Predictors of use of direct oral anticoagulants in acute pulmonary embolism.

Arch Cardiol Mex

November 2022

Servicio de Cardiología, Sanatorio Güemes, Ciudad Autónoma de Buenos Aires. Argentina.

Introduction: In the last decade, direct oral anticoagulants (DOACs) have been incorporated as an anticoagulation tool in patients with acute pulmonary thromboembolism (PTE). Although they have a better pharmacological profile than vitamin K antagonists (VKA), the use of these drugs is not massive. The objective of this study was to evaluate the use of DOACs in patients with acute PE and to detect determinants of its use.

Introduction: There is little evidence regarding the influence of emotional variables on breastfeeding among mothers of very low birth weight infants (VLBWIs). The objective of this study was to measure breast milk production (BMP) at two points in time during neonatal hospitalization and its association with anxiety, depression, and breastfeeding self-efficacy levels among mothers of VLBWIs.

Population And Methos: Prospective, observational, and multicenter study in mothers of VLBWIs (500-1500 g) from 9 NEOCOSUR Network centers.

Impaired bone microarchitecture and strength in patients with tumor-induced osteomalacia.

J Bone Miner Res

IDIM, Instituto de Diagnóstico e Investigaciones Metabólicas, Universidad del Salvador, LIBERTAD 836 1 PISO, BUENOS AIRES, Argentina, C1012AAR, Argentina.

Some studies based on bone biopsy have demonstrated that in patients with tumor-induced osteomalacia (TIO) the mineralization process of the bone matrix is profoundly disturbed. However, the interrelationship between clinical and biochemical features and bone microarchitecture in this disease needs further analysis. With this purpose in mind, we set out three objectives: (i) to determine bone microarchitecture and estimated bone strength in a group of patients with tumor-induced osteomalacia using high-resolution peripheral quantitative computed tomography (HR-pQCT) and finite element analysis (FEA), (ii) to investigate correlations between duration of disease, biochemical features, bone density, HR-pQCT and FEA parameters, and (iii) to compare HR-pQCT and FEA parameters with a healthy control group.

Burden of Disease in Patients With Tumor-Induced Osteomalacia.

JBMR Plus

February 2021

Instituto Diagnóstico e Investigaciones Metabólicas, Universidad del Salvador Buenos Aires Argentina.

Tumor-induced osteomalacia (TIO) is a chronic condition associated with muscle weakness and long-term disability. We conducted a cross-sectional study of patients diagnosed with TIO who had been referred to our institution between May 2018 and December 2019. Our aim was to assess health-related quality of life (HRQoL), fatigue, pain, and muscle mass and strength in these patients.

Cerebral venous thrombosis in Argentina: clinical presentation, predisposing factors, outcomes and  literature review.

J Stroke Cerebrovasc Dis

October 2020

Stroke Outcomes and Decision Neuroscience Research Unit, Department of Medicine, St. Michael's Hospital, University of Toronto, Canada.

Background: Cerebral venous thrombosis (CVT) is a rare medical condition that primarily affects young adults. The clinical spectrum is broad and its recognition remains a challenge for clinicians. Limited information is available on CVT in Argentina.

Introduction & Objectives: Liver cirrhosis is a major cause of mortality worldwide. Adequate diagnosis and treatment of decompensating events requires of both medical skills and updated technical resources. The objectives of this study were to search the demographic profile of hospitalized cirrhotic patients in a group of Latin American hospitals and the availability of expertise/facilities for the diagnosis and therapy of decompensation episodes.

Background: Resuscitation of septic patients regarding goals, monitoring aspects and therapy is highly variable. Our aim was to characterize cardiovascular and fluid management of sepsis in Argentina, a low and middle-income country (LMIC). Furthermore, we sought to test whether the utilization of dynamic tests of fluid responsiveness, as a guide for fluid therapy after initial resuscitation in patients with persistent or recurrent hypoperfusion, was associated with decreased mortality.

Background: Socioeconomic variables impact health outcomes but have rarely been evaluated in critical illness. Low- and middle-income countries bear the highest burden of sepsis and also have significant health inequities. In Argentina, public hospitals serve the poorest segment of the population, while private institutions serve patients with health coverage.

Background: Multiple sclerosis (MS) has a low prevalence in Argentina. However, MS has a high burden of disease, which implies frequent neurological visits, magnetic resonance images (MRI) use and chronic MS medication during follow-up, with a high impact on the healthcare system. Therefore, MS care is essential to optimize disease management and improve patients' outcome.

Objectives: The new Sepsis-3 definitions have been scarcely assessed in low- and middle-income countries; besides, regional information of sepsis outcomes is sparse. Our objective was to evaluate Sepsis-3 definition performance in Argentina.

Design: Cohort study of 3-month duration beginning on July 1, 2016.

Objective: To report the impact of multiple sclerosis (MS) on patients' quality of life (QoL) compared to systemic lupus erythematosus (SLE) using the 36-Item Short Form (SF-36) health questionnaire in Argentina.

Patients And Methods: Cross-sectional study. All consecutive MS patients, SLE and healthy controls (HC) were included.

The purpose of this study was to evaluate the frequency of osteoporosis (OP) in patients with Gaucher disease (GD) in Argentina. GD patients from 28 centers were consecutively included from April 2012 to 2014. Bone mineral density (BMD) was determined by dual X-ray absorptiometry in the lumbar spine and the femoral neck or the total proximal femur for patients ≥20 yr of age, and by whole-body scan in the lumbar spine in patients <20 yr of age.

Patients with Gaucher type 1 (GD1) throughout Argentina were enrolled in the Argentine bone project to evaluate bone disease and its determinants. We focused on presence and predictors of bone lesions (BL) and their relationship to therapeutic goals (TG) with timing and dose of enzyme replacement therapy (ERT). A total of 124 patients on ERT were enrolled in a multi-center study.

División de Pediatría, Escuela de Medicina, Pontificia Universidad Católica de Chile, Santiago, Chile.

Introduction: The birth of a premature baby is a stressful event for parents. The objective of this study was to determine early stress in parents of very low birth weight infants (VLBWIs) hospitalized in 12 neonatal intensive care units from a South American Neonatal Network, to identify associated factors, and to compare the level of parental stress in public versus private healthcare facilities.

Population And Methods: Cross-sectional study in mothers/fathers of VLBWIs (500 to 1500 g).

[Human milk and very low birth weight nutrition].

February 2005

Servicio de Neonatología, Hospital Lagomaggiore, Mendoza, Argentina.

Introduction: Enteral feeding strategies of very low birth weight (VLBW) infants and when to start them have changed significantly in the last few years. Controversy exists on which is the best regimen to feed this high risk group, since human milk has insufficient quantities of some nutrients. Fortification of human milk improves growth rates and maintains immunologic, metabolic and emotional benefits.

A woman experienced severe vaginal bleeding from an exophytic, eroded cervical carcinoma. When vaginal tamponade and blood transfusion were ineffective in stopping the hemorrhage, bilateral laparoscopic ligation of the hypogastric arteries was performed. A Hulka clip was used to collapse the artery walls.

We report a patient with nail and bone disorders of the index and middle fingers (Iso and Kikuchi syndrome) associated with chronic discoid lupus erythematosus. Angiographic studies showed filiform arteries of the fingers and slow blood circulation. Since a vascular pathogenic mechanisms is probable, the designation "Congenital ischemic onychodystrophy" seems more suitable.
