Curr Pediatr Rep

July 2021

Institutional Development Program of the Brazilian National Health System (PROADI-SUS), Hospital Moinhos de Vento (HMV), R. Ramiro Barcelos, 910, Porto Alegre, RS 90035-000 Brazil.

Purpose Of Review: To present the implementation of a telemedicine project (TeleICU) in pediatric intensive care units (ICU) throughout different Brazilian regions.

Recent Findings: Although telemedicine in pediatric ICUs has shown evidence of benefit in numerous studies with potential to 18 mitigate existing disparities, in Brazil, its use is still under development. Brazil has several opportunities for implementing this resource since, according to the National Registry of Healthcare 20 Establishments (NRHE), there is a discrepancy in the density of pediatric intensive care physicians per patient and the availability 21 of pediatric ICU beds per number of inhabitants.

Most patients with chronic venous disease (CVD) and reflux in the saphenous vein are treated with saphenous stripping or ablation. The venous hemodynamics approach offers the possibility of treating saphenous reflux without eliminating the saphenous vein. We present 2 cases in which venous reflux was eliminated while preserving the great saphenous vein, after treatment with hemodynamic sclerotherapy using a protocol of synergic use of Dextrose and long pulse Nd YAG 1064 laser.

Novel approaches to facilitate the implementation of guidelines in the ICU.

J Crit Care

December 2020

Department of Internal Medicine, Pulmonary and Critical Care Medicine and Institute for Healthcare Policy & Innovation, Michigan Center for Integrated Research in Critical Care, University of Michigan, Ann Arbor, MI, USA.

The effective implementation of evidence-based recommendations in routine intensive care unit (ICU) practice is challenging. Barriers related to the proposed recommendations, local contexts and processes can make the adoption of evidence-based practices difficult, contributing to healthcare inefficiency and worse patient and family outcomes. This review discusses the common barriers to guideline implementation in critical care settings, explores how implementation science provides an important framework for guiding implementation interventions, and discusses some specific and proven implementation strategies to improve adherence to evidence-based practices in the ICU.

Article Synopsis
  • In Brazil, cancer registries are lacking, leading to insufficient data about breast cancer patients' characteristics and outcomes, which prompted the AMAZONA III study to better understand these factors.
  • The study included 2,950 women diagnosed with invasive breast cancer between January 2016 and March 2018, examining their health insurance type and disease stage at diagnosis.
  • Results showed that publicly insured patients typically had later-stage cancer and were more frequently diagnosed through symptoms, while privately insured patients were more likely to have earlier-stage disease and were often diagnosed through screening.

Background: The quality of clinical care of brain-dead potential organ donors may help reduce donor losses caused by irreversible or unreversed cardiac arrest and increase the number of organs donated. We sought to determine whether an evidence-based, goal-directed checklist for donor management in intensive care units (ICUs) can reduce donor losses to cardiac arrest.

Methods/design: The DONORS study is a multicentre, cluster-randomised controlled trial with a 1:1 allocation ratio designed to compare an intervention group (goal-directed checklist for brain-dead potential organ donor management) with a control group (standard ICU care).

Clinical practice guidelines in Brazil - developing a national programme.

Health Res Policy Syst

June 2020

Department of Health Research Methods, Evidence and Impact and of Medicine, McMaster GRADE Centre, McMaster University Health Sciences Centre, Hamilton, Canada.

In Brazil, governmental and non-governmental organisations develop practice guidelines (PGs) in order to optimise patient care. Although important improvements have been made over the past years, many of these documents still lack transparency and methodological rigour. In order to conduct a critical analysis and define future steps in PG development in Brazil, we carried out a structured assessment of strengths, weaknesses, opportunities and threats (SWOT analysis) for the development of a national guideline programme.

TP53 variants of uncertain significance: increasing challenges in variant interpretation and genetic counseling.

Fam Cancer

October 2019

Programa de Pós-Graduação em Genética e Biologia Molecular (PPGBM), Universidade Federal do Rio Grande do Sul (UFRGS), Av. Bento Gonçalves, 9500 - Prédio 43323 M, Porto Alegre, RS, 91501-970, Brazil.

Li-Fraumeni syndrome (LFS) and Li-Fraumeni Like (LFL) are autosomal dominant cancer predisposition syndromes caused by pathogenic germline variants in the TP53 gene. Recent studies have shown that the incorporation of next-generation sequencing by using multigene panels in clinical practice has resulted in the frequent identification of variants of uncertain significance (VUS). Given that there is no established medical management for VUS carriers, the identification of these variants may cause confusion and anxiety for both patients and practitioners.
