Background And Objective: To evaluate the effectiveness and safety of intradialytic parenteral nutrition (IDPN) on different nutritional outcomes.

Methods: This was a retrospective analysis for a "routinely collected data bank" in a multicenter cohort, conducted on consecutive malnourished or at-risk of malnutrition patients with chronic kidney disease on hemodialysis who underwent IDPN with a three-in-one parenteral nutrition formula for a period ≥ 2 weeks. The primary endpoint was the mean change in the malnutrition inflammation score (MIS) score between baseline and the last follow-up visit on IDPN.

Background/objectives: A specialized microenvironment in the bone marrow, composed of stromal cells including mesenchymal stem cells (MSCs), supports hematopoietic stem cell (HSC) self-renewal, and differentiation bands play an important role in leukemia development and progression. The reciprocal direct interaction between MSCs and CD34 HSCs under physiological and pathological conditions is yet to be fully characterized.

Methods: Here, we established a direct co-culture model between MSCs and CD34 HSCs or MSCs and acute myeloid leukemia cells (THP-1, Molm-13, and primary cells from patients) to study heterocellular communication.

Liver transplantation (LT) has been accepted as a cornerstone of care in hepatocellular carcinoma (HCC) for almost three decades. In recent years, its role has been evolving to include patients with disease burden beyond the widely used Milan criteria. The integration of dynamic biomarkers such as alpha-fetoprotein together with downstaging approaches and tumor evolution after enlistment has allowed the selection of patients most likely to benefit, resulting in 5-year survival rates greater that 70%.

Global airway disease: mepolizumab simultaneously improves outcomes in severe CRSwNP and asthma.

Rhinology

December 2024

Otorhinolaryngology, Catholic University of Leuven, and Upper Airways Research Laboratory, University of Ghent, Belgium.

Chronic rhinosinusitis with nasal polyps (CRSwNP) often co-exists with asthma and non-steroidal anti-inflammatory drug- exacerbated respiratory disease (N-ERD), creating a more severe phenotype and an additional burden compared with CRSwNP disease alone (1-3). The relationship between these diseases in terms of shared immunological disbalance has been coined in the literature as 'global airway disease' or 'unified airway disease' and requires integrated treatment strategies (4-6). Our post hoc analysis of the Phase III randomised, double-blind, placebo-controlled, multicentre SYNAPSE study (GSK ID: 205687; NCT03085797 (7)) assessed the efficacy of mepolizumab, an anti-interleukin-5 monoclonal antibody, in simultaneously improving both CRSwNP and asthma outcomes versus placebo.

We present the case of a 22-year-old male with a left adrenal pheochromocytoma, initially diagnosed during a workup for thoracic pain. The patient's tumor was refractory to medical management, and surgical resection was ruled out due to high cardiovascular risk, stemming from cyanotic congenital heart disease, aortic aneurysm, and factor VII deficiency. The patient underwent adrenal artery embolization (AAE) as a salvage treatment.

Background: While the high accuracy of reported AI tools for melanoma detection is promising, the lack of holistic consideration of the patient is often criticized. Along with medical history, a dermatologist would also consider intra-patient nevi patterns, such that nevi that are different from others on a given patient are treated with suspicion.

Objective: To evaluate whether patient-contextual lesion-images improves diagnostic accuracy for melanoma in a dermoscopic image-based AI competition and a human reader study.

Article Synopsis
  • Idiopathic REM sleep behavior disorder (iRBD) is linked to alpha-synucleinopathies, and the study aimed to evaluate autonomic nervous system (ANS) dysfunction in individuals with iRBD and its potential to predict clinical progression.
  • In a study of 25 iRBD patients, 28% developed synucleinopathy, primarily Parkinson's disease, with many exhibiting abnormal ANS test results; those who progressed had significantly higher rates of dysfunction compared to those who did not.
  • Results indicate that both symptomatic and subtle ANS dysfunctions are prevalent in iRBD, and specific adrenergic system issues may signal a higher short-term risk of developing synucleinopathy.

