Background: Cardiogenic shock complicating acute myocardial infarction is associated with a high mortality rate. Cardiogenic shock after out-of-hospital cardiac arrest (OHCA) can be due to transient myocardial stunning but also reflect the increasing severity of ongoing heart failure. The Society for Cardiovascular Angiography and Interventions (SCAI) proposed a division of cardiogenic shock into 5 phenotypes, with cardiac arrest being a modifier.

Article Synopsis
  • In managing patients revived from cardiac arrest, a key objective is to ensure enough oxygen delivery (DO) to meet oxygen consumption (VO) needs.
  • The study analyzed two target mean arterial blood pressures (MAP) of 63 mm Hg (MAP63) and 77 mm Hg (MAP77), alongside different arterial oxygen partial pressure (PaO) targets, in comatose patients who suffered out-of-hospital cardiac arrests.
  • Results demonstrated that a MAP of 77 mm Hg increased DO and VO compared to 63 mm Hg, while varying PaO targets didn’t affect DO or VO outcomes.

Third-Trimester Cardiovascular Function and Risk of Hypertensive Disorders of Pregnancy.

J Am Heart Assoc

October 2024

Department of Epidemiology Research Statens Serum Institut Copenhagen Denmark.

Article Synopsis
  • The study investigates the link between cardiac output (CO) and hypertension disorders during pregnancy, specifically preeclampsia, in women with and without fetal growth restriction (FGR).
  • In a cohort of 906 pregnant women, findings suggest that low CO is a significant risk factor for preeclampsia, especially in those below 37 weeks of gestation, with women having FGR being at an even higher risk.
  • The research highlights the potential importance of monitoring third-trimester cardiovascular function to identify women at risk for preeclampsia, although more studies are needed to confirm these findings.

Increasing recognition of germline variants in patients with hematological malignancies prompted us to provide -specific recommendations for diagnosis, surveillance, and treatment. Causative germline variants in the predispose to the development of myeloid neoplasms (MNs), especially myelodysplastic syndrome (MDS) and acute myeloid leukemia (AML). Almost 3%-5% of all patients with MDS or AML carry a pathogenic or likely pathogenic germline variant, while half of them acquire a somatic second hit in the other allele.

Article Synopsis
  • Advancements in understanding myelodysplastic neoplasms (MDS) have revealed important cellular and molecular factors that influence disease progression, highlighting the significance of immune dysregulation in the bone marrow during MDS evolution.
  • Despite these advancements, immunotherapy for MDS has lagged due to a lack of effective immune classifications for patient stratification and no widely accepted immune panels for clinical use.
  • To address these challenges, the i4MDS consortium proposes standardized immune monitoring approaches, including flow cytometry panels and cytokine assays, aiming to improve patient stratification and develop predictive markers for treatment response in MDS.

Background: Patients with out-of-hospital cardiac arrest (OHCA) in rural areas experience longer emergency response times and have lower survival rates compared with patients in urban areas. Volunteer responders might improve care and outcomes for patients with OHCA specifically in rural areas. Therefore, we investigated volunteer responder interventions based on the degree of urbanization.

Background High-quality cardiopulmonary resuscitation (CPR) is associated with improved survival from out-of-hospital cardiac arrest and includes chest compression depth, chest compression rate, and chest compression fraction within international guideline recommendations. Previous studies have demonstrated divergent results of real-time feedback on CPR performance and patient outcomes. This study investigated the association between emergency medical service CPR quality and real-time CPR feedback for out-of-hospital cardiac arrest.

Background GLP-1 (glucagon-like peptide-1) receptor agonists exert beneficial long-term effects on cardiovascular and renal outcomes. In humans, the natriuretic effect of GLP-1 depends on GLP-1 receptor interaction, is accompanied by suppression of angiotensin II, and is independent of changes in renal plasma flow. In rodents, angiotensin II constricts vasa recta and lowers medullary perfusion.
