory molecules. A high frequency of myeloid cells in the TME is correlated with an unfavorable prognosis, but more specific and rare cell populations lack precise markers. Myeloid-derived suppressor cells (MDSCs) have been identified in the peripheral blood of cHL patients, where they appear to be correlated with disease aggressiveness.

Androgenetic alopecia, the most common cause of hair loss affecting both men and women, is typically treated using pharmaceutical options, such as minoxidil and finasteride. While these medications work for many individuals, they are not suitable options for all. To date, the only non-pharmaceutical option that the United States Food and Drug Administration has cleared as a treatment for androgenetic alopecia is low-level laser therapy (LLLT).
