Article Synopsis
  • Biallelic pathogenic variants in the NTHL1 gene are linked to a hereditary cancer syndrome, increasing risks for adenomatous polyposis and colorectal cancer, as well as other tumors like breast and brain cancers.
  • The study, using data from the French oncogenetic consortium, describes 10 patients with these variants, identifying them as the second-largest series on NTHL1, all of whom showed signs of adenomatous polyps.
  • The findings suggest that testing for NTHL1 should be included in diagnostic panels for hereditary cancers, with recommendations for colon and extra-colonic cancer surveillance based on existing guidelines.

Adaptive processes in chronic bacterial infections are well described, but much less is known about the processes at play during acute infections. Here, by sequencing seven randomly selected isolates per patient, we analyzed populations from three acute extraintestinal infections in adults (meningitis, pyelonephritis, and peritonitis), in which a high-mutation-rate isolate or mutator isolate was found. The isolates of single patients displayed between a few dozen and more than 200 independent mutations, with up to half being specific to the mutator isolate.

Sirolimus for treatment of patients with inclusion body myositis: a randomised, double-blind, placebo-controlled, proof-of-concept, phase 2b trial.

Lancet Rheumatol

January 2021

Department of Internal Medicine and Clinical Immunology, Neuromuscular Rare Diseases Reference Center of Paris, Sorbonne Université, INSERM U974, Assistance Publique-Hôpitaux de Paris (AP-HP), Pitié-Salpêtrière University Hospital, Paris, France.

Background: Inclusion body myositis is the most frequent myositis in patients older than 50 years. Classical immunosuppressants are ineffective in treating inclusion body myositis, and to date there are no recommendations for pharmacological approaches to treatment. When used after organ transplantation, sirolimus can block the proliferation of effector T cells, while preserving T regulatory cells, and induce autophagy, all of which are processes that are impaired in inclusion body myositis.

Lipopolysaccharide (LPS) is a key player for innate immunity activation. It is therefore a prime target for sepsis treatment, as antibiotics are not sufficient to improve outcome during septic shock. An extracorporeal removal method by polymyxin (PMX) B direct hemoperfusion (PMX-DHP) is used in Japan, but recent trials failed to show a significant lowering of circulating LPS levels after PMX-DHP therapy.
