[Successful treatment of thrombotic thrombocytopenic purpura with vincristine report of one case].

Thrombotic thrombocytopenic purpura presents as a multisystemic disease with thrombocytopenia, microangiopathic hemolytic anemia, fever, neurological and renal involvement. We report a 24 years-old male presenting with purpura and a generalized seizure. His blood tests showed an hemolytic anemia, unconjugated hyperbilirubinemia, increased lactated dehydrogenase, thrombocytopenia and impairment of renal function.

Introduction: The term dementia refers to the deterioration of the intellectual or cognitive functions, with little or no alteration of consciousness, which is capable of interfering with the activities of daily living and the ability to cope by oneself. One infrequent cause of dementia is its being secondary to a thalamic lesion and is normally due to the involvement of both thalami.

Case Report: We report a case of sudden onset dementia caused by lesions affecting only the left thalamus.

[Hepatic resection in localized Caroli disease].

Caroli disease is a cystic congenital malformation of the intrahepatic biliary tract, frequently associated with lithiasis. Commonly, cystic dilatations are bilateral and infrequently they affect only one hepatic lobule or segment. We report six patients with localized Caroli disease, three in the right and three in the left hepatic lobule, that were subjected to a hepatic resection.

[Common bilio pancreatic channel syndrome in adults].

[Acute fatty liver of pregnancy: 2 clinical cases].

The clinical evolution and the laboratory pattern of two patients with histopathologically confirmed fatty liver of pregnancy is described. Both babies were delivered vaginally after oxytocic induction and the mothers recovered completely during the puerperium. There was one fetal death in labor secondary to severe abdominal vascular malformation.

The observation that in Chilean Maternities, newborn infants are separated from their mothers during their first hours of life, originated this experience, destined to strengthen the early and permanent mother-infant bonding, where both of them were attended by the same assistant nurse, in a joined observation room. The influence of this method on the neonatal cardiorespiratory adaptation, as well as the initial moment of the lactation, was evaluated. In three groups of healthy newborn infants, full term newborn infants appropriate for gestational age (250), full term newborn infants small for gestational age (94) and preterm newborn infants (38), both vital signs and the initial moment of lactation were registered, during the first hours of life, period in which they remained with their mothers.

An experience of feeding healthy premature infants (< 1,800 g) on their mother's milk is communicated. Feeding starts at an early time (15 hours); achieving maximum volumes at the 12th day. Loss of weight is 10% less than the birth weight and recovery occurs at the 12th day.

The first hundred laparoscopies carried out in the "Hospital Parroquial de San Bernardo", between november 1986 and june 1988; were evaluated. The indications for the procedure were: 38% study of infertility, in which a 78% of tubal obstruction or adherencial syndrome was demonstrated; 37% because of ectopic pregnancy, with a diagnostic correlation of 48.6%.
