A retrospective review over 6 yr of patients presenting to the hand clinic was performed to identify cases of postoperative brachial plexopathy (PBP) and to assess both prognosis and early indices of prognosis. Over this period (1989-1995), 22 patients were referred by the hospital's surgical departments to the hand clinic because of PBP. Unlabelled: Recent concern regarding lidocaine neurotoxicity has prompted efforts to find alternatives to lidocaine spinal anesthesia. Small-dose dilute bupivacaine spinal anesthesia yields a comparably rapid recovery profile but may provide insufficient anesthesia. The impact of wearing gloves on surface contamination and on the incidence of percutaneous injury were prospectively compared for two 4-mo periods in a single anesthesia department. Period I was immediately prior to the institution of mandatory glove wearing, and Period II followed a 1-mo adjustment period of mandatory glove use. Objective: To demonstrate the benefit of cooperation between medical and chiropractic specialists and the usefulness of combining chiropractic and epidural injection in particular cases of back pain.

Clinical Features: Two cases of low back pain, both with disk protrusions and osteophytic changes, had no response to several weeks of alternate day (or more) chiropractic care. A third case of herniated intervertebral disks and low back pain had no improvement from treatment with epidural steroidal injection.
