Article Synopsis
  • The 2023 monkeypox epidemic originated from a specific lineage of the monkeypox virus traced back to Nigeria in 1971, with a subclade IIb showing higher person-to-person transmission rates due to genomic changes.
  • Researchers used advanced techniques to analyze the genome of the monkeypox virus from the current epidemic, highlighting significant variations in low-complexity regions (LCRs) that are often overlooked.
  • The study found that variations in LCRs may influence the expression and function of key poxvirus genes, suggesting that future genome studies should focus on these regions to better understand differences in the virus's behavior.

  • This study compares two surgical methods, Hartmann's procedure (HP) and resection with primary anastomosis (RPA), for treating acute left-sided colonic emergencies among 1215 patients from 204 centers globally.
  • Results showed that while HP was the more common treatment (57.3%), RPA was favored for younger patients with fewer health issues and those needing surgery sooner.
  • The study concluded that although HP is still widely used, RPA might be the better option, emphasizing the importance of patient characteristics and surgeon experience in determining treatment choice.

Management of hypertension in heart transplant recipients: an ongoing conundrum.

Curr Opin Cardiol

July 2024

Section of Cardiomyopathy & Heart Transplantation, John Ochsner Heart and Vascular Institute, Ochsner Clinic Foundation.

Purpose Of Review: Hypertension remains one of the most common clinical problems leading to significant posttransplant complications. This study reviews the pathophysiology of hypertension in the postcardiac transplant phase and provides an update on currently available antihypertensive therapies for heart transplant patients.

Recent Findings: The true prevalence of hypertension in the heart transplant population remains unknown.

The prevalence of cardiovascular diseases (CVDs) is a growing global health concern. Recent advances have demonstrated significant reductions in acute cardiovascular events through the management of modifiable cardiovascular risk factors. However, these factors are responsible for about 50% of the global cardiovascular disease burden.

  • A national study in Spain examined the mpox outbreak, focusing on the clinical profiles and management strategies in place.
  • Around 1,472 patients were analyzed, predominantly cisgender men, with sexual transmission as the main route and a significant population having HIV-related immunosuppression.
  • Most patients displayed common symptoms like rash and fever, with one-third receiving no treatment, while others were treated with various therapies, indicating a need for better preventive measures and healthcare responses.

Article Synopsis
  • The study investigates the CD4/CD8 ratio as a possible marker for mortality risk in people living with HIV, highlighting its potential significance compared to CD4 counts alone.
  • A systematic review and meta-analysis were conducted, analyzing data from 20 studies, revealing that a low CD4/CD8 ratio (below 0.5) correlates with a significantly higher mortality risk.
  • While the analysis couldn't confirm a clear trend regarding CD8+ T-cell counts, it suggests that high counts may negatively affect long-term outcomes, emphasizing the need for standardized measurement protocols in future research.*

Numerous international organizations, including the World Health Organization, have been drawing attention to the global increase in sexually transmitted infections. Twenty years ago, lymphogranuloma venereum (LGV) was mainly considered a tropical disease; in recent decades, however, LGV has been increasingly present in high-income countries. This increase has been linked to men who have sex with men who participate in highly interconnected sexual networks, leading to a rapid spread of LGV.

Contrast-enhanced mammography (CEM) involves addition of intravenous iodinated contrast material at digital mammography, thus increasing the ability to detect breast cancer owing to tumor contrast enhancement. After image acquisition, interpretation includes careful assessment of the technique, artifacts, and pitfalls and reporting with a standard lexicon category and appropriate follow-up recommendations. Artifacts and pitfalls that may cause image misinterpretation should be detected and distinguished from pathologic conditions.
