Hidden infections and late diagnoses are currently the main challenges of the HIV pandemic. Emergency departments (EDs) are one of the health care system's key resources addressing these challenges. In 2020, the Spanish Society of Emergency Medicine (SEMES) published recommendations for ordering HIV serology testing for patients with certain health conditions, and in 2021 SEMES launched the "Leave Your Mark" (Deja tu Huella - DTH) program to facilitate implementing the recommendations during emergency care.

Article Synopsis
  • The study investigates how patients with chronic obstructive pulmonary disease (COPD) are managed in emergency departments when they present with acute heart failure (AHF), comparing them to AHF patients without COPD.
  • It analyzes data from 21,694 AHF patients, focusing on demographics, clinical characteristics, and differences in treatment approaches between the two groups over a ten-year period.
  • Findings reveal that COPD patients were younger with more comorbidities, had different triggers and management strategies in emergencies, and exhibited slightly higher in-hospital (8.1%) and 30-day (11.0%) mortality rates compared to non-COPD patients (7.5% and 10.0%, respectively).

Article Synopsis
  • Intravenous digoxin is commonly used in emergency departments to treat acute heart failure, particularly in patients experiencing rapid atrial fibrillation, but concerns about its risks in older patients and those with kidney issues exist.
  • The study aimed to determine if digoxin influences 30-day mortality rates in patients with acute heart failure, taking into account factors like age, kidney function, and potassium levels.
  • The analysis revealed no significant difference in mortality rates between patients treated with digoxin and those who were not, despite a high median age and serious health conditions among participants.

COVID-19 continues to pose a significant threat: mortality stands at nearly twice that of influenza, and the incidence rate is growing as the population's vaccination rate decreases, particularly in Spain and other areas of Europe. Given this situation, it is vitally important know whether medical protocols are consistent and appropriately implemented by health care staff in the interest of preventing possible inefficiency or inequity. Physicians from hospital emergency departments met to study their hospitals' usual clinical practices for managing SARS-CoV-2 infection and to determine their expert opinions on the use of antiviral agents.

Human inborn errors of immunity (IEI) comprise a group of diseases resulting from molecular variants that compromise innate and adaptive immunity. Clinical features of IEI patients are dominated by susceptibility to a spectrum of infectious diseases, as well as autoimmune, autoinflammatory, allergic, and malignant phenotypes that usually appear in childhood, which is when the diagnosis is typically made. However, some IEI patients are identified in adulthood due to symptomatic delay of the disease or other reasons that prevent the request for a molecular study.
