The ocular microbiome and microbiota and their effects on ocular surface pathophysiology and disorders.

Surv Ophthalmol

March 2022

Ophthalmology Départment, Hôpitaux Universitaires Paris-Sud, APHP, Université Paris-Saclay, IDMIT Infrastructure, Fontenay-aux-Roses Cedex, France.

The ocular surface flora perform an important role in the defense mechanisms of the ocular surface system. Its regulation of the immunological activity and the barrier effect against pathogen invasion are remarkable. Composition of the flora differs according to the methods of investigation, because the microbiome, composed of the genetic material of bacteria, fungi, viruses, protozoa, and eukaryotes on the ocular surface, differs from the microbiota, which are the community of microorganisms that colonize the ocular surface.

In critically ill and high-risk surgical room patients, an invasive arterial catheter is often inserted to continuously measure arterial pressure (AP). The arterial waveform pressure measurement, however, may be compromised by damping or inappropriate reference placement of the pressure transducer. Clinicians, decision support systems, or closed-loop applications that rely on such information would benefit from the ability to detect error from the waveform alone.

Immune checkpoint inhibitors for progressive multifocal leukoencephalopathy: a new gold standard?

J Neurol

July 2021

Sorbonne Université, AP-HP, Hôpitaux Universitaires Pitié-Salpêtrière Charles Foix, Institut Pierre Louis d'Épidémiologie et de Santé Publique, Service de Maladies Infectieuses et Tropicales, INSERM 1136, 47-83 Bd de l'Hôpital, 75651, Paris Cedex, France.

Objectives: Progressive multifocal leukoencephalopathy (PML) is a very rare and opportunistic encephalitis caused by JC polyomavirus that is linked to profound immunosuppression and is usually fatal unless immune function can be restored. Immune checkpoint inhibitors (ICI) are monoclonal antibodies (mAbs) that block either CTLA-4 or PD-1 inhibitor receptors, thus enhancing antiviral T-cell activity. Successful treatment of PML by ICI has recently generated some enthusiasm in case reports/small series of patients.

Background: Several methods have been proposed to measure cerebrovascular autoregulation (CA) in traumatic brain injury (TBI), but the lack of a gold standard and the absence of prospective clinical data on risks, impact on care and outcomes of implementation of CA-guided management lead to uncertainty.

Aim: To formulate statements using a Delphi consensus approach employing a group of expert clinicians, that reflect current knowledge of CA, aspects that can be implemented in TBI management and CA research priorities.

Methods: A group of 25 international academic experts with clinical expertise in the management of adult severe TBI patients participated in this consensus process.

Article Synopsis
  • - CBCT has been used in dentistry since the late 1990s, but EU laws require better patient protection and quality checks that aren't consistently applied across countries.
  • - A survey by EURADOS aimed to gather info on CBCT practices from 22 EU countries, resulting in 28 responses that revealed inconsistencies in the exposure factors and patient doses.
  • - The findings show that patient doses aren't well optimized and DRLs are often nonexistent, highlighting the need for improved QC testing and establishment of DRLs to ensure safer imaging practices.

Background: Acute kidney injury (AKI) occurs frequently after liver transplant surgery and is associated with significant morbidity and mortality. While the impact of intraoperative hypotension (IOH) on postoperative AKI has been well demonstrated in patients undergoing a wide variety of non-cardiac surgeries, it remains poorly studied in liver transplant surgery. We tested the hypothesis that IOH is associated with AKI following liver transplant surgery.
