Prevalence of Bacterial Codetection and Outcomes for Infants Intubated for Respiratory Infections.

Pediatr Crit Care Med

July 2024

Red Colaborativa Pediátrica de Latinoamérica (LARed Network), Montevideo, Uruguay.

Article Synopsis
  • The study aimed to find out how common respiratory bacterial codetection is in children under 2 years old who are intubated for acute lower respiratory tract infections, mainly viral bronchiolitis, and to see how it affects the duration of mechanical ventilation.
  • It analyzed 472 children, finding that 29% had bacterial codetection, with younger age and positive RSV tests linked to higher rates, while codetection didn't show a clear connection to longer mechanical ventilation duration.
  • The research suggests that while bacterial codetection is common, it may not significantly impact the length of time patients need invasive mechanical ventilation.

Cardiac amyloidosis (CA) is a form of cardiomyopathy characterized by the extracellular deposit of protein fibers in the myocardium, leading to the development of heart failure, arrhythmias, and electrical conduction system alterations. It is known that most cardiomyopathies have a close relationship with heart rhythm abnormalities, however, CA is specially related to different kinds of arrhythmias even in pre-diagnosis stages. Arrhythmias like atrial fibrillation are present in up to 70% of patients with CA associated with a high risk of cardioembolic complications independent of the risk stratification.

Pneumobilia is a phenomenon associated with the presence of a biliary-enteric fistula or manipulation of the bile duct during procedures or surgical interventions that cause dysfunction of the sphincter of Oddi. A known, but infrequently reported event, is the increase in intraabdominal pressure after closed abdominal trauma, which causes pneumobilia due to a mechanism of retrograde air leakage towards the bile duct. Depending on the general compromise of each patient, the prognosis can vary from a benign condition that only requires conservative management, to being life threatening.
