A 14-year-old boy presented with a report of sudden-onset loss of vision in the right eye for the previous 10 days. The patient was diagnosed with a total cataract and ruptured posterior lenticonus. Lenticular aspiration was performed with an irrigation/aspiration probe.

Unintended consequences of the electronic medical record on physicians in training and their mentors.

For physicians in training and their mentors, the process of learning and teaching clinical medicine has become challenging in the electronic medical record (EMR) era. Trainees and their mentors exist in a milieu of incessant box checking and laborious documentation that has no clinical educational value, limits the time for teaching and curtails clinical cognitive skill development. These unintended consequences of the EMR are juxtaposed against the EMR's intended benefits of improved patient care and safety with reduced medical errors, improved clinical support systems, reduced potential for negligence with clinical data and metadata data supporting compliance with the standard of care.

Due to the complexity of the skin, diverse approaches are needed, including both traditional and advanced, to improve cutaneous wound healing.

Meaningful Use of Electronic Health Records for Quality Assessment and Review of Clinical Ethics Consultation.

Evolving practice requires peer review of clinical ethics (CE) consultation for quality assessment and improvement. Many institutions have identified the chart note as the basis for this process, but to our knowledge, electronic health record (EHR) systems are not necessarily designed to easily include CE consultation notes. This article provides a framework for the inclusion of CE consultation notes into the formal EHR, describing a developed system in the Epic EHR that allows for the elaborated electronic notation of the CE chart note.

An unusual case of corneal lipoma.

A 24-year-old male presented with a painless yellowish lesion on the nasal part of his cornea that extended to the paracentral area. Without a confirmed clinical diagnosis, the patient was advised surgical removal of the lesion. The lesion showed a smooth, elevated anterior surface without any signs of inflammation.

Cupping device was developed through the integration of crude herb extract for moxibustion and silica gel, which is used specially on the governor vessel, Jiaji (EX-B2) and the bladder meridian of food- for chronic disorders and healthcare. The healthcare cloth was prepared with hi-tech thermal material and on the basis of nourishment of the governor vessel, acting on warming , benefiting and promoting circulation.

As the landscape of health care continues to change, including the increased use of technology to maintain patient records, highlights this need. The changes in the regulations on confidentiality of those who are receiving substance use treatment, 42 CFR Part 2, are presented.

Methods: We did a phase 3, open-label, randomised trial in patients with chronic myeloid leukaemia in chronic phase with suboptimal cytogenetic response to imatinib according to the 2009 European LeukemiaNet criteria, in Latin America, Europe, and Asia (59 hospitals and care centres in 12 countries).
