Colorectal cancer is one of the most prevalent forms of cancer globally. The most common routine diagnostic methods are the examination of the interior of the colon during colonoscopy or sigmoidoscopy, which frequently includes the removal of a biopsy sample. Optical methods, such as Raman spectroscopy (RS) and optical coherence tomography (OCT), can help to improve diagnostics and reduce the number of unnecessary biopsies.

Article Synopsis
  • Oncological patients experience higher risks of infections due to weakened immune systems from diseases or treatments, yet vaccination rates against respiratory pathogens like Streptococcus pneumoniae and influenza are low.
  • In a study analyzing 370 oncology patients, only 28.1% were vaccinated against influenza and 32.2% against Streptococcus pneumoniae, with just 7.3% meeting German vaccination recommendations.
  • The findings highlight a significant gap in vaccination coverage, particularly among patients with thoracic cancers, indicating the need for improved vaccination strategies for this high-risk group.

Postoperative Harris Hip Score Versus Harris Hip Score Difference in Hip Replacement: What to Report?

Orthop Surg

January 2025

Center of Orthopaedics and Traumatology, University Hospital Brandenburg an der Havel, Brandenburg Medical School Theodor Fontane, Brandenburg an der Havel, Germany.

Background: Reliable scientific information is crucial for assessing hip function and evaluating the success of hip surgery. The Harris Hip Score (HHS) is the most widely used tool for measuring hip function and, in particular, the outcomes of hip surgery. The aim of this study was to conduct a systematic review of the literature to identify randomized controlled trials (RCTs) that reported the HHS for hip replacement treatment groups and to test whether there was a substantial difference between reporting only the postoperative HHS or the HHS difference (HHSdiff).

Purpose: Unicondylar arthroplasty was performed using robotic medial unicompartmental knee arthroplasty (R-mUKA) and gap-balancing instrumentation. Our hypothesis was that robotic unicondylar knee arthroplasty accurately restores component positioning and lower limb alignment when compared to preoperative planning with actual implantation throughout the range of knee motion due to proper knee balancing.

Methods: Data were collected prospectively and were analysed for patients undergoing RM-UKA.

Article Synopsis
  • The study investigates the effectiveness of artificial intelligence (AI) in radiological assessments for femoral neck fractures through a systematic review and meta-analysis of existing research.
  • It analyzed data from 11 studies involving over 21,000 radiographs, finding that AI achieved an accuracy of 91% and good sensitivity of 87% in assessing these fractures.
  • The research followed established protocols to ensure quality, revealing moderate overall study quality and some publication bias, while highlighting the potential benefits of AI in radiology.

Introduction: Polypropylene material is commonly used for posterior wall reconstruction in hernia repair, in contrast with the classically described stainless-steel wire used at Shouldice Hospital. This study was conducted to evaluate possible differences in Shouldice Repair outcomes using polypropylene or stainless-steel wire sutures.

Methods: A prospective follow-up of consecutive patients who underwent elective unilateral Shouldice primary inguinal hernia repair at Shouldice Hospital between December 6, 2021, and September 1, 2022, was conducted.

Purpose: There is limited research on the impact of an ilioinguinal-iliohypogastric nerve block on intraoperative opioid consumption when conducting groin hernia repair in adults. Thus, the aim was to evaluate ilioinguinal-iliohypogastric nerve block for groin hernia patients at Shouldice Hospital.

Methods: The study was a pilot retrospective chart review on patients who underwent a Shouldice Repair from November 2023 to December 2023.

Purpose: The aim of this study was to conduct a systematic review and meta-analysis on the reliability and applicability of artificial intelligence (AI)-based analysis of leg axis parameters. We hypothesized that AI-based leg axis measurements would be less time-consuming and as accurate as those performed by human raters.

Methods: The study protocol was registered with the International Prospective Register of Systematic Reviews (PROSPERO).

Objective: To compare the survival of palliative stage IV colorectal cancer patients selected for primary tumor resection and systemic treatment (PTR+SYST) to patients with systemic treatment only (SYST).

Background: About 20-25% of all colorectal cancer patients are diagnosed with stage IV disease. The benefit of primary tumor resection in the palliative situation is therefore of high concern.

Assessment of Cirrhotic Patients by the EncephalApp Fails to Predict Low-Grade Hepatic Encephalopathy.

Dig Dis

December 2024

Department of Gastroenterology, Hepatology and Diabetology, Center of Internal Medicine II, University Hospital Brandenburg an der Havel, Brandenburg an der Havel, Germany.

Introduction: An early detection of low-grade hepatic encephalopathy (HE) is of high importance. The aim of the study was to compare a neuropsychological with a psychophysical test on the basis of the psychometric hepatic encephalopathy score (PHES) regarding effectiveness in diagnosing minimal HE (MHE).

Methods: In our prospective controlled observational study, we examined a total of 103 patients with liver cirrhosis for HE.

Unlabelled: Artificial intelligence's (AI) accelerating progress demands rigorous evaluation standards to ensure safe, effective integration into healthcare's high-stakes decisions. As AI increasingly enables prediction, analysis and judgement capabilities relevant to medicine, proper evaluation and interpretation are indispensable. Erroneous AI could endanger patients; thus, developing, validating and deploying medical AI demands adhering to strict, transparent standards centred on safety, ethics and responsible oversight.

Background: A higher prevalence of cardiovascular risk factors has previously been shown to be associated with adverse social determinants of health (SDoH) and to explain some of their impact on cardiovascular risk. Whether there is a relationship between lipid parameters, specifically apolipoprotein B (apoB), apolipoprotein A1 (apoA1), their ratio (apoB/apoA1), and SDoH, and whether coronary artery disease (CAD) mortality risk associated with circulating apoB and apoA1 is modified by SDoH was unclear.

Methods: We investigated associations of apoA1, apoB, and apoB/apoA1 with the level of education and household income and their joint impact on CAD mortality in participants of the UK Biobank (UKB) with and without prevalent CAD at enrollment.

Shouldice Hospital has grown beyond its beginnings in the 1940s to become a leading centre of excellence for mesh-free hernia repair. Little is known to the international surgical community about the preoperative and postoperative care at Shouldice Hospital, and colleagues working at Shouldice Hospital have been repeatedly asked to provide more details at international and national congresses. Therefore, this article aims to summarize preoperative and postoperative care at Shouldice Hospital based on previously published literature.

Understanding the ant's unique biting system can improve surgical needle holders.

Proc Natl Acad Sci U S A

February 2024

Department of Pediatric Surgery, Theodor Fontane Medical School, University Hospital Brandenburg an der Havel, 14770 Brandenburg, Germany.

Mechanical grasping and holding devices depend upon a firm and controlled grip. The possibility to improve this gripping performance is severely limited by the need for miniaturization in many applications, such as robotics, microassembly, or surgery. In this paper, we show how this gripping can be improved in one application (the endoscopic needle holder) by understanding and imitating the design principles that evolution has selected to make the mandibles of an ant a powerful natural gripping device.
