February 2024

Universidad de Extremadura, Departamento de Terapeútica Médico Quirúrgica, Facultad de Medicina y Ciencias de la Salud, Badajoz, Spain; Department of HBP Surgery and Liver Transplantation, Hospital Universitario de Badajoz, Badajoz, Spain. Electronic address:

Article Synopsis
  • The study focuses on the recurrence of isolated pancreatic metastases from Renal Cell Carcinoma (RCC) and evaluates the outcomes of repeat pancreatic resections after initial surgery.
  • It involved a retrospective analysis of 131 resections in 116 patients from January 2010 to May 2020, comparing those who had single versus iterative pancreatic surgeries.
  • Results indicated that while disease-free survival (DFS) rates were slightly better in the iterative group, overall survival (OS) rates were similar for both groups, suggesting that repeat surgery is a viable option for managing RCC-related pancreatic metastases.

Introduction: Psoriatic arthritis (PsA) is a complex and heterogeneous inflammatory disease. Secukinumab, a biologic disease-modifying antirheumatic drug (bDMARD), has extensive clinical evidence of efficacy and safety in the treatment of PsA but data in clinical practice are still limited. This study aims to provide real-world evidence on secukinumab use, effectiveness, and persistence in PsA.

Introduction: Teriflunomide is a once-daily oral immunomodulator approved for relapsing forms of multiple sclerosis (MS) or relapsing-remitting multiple sclerosis (RRMS; depending on the local label), based on extensive evidence from clinical trials and a real-world setting on efficacy, tolerability and patient-reported benefits. The TERICARE study assessed the impact of teriflunomide treatment over 2 years on health-related quality of life (HRQoL) and some of the most common and disabling symptoms of MS, such as fatigue and depression.

Methods: This prospective observational study in Spain included RRMS patients treated with teriflunomide for ≤ 4 weeks.

Post-Ureteroscopic Lesion Scale to determine ureteral wall damage, not so easy to employ.

Actas Urol Esp (Engl Ed)

March 2024

Departamento de Enfermería Comunitaria, Medicina Preventiva y Salud Pública e Historia de la Ciencia, Universidad de Alicante, Alicante, Spain.

Objective: To analyze the level of agreement of the Post-Ureteroscopy Lesion Scale (PULS) and the consequences on its application in clinical practice with more reliable statistical data than the one used in the original work.

Methods: 14 URS and 14 micro-URS were performed in 14 female porcine model. All the procedures were video recorded and an anatomopathological analysis was performed in each ureter.

Self-healing juvenile cutaneous mucinosis is a rare entity, characterized by the presence of subcutaneous nodules together with frequent nonspecific systemic symptoms, which occurs in the pediatric age and characteristically resolves spontaneously. Although the diagnostic criteria do not require a biopsy to be performed, it is frequently performed, and an abundant dermal mucin deposition will be observed together with other features such as fibroblastic proliferation. Although the prognosis is benign, follow-up is required for the eventual development of a rheumatologic disease.

Article Synopsis
  • Fosfomycin trometamol is being explored as a potential step-down therapy for patients with bacteraemic urinary tract infections caused by multidrug-resistant E. coli (MDR-Ec), though data is limited.
  • In the FOREST trial, patients who transitioned from IV treatments to either oral fosfomycin or other oral medications showed similar clinical and microbiological cure rates, with fosfomycin achieving 78.7% success compared to 80.9% for other drugs.
  • However, fosfomycin had a significantly higher relapse rate (15% vs. 4.3%), indicating that while it is a viable option, its effectiveness needs further evaluation due to this concern.

Managing moderate to severe psoriasis in older adults is complex due to factors characteristic of the later years of life, such as associated comorbidity, polypharmacy, and immunosenescence. This consensus statement discusses 17 recommendations for managing treatment for moderate to severe psoriasis in patients older than 65 years. The recommendations were proposed by a committee of 6 dermatologists who reviewed the literature.

Anti-programmed death-1 (anti-PD1) treatment has significantly improved outcomes of advanced melanoma with a considerable percentage of patients achieving complete response (CR). This real-world study analyzed the feasibility of elective anti-PD1 discontinuation in advanced melanoma patients with CR and evaluated factors related to sustained response. Thirty-five patients with advanced cutaneous or primary unknown melanoma with CR to nivolumab or pembrolizumab from 11 centers were included.

Introduction: The association between cancer and stroke is well documented and entails a worse prognosis for both pathologies. However, the prevalence of active and occult cancer among stroke patients is not sufficiently established, and neither are the predictors of cancer-associated stroke. Their knowledge is important for better identification and optimisation of the treatment of these patients.

Analysis of urine samples from COVID-19 patients by H NMR reveals important metabolic alterations due to SAR-CoV-2 infection. Previous studies have identified biomarkers in urine that reflect metabolic alterations in COVID-19 patients. We have used H NMR to better define these metabolic alterations since this technique allows us to obtain a broad profile of the metabolites present in urine.

Article Synopsis
  • Dermoscopy is an important noninvasive technique that helps in the diagnosis of melanoma and can aid in assessing its thickness before surgery.
  • This study examined the relationship between blood vessel morphology in primary melanomas and factors like the Breslow index (a measure of melanoma thickness) and ulceration.
  • Results indicated that certain types of blood vessels are linked to thicker melanomas and that specific vessel types may indicate ulceration or dermoscopic regression, though the study faced challenges with case selection and identifying vessels in pigmented melanomas.

Background: We carried out a meta-analysis since there is not enough evidence to recommend for or against therapeutic-dose anticoagulation compared with thromboprophylaxis in noncritically ill patients hospitalized with Covid-19.

Methods: We performed a systematic literature search using PubMed, Embase, Cochrane Library, and MedRxiv for randomized trials that included therapeutic-dose with low-molecular-weight heparin (LMW) or thromboprophylaxis with LMW heparin in noncritically ill patients admitted to the hospital with Covid-19. We identified five open-label studies for analysis with a total of 3220 patients.
