Er is echter weinig bekend over hoe zorgmedewerkers in verpleeghuizen invulling geven aan persoonsgericht werken. In deze kwalitatieve studie staat de vraag centraal op welke manier zorgprofessionals en naasten de persoonsgerichte elementen persoonsbeeld, communicatie, omgeving en waardigheid terugzien en toepassen in de dagelijkse zorg en wat zij hiervan vinden.

Background: This practice based explorative study aims to provide insight into the ways in which case managers shape and fill up the evaluation phase of their support of the informal care network of persons with dementia.

Method: A combination of quantitative and qualitative research methods were used. A group of 57 case managers of persons with dementia in three different organisational networks took part in this study.

Objectives: Due to the increasing use of electronic patient records and other health care information technology, we see an increase in requests to utilize these data. A highly level of standardization is required during the gathering of these data in the clinical context in order to use it for analyses. Detailed Clinical Models (DCM) have been created toward this purpose and several initiatives have been implemented in various parts of the world to create standardized models.
