Epidemiological and clinicopathological characteristics of vascular-limited renal AL amyloidosis.

Nephrol Dial Transplant

December 2024

Department of Nephrology, APHP Hôpital Bichat, 46 Rue Henri Huchard, Paris, INSERM U1149 centre de recherche sur l'inflammation, Université Paris Cité.

Background And Hypothesis: Kidney involvement, along with cardiac disease, is the most frequent manifestation of systemic AL amyloidosis usually resulting in nephrotic-range proteinuria. Rarely, deposits predominantly or exclusively affect the intrarenal arterioles or arteries, these vascular-limited forms following a distinct clinical course, but very little is known about these forms. Our work plan at better characterizing renal vascular limited AL amyloidosis.

Laboratoire de Chimie de la Matière Condensée de Paris (LCMCP), Sorbonne Université, CNRS, Collège de France, Paris, France.

Autologous bone (AB) is the gold standard for bone-replacement surgeries, despite its limited availability and the need for an extra surgical site. Traditionally, competitive biomaterials for bone repair have focused on mimicking the mineral aspect of bone, as evidenced by the widespread clinical use of bioactive ceramics. However, AB also exhibits hierarchical organic structures that might substantially affect bone regeneration.

Background: The PEXIVAS (Plasma exchange and glucocorticoids in severe antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis) trial showed that a reduced-dose glucocorticoid regimen (redGC) was non-inferior to a standard-dose regimen (standGC) with respect to death or end-stage kidney disease (ESKD) in patients with ANCA-associated vasculitis (AAV). However, the primary endpoint did not include disease progression or relapse, cyclophosphamide was the main induction therapy and rituximab (RTX)-treated patients tended to have a higher risk of death or ESKD with redGC. We aimed to evaluate the real-world use of redGC.

Centre de Recherche des Cordeliers, Inserm U1138, Université de Paris, Sorbonne Université, Equipe labellisée par la Ligue contre le cancer, Institut Universitaire de France, Paris, France.

Article Synopsis
  • ACBP/DBI is a protein linked to metabolic-associated steatohepatitis and liver fibrosis, showing higher levels in affected patients, correlating strongly with NAFLD and FIB4 scores, regardless of age or body mass index.
  • A study used a monoclonal antibody to neutralize ACBP/DBI in various mouse models of liver disease, resulting in reduced signs of liver damage and halting disease progression.
  • The results suggest ACBP/DBI plays a causal role in liver conditions and could be a potential therapeutic target for treating liver diseases.

The main monogenic causes of pulmonary fibrosis in adults are mutations in telomere-related genes. These mutations may be associated with extrapulmonary signs (hepatic, haematological and dermatological) and typically present radiologically as usual interstitial pneumonia or unclassifiable fibrosis. In children, the monogenic causes of pulmonary fibrosis are dominated by mutations in surfactant-related genes.

  • Arrhythmic risk stratification in Brugada syndrome is challenging, and this study aimed to evaluate the predictive value of automatically measured ECG parameters from digitized paper ECGs.
  • In a multicenter study involving 301 patients, two ECG parameters (QRS prolongation in lead V1 and S duration on DI) were found to independently predict major cardiac events.
  • The study concluded that automatic ECG analysis could effectively assess risk in Brugada syndrome patients by confirming known parameters and identifying new ones.

Atrial fibrillation (AF) is the primary cause of ischaemic stroke and transient ischaemic attack (TIA). AF is associated with a high risk of recurrence, which can be reduced using optimal prevention strategies, mainly anticoagulant therapy. The availability of effective prophylaxis justifies the need for a significant, coordinated and thorough transdisciplinary effort to screen for AF associated with stroke.
