This study investigates the relationship between SARS-CoV-2 RT-PCR cycle threshold (Ct) values and key COVID-19 transmission and outcome metrics across five years of the pandemic in Jalisco, Mexico. Utilizing a comprehensive time-series analysis, we evaluated weekly median Ct values as proxies for viral load and their temporal associations with positivity rates, reproduction numbers (Rt), hospitalizations, and mortality. Cross-correlation and lagged regression analyses revealed significant lead-lag relationships, with declining Ct values consistently preceding surges in positivity rates and hospitalizations, particularly during the early phases of the pandemic.

Interleukin-10 (IL-10) is an immunomodulatory molecule that may play an immunosuppressive role in nonmelanoma skin cancer (NMSC), specifically basal cell carcinoma (BCC). Shoulder injuries, specifically proximal humeral fractures, are uncommon in skeletally immature patients. The anatomic characteristics of the humerus are determined by its ossification development, which is quite particular in the proximal segment where four principal segments have been outlined as fracture components in adults. Hereby, we present the case of an 18-year-old female who suffered a traffic accident that elicited a proximal humeral injury through the physeal line of the anatomical neck, as well as the treatment given and her clinical outcome with an effort to highlight the complexity of the diagnosis and management of this injury given the transitional bone age.

People Living with HIV (PLHIV) experience accelerated aging, yet strategies for healthy aging in this group are not well studied. Although survival rates have improved, non-infectious comorbidities, like cardiovascular diseases, diabetes, and cancers, are increasing and tend to appear earlier and more severely in PLHIV frailty, defined as increased vulnerability to stressors, which is a growing concern among aging PLHIV, driven by factors, like chronic inflammation, antiretroviral therapy toxicity, and traditional risk factors. The relationship between diabetes and the development of kidney complications is well known, but the understanding of prediabetes and insulin resistance with impaired kidney function has been scarcely assessed. Various factors could explain this phenomenon, from the lack of standardization in the definitions of prediabetes, to the erratic and inconsistent evidence in large-scale epidemiological and cohort studies. It seems that the pathophysiological pathway of prediabetes could be related to inflammation and neurohormonal hyperactivation, factors present even before the onset of diabetes, which might be the main drivers of glomerular hyperfiltration, albuminuria, and impaired glomerular filtration rate.
