Griffith University, School of Nursing and Midwifery, Gold Coast Campus, Parklands Drive, Southport, Queensland, 4222, Australia; Gold Coast University Hospital, 1 Hospital Blvd, Southport, Queensland, 4215, Australia. Electronic address:

Background: Laws that regulate healthcare practice at the end of life reflect the values of the society where they apply. Traditionally, healthcare professionals rely on their clinical knowledge to inform treatment decisions, but the extent to which the law also informs health professionals' decision-making at the end of life is uncertain.

Objective: The objective of this study was to describe what healthcare professionals working in emergency departments and intensive care units know about the law that relates to end-of-life decision-making for hospitalised adults and what affects its application.

Abiotrophia defectiva or nutritionally variant streptococci (NVS) was first described in 1961 as a rare cause of infectious endocarditis (IE). It is distinct to the Streptococcus genus, pleomorphic, and demonstrates variable Gram-staining characteristics due to poor growth in culture media requiring extended incubation or 16S rRNA sequencing in culture negative situations. NVS has also been implicated in the pathogenesis of culture-negative endocarditis.

An unusual case of abdominal compartment syndrome from a massive faecaloma.

J Surg Case Rep

August 2022

General Surgical Department, Gold Coast University Hospital, 1 Hospital Blvd, Southport, QLD, Australia.

Severe constipation is a frequent presentation but progression into a life-threatening acute compartment syndrome (ACS) is few and far between. This case highlights the typical physiological manifestations of ACS and the immediate benefits of correcting these physiological imbalances through the disimpaction of a massive faecaloma. Furthermore, in adult patients with a history of colonic dysmotility, adult Hirschsprung's disease should be considered.

Spontaneous hyphema from iris microhemangioma in Eisenmenger syndrome.

Am J Ophthalmol Case Rep

June 2022

Gold Coast University Hospital, 1 Hospital Blvd, Southport, QLD, 4215, Australia.

Purpose: We describe a patient with Eisenmenger syndrome and spontaneous hyphema from iris microhemangioma, two rare entities with a plausible pathophysiological connection.

Observations: A 56-year-old Caucasian female with a background of cyanotic congenital heart disease complicated by Eisenmenger syndrome presented with non-traumatic hyphema and blurred vision. Multiple vascular tufts consistent with iris microhemangiomas were observed around the pupil margins bilaterally, with no iris or retinal neovascularization.

Background: Increasing numbers of older adults with complex health deficits presenting to emergency departments has prompted the development of innovative models of care. One such model designed to reduce poor outcomes associated with acute healthcare, is the Geriatric Emergency Department Intervention. This intervention is a nurse-led, physician-championed, Emergency Department intervention that improves the health outcomes for frail older adults in the emergency department.

Electronic address:

Background: Early mobilisation is beneficial to minimise complications and promote the recovery and physical function of critically ill adult patients. An interprofessional team approach may assist in early mobilisation of these patients; however, adopting this approach may be challenging and optimal strategies to support early mobilisation are unknown. Understanding specific implementation strategies is required to effectively support its implementation in the critically ill adult patient population.

Rituximab rescue therapy for autoimmune pulmonary alveolar proteinosis.

Respir Med Case Rep

March 2022

Department of Respiratory Medicine, Gold Coast University Hospital, Hospital Blvd, Southport, Queensland, Australia, 4215.

Autoimmune pulmonary alveolar proteinosis (aPAP) is a rare lung disease characterised by abnormal alveolar surfactant accumulation due to macrophage dysfunction. Whole lung lavage (WLL) is the cornerstone of first-line aPAP therapy, but effective rescue treatments have not yet been well established. We report a case of a 41-year-old man with aPAP in whom further WLL is contraindicated.

Most prior studies have compared myocardial infarction with non-obstructive coronary arteries (MINOCA), to obstructive acute coronary syndrome (ACS) often requiring revascularisation. However, these were subject to treatment bias given the significant differences in management. This study uniquely compares the management and outcomes of MINOCA patients with a medically managed obstructive ACS (M-ACS) population.

Efficacy and cost-effectiveness of universal pre-operative iron studies in total hip and knee arthroplasty.

J Orthop Surg Res

August 2021

Adelaide Health Technology Assessment (AHTA), School of Public Health, University of Adelaide, Adelaide, Australia.

Background: The prevalence of anaemia in patients planned for total hip and knee arthroplasty is about 20%. Optimising pre-operative haemoglobin levels by iron supplementation has been shown to decrease transfusion rates, complications and associated morbidity. The need for universal screening with iron studies of all elective arthroplasty patients is not clearly defined at present.

Such opposing therapies can result in unexpected pathophysiology and there is a real risk of misdiagnosis resulting in harm. This case presents a previously unreported complication of prevention and management of thromboembolism- STEMI (ST elevation myocardial infarction) and tamponade mimic secondary to retroperitoneal haematoma.

Purpose: Rotational thromboelastometry (ROTEM®) allows guided blood product resuscitation to correct trauma-induced coagulopathy in bleeding trauma patients. FIBTEM amplitude at 10 min (A10) has been widely used to identify hypofibrinogenaemia; locally a threshold of < 11 mm has guided fibrinogen replacement. Amplitude at 5 min (A5) carries an inherent time advantage.
