Department of Surgery, Division of Coloproctology, Hospital de Clínicas de Porto Alegre, Federal University of Rio Grande Do Sul. Room 600 A, Rua Ramiro Barcelos, Porto Alegre, RS, 2350, Brazil.

Article Synopsis
  • The study investigates female patients' feelings about having chaperones during proctological exams by male physicians, noting ongoing debate in medical guidelines.
  • In a randomized trial with 190 women, one group had a chaperone and the other did not, with follow-up questionnaires assessing comfort and future preferences.
  • Results showed no significant comfort difference between groups, but more women without a chaperone preferred the exam format for the future, suggesting chaperones should be optional based on individual patient needs.

Alveolar Paratesticular Rhabdomyosarcoma in an Adult Patient With PAX3-FOXO1 Fusion and Unfavorable Evolution.

Cureus

October 2024

Oncology, Pontifícia Universidade Católica Do Rio Grande Do Sul, Porto Alegre, BRA.

Rhabdomyosarcomas, malignant mesenchymal tumors of skeletal striated muscle tissue cells, are usually rare in adults. However, when they occur in this population, the prognosis is usually poor, especially if the condition is associated with molecular factors such as the PAX3-FOXO1 fusion. Here, We report a case of paratesticular alveolar rhabdomyosarcoma in an adult patient who initially complained of increased scrotal volume for two years and presented with a PAX3-FOXO1 fusion.

Aims: The use of ultrasound (US)-guided venous puncture for cardiac pacing/defibrillation lead placement may minimize the risk of periprocedural complications and radiation exposure. However, none of the published studies have been sufficiently powered to recommend this approach as the standard of care. We compare the safety and efficacy of ultrasound-guided axillary venous puncture (US-AVP) vs.

Background: Proper analysis and interpretation of health care data can significantly improve patient outcomes by enhancing services and revealing the impacts of new technologies and treatments. Understanding the substantial impact of temporal shifts in these data is crucial. For example, COVID-19 vaccination initially lowered the mean age of at-risk patients and later changed the characteristics of those who died.

  • - The MONARCH plus study found that adding abemaciclib to endocrine therapy significantly improved progression-free survival (PFS) and objective response rate (ORR) in postmenopausal women with HR+/HER2- advanced breast cancer.
  • - In the study, cohort A (no prior systemic therapy) showed a median PFS of 28.27 months for abemaciclib users compared to 14.73 months for those on placebo, while cohort B (progression on prior therapy) showed 11.41 months vs. 5.59 months, respectively.
  • - Although abemaciclib was associated with some adverse events, they were manageable, and it demonstrated a trend toward overall survival (OS

Background: COVID-19 vaccines effectively prevent infection and hospitalization. However, few population-based studies have compared the clinical characteristics and outcomes of patients hospitalized for COVID-19 using advanced statistical methods. Our objective is to address this evidence gap by comparing vaccinated and unvaccinated patients hospitalized for COVID-19.

Spinocerebellar ataxia (SCA) is an autosomal dominant hereditary disease with a low prevalence, for which more than 50 types have been described. This group of neurodegenerative diseases can present as different phenotypes with varying progression rates and clinical manifestations of different severities. Herein, we systematically reviewed existing medical literature to describe the main characteristics of polyneuropathy in patients with SCA types 2, 3, and 10.

Present and Future Surgical Options for Tricuspid Regurgitation.

Rev Cardiovasc Med

May 2024

Clinic of Cardiac Surgery, HerzZentrum Hirslanden Zurich, 8008 Zurich, Switzerland.

Tricuspid regurgitation, once considered a relatively benign condition, has now gathered significant attention due to new evidence showing its impact on both short- and long-term follow-up. While surgical intervention remains the established standard approach for treating severe tricuspid regurgitation, current guidelines provide Class I indication for intervention in only a limited set of scenarios. This review delves into the present and future perspectives of surgical tricuspid regurgitation management, examining aspects such as disease prognosis, surgical indications, outcomes, and a comprehensive overview of past and upcoming clinical trials.

Background: Isatuximab is an anti-CD38 monoclonal antibody approved for the treatment of relapsed or refractory multiple myeloma. Previous analyses of the IKEMA trial showed prolonged progression-free survival in patients with this disease who received isatuximab in combination with carfilzomib-dexamethasone as compared with those who received carfilzomib-dexamethasone alone. Herein, we report the analysis of overall survival from the IKEMA trial.

Importance: Cervical cancer is a common and lethal cancer worldwide. Bintrafusp alfa is a first-in-class bifunctional fusion protein composed of the extracellular domain of the human transforming growth factor β receptor II (or transforming growth factor β trap) fused via a flexible linker to the C-terminus of each heavy chain of an immunoglobulin G1 antibody blocking programmed cell death 1 ligand 1.

Objective: To evaluate the safety and response rates of bintrafusp alfa in patients with recurrent or metastatic cervical cancer.

Patients with chronic kidney disease (CKD), especially those on dialysis or who have received a kidney transplant (KT), are considered more vulnerable to severe COVID-19. This susceptibility is attributed to advanced age, a higher frequency of comorbidities, and the chronic immunosuppressed state, which may exacerbate their susceptibility to severe outcomes. Therefore, our study aimed to compare the clinical characteristics and outcomes of COVID-19 in KT patients with those on chronic dialysis and non-CKD patients in a propensity score-matched cohort study.

Lung cancer is a major cause of cancer deaths worldwide. Non-small-cell lung cancer (NSCLC) represents most lung cancer cases, and approximately one-third of patients present with stage III disease at diagnosis. As multiple treatment plans can be adopted for these patients depending on tumor size and nodal staging, stage III NSCLC management is challenging.
