Missed Opportunities for HIV Diagnosis and Their Clinical Repercussions in the Portuguese Population-A Cohort Study.

Pathogens

August 2024

Clínica de Doenças Infecciosas, Faculdade de Medicina, Universidade de Lisboa, Avenida Professor Egas Moniz, 1649-028 Lisboa, Portugal.

Late human immunodeficiency virus (HIV) diagnosis has been associated with missed opportunities for earlier diagnosis. We conducted a retrospective, longitudinal, single-centre cohort study evaluating these missed opportunities and their clinical repercussions in adults with a new HIV diagnosis or who were drug-naïve, who attended our Infectious Diseases Department between 2018 and 2023. We assessed missed opportunities in the two years prior to diagnosis or after the last negative HIV test.

In this study, we employed the dithiothreitol-based protein equalisation technique and analytical proteomics to better understand myeloma diseases by comparing the proteomes of pellets and supernatants formed upon application of DTT on serum samples. The number of unique proteins found in pellets was 252 for healthy individuals and 223 for multiple myeloma patients. The comparison of these proteomes showed 97 dysregulated proteins.

June 2024

Department of Internal Medicine, Ghent University, Ghent, Belgium; Department of Rheumatology, Ghent University Hospital, Ghent, Belgium; Unit for Molecular Immunology and Inflammation, VIB Inflammation Research Center (IRC), Ghent, Belgium. Electronic address:

Background: The early detection and management of (progressive) interstitial lung disease in patients with connective tissue diseases requires the attention and skills of a multidisciplinary team. However, there are currently no well-established standards to guide the daily practice of physicians treating this heterogenous group of diseases.

Research Question: This paper aimed to identify gaps in scientific knowledge along the journey of patients with connective tissue disease-related interstitial lung disease and to provide tools for earlier identification of interstitial lung disease and progressive disease.

July 2024

Copenhagen Center for Arthritis Research (COPECARE), Center for Rheumatology and Spine Diseases, Centre for Head and Orthopedics, Rigshospitalet Glostrup, Glostrup, Denmark.

Objectives: To compare the treatment effectiveness of secukinumab in radiographic (r) versus non-radiographic (nr) axial spondyloarthritis (axSpA) patients treated in routine care across Europe.

Methods: Prospectively collected data on secukinumab-treated axSpA patients with known radiographic status were pooled from nine countries.Remission rates based on patient-reported outcomes (PROs; Numeric Rating Scale (0-10), for example, pain ≤2/Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) ≤2 and Ankylosing Spondylitis Disease Activity Score (ASDAS) inactive disease (ID) <1.

Article Synopsis
  • The study aimed to explore sex differences in patient-reported outcomes (PROMs) among patients with axial spondyloarthritis (axSpA) starting their first tumor necrosis factor inhibitor (TNFi).
  • Data from 15 registries were analyzed, focusing on changes in BASDAI and BASFI scores over 24 months using linear mixed models.
  • Results indicated that women reported significantly worse outcomes than men, with the sex differences in scores nearly doubling after 6 months, and baseline characteristics such as HLA-B27 positivity and disease duration helped explain some but not all of the differences.

Departamento de Patologia, Faculdade de Medicina, Universidade de São Paulo, SP, Brazil.

Article Synopsis
  • Dengue virus (DENV) is a serious global health issue that can cause severe disease affecting various organs, including the kidneys, where immune responses and cell death mechanisms like pyroptosis and necroptosis play key roles.
  • The study focused on analyzing kidney tissue from victims of DENV to identify specific markers related to different cell death pathways (necroptosis, pyroptosis, and autophagy) and to understand tissue changes resulting from the infection.
  • Results showed significant kidney damage, including glomerular congestion and acute injury, with varying expressions of markers for autophagy, pyroptosis, and necroptosis, indicating an intricate

International External Validation of Risk Prediction Model of 90-Day Mortality after Gastrectomy for Cancer Using Machine Learning.

Cancers (Basel)

July 2024

Hospital del Mar Research Institute (IMIM), Section of Gastrointestinal Surgery, Hospital del Mar, Department of Surgery, Universitat Autònoma de Barcelona, 08003 Barcelona, Spain.

Background: Radical gastrectomy remains the main treatment for gastric cancer, despite its high mortality. A clinical predictive model of 90-day mortality (90DM) risk after gastric cancer surgery based on the Spanish EURECCA registry database was developed using a matching learning algorithm. We performed an external validation of this model based on data from an international multicenter cohort of patients.

Aims: To evaluate the prognostic implications of the left atrial reservoir strain-defined diastolic dysfunction (LARS-DD) grade in patients undergoing transcatheter aortic valve implantation (TAVI) for severe aortic stenosis (AS) and to determine whether post-TAVI LARS was more closely associated with new-onset atrial fibrillation than pre-TAVI LARS.

Methods And Results: Pre-TAVI LARS-DD was evaluated by speckle-tracking echocardiography and was assigned as Grade 0 to 1 (LARS ≥24%), Grade 2 (LARS 19-24%), and Grade 3 (LARS <19%). Patients were followed up for the primary endpoint of all-cause mortality from the date of TAVI.

Article Synopsis
  • The study aimed to determine if a simulation-based training course for external cephalic version (ECV) could increase its adoption in hospitals and improve success rates where it's already practiced.
  • Ten Portuguese public maternity hospitals participated, with specialists receiving training. The outcomes measured were the frequency and success rates of ECV performed over two years before and after the training.
  • Results showed that four additional hospitals began performing ECV after training, but no significant improvement in success rates was found in hospitals already doing it, highlighting the need for better access and centralized services for ECV.