Introduction: Line-field confocal optical coherence tomography (LC-OCT) represents one of the newest non-invasive in vivo skin imaging techniques. Previous studies described morphologic criteria of basal cell carcinoma (BCC) under LC-OCT examination. Preliminary data on LC-OCT diagnostic performance for BCC have recently been published but showed only a modest improvement compared to dermoscopy, possibly due to study limitations.

Objetives: Patients with digitalis intoxication (DI) and hyperkalaemia are frequently encountered in the emergency department (ED). This alteration may require intravenous (iv) calcium, but its administration has been considered to increase cardiotoxicity and mortality in patients with DI. We studied the effect of iv calcium on mortality and 30-day readmission in patients with hyperkalaemia and DI.

Article Synopsis
  • The study investigates the transorbital approach (TO) for accessing vascular lesions, specifically the carotid and middle cerebral arteries, using cadaveric specimens to understand its feasibility for vascular clipping.
  • Dissection involved a stepwise TO method that included procedures like lateral orbital craniectomy and anterior clinoidectomy, allowing for exposure of various vascular segments and cranial nerves.
  • Findings suggest that the TO approach effectively exposes key artery segments, indicating its potential use in treating paraclinoid and posterior communicating aneurysms.

Article Synopsis
  • Researchers investigated the relationship between the integrity of deep medullary veins and markers of cerebral small vessel disease in patients who had experienced a recent small subcortical infarct.
  • They collected data from 50 patients using 3 Tesla-MRI imaging, examining various factors such as the presence of microbleeds, white matter hyperintensities, and diffusivity measures to assess glymphatic function.
  • Results indicated that several imaging variables correlated with venular integrity and small vessel disease markers, with specific measures of diffusivity showing significant associations related to the appearance of the veins.

Article Synopsis
  • Surgical site infections (SSI) are a common issue following surgeries, particularly in incisional hernia repairs, and the effectiveness of topical antibiotics like gentamicin for prevention remains uncertain.
  • A randomized controlled trial was conducted with 146 patients, comparing outcomes between those given a gentamicin solution and those given saline solution during surgery.
  • The study found no significant differences in SSI rates or other complications between the two groups, indicating that topical gentamicin did not effectively reduce SSI incidence in hernia repair surgeries.

Acute myeloid leukemia (AML) with translocation t(8;16)(p11;p13) represents a rare entity that has been categorized as a disease-defining recurring cytogenetic abnormality with adverse risk in the 2022 European LeukemiaNet classification. This rating was mainly based on a retrospective analysis comprising patients from several large clinical trials, which, however, included only 21 patients treated with allogeneic stem cell transplantation (alloSCT). Therefore, the European Society for Blood and Marrow Transplantation performed a registry study on a larger cohort to evaluate the role of alloSCT in t(8;16) AML.

Incidence and prognosis of cutaneous melanoma in European adolescents and young adults (AYAs): EUROCARE-6 retrospective cohort results.

Eur J Cancer

December 2024

Evaluative Epidemiology Unit, Department of Epidemiology and Data Science, Fondazione IRCCS Istituto Nazionale dei Tumori di Milano, Milan, Italy.

Background: Cutaneous melanoma (CM) is rare in adolescents and young adults (AYA, 15-39 years at cancer diagnosis) and studies on CM in AYAs are scarce. Our aim is to update CM incidence and survival in European AYAs and to compare incidence and survival both with other age groups and over time.

Methods: We used the EUROCARE-6 database (108 cancer registries; 29 EU countries), calculating incidence rates (IR) per 100,000 individuals/year in the European population (years of diagnosis: 2006-2013), 5-year relative survival (RS), and 5-year RS conditional to surviving the first year after diagnosis, for the follow-up period 2010-2014 (cases diagnosed in 2006-2013).
