Article Synopsis
  • The study investigates a new method using omnipolar technology to visualize the slow pathway potential, which is important for treating a specific type of heart arrhythmia.
  • Researchers enrolled 37 patients with atrioventricular nodal reentrant tachycardia and compared outcomes between those treated using a peak frequency map and those who received the standard approach.
  • Findings showed that targeting high-frequency sites near the tricuspid annulus resulted in significantly better ablation success rates, faster recovery times, and higher efficiency compared to traditional methods.

Research on complications necessitating reoperation following vertebroplasty related to hydroxyapatite (HA) blocks is limited. We present the case of a 25-year-old woman who underwent posterior fixation and vertebroplasty using HA blocks for a T12 burst fracture. Postoperative computed tomography revealed anterior protrusion of some blocks, with consequent compression of the descending aorta.

Background: The HACOR score for predicting treatment failure includes vital signs and acid-base balance factors, whereas the ROX index only considers the respiratory rate, oxygen saturation, and fraction of inspired oxygen (FiO). We aimed to externally validate the HACOR score and ROX index for predicting treatment failure in patients with coronavirus disease 2019 (COVID-19) on high-flow nasal cannula (HFNC) therapy in Japan.

Methods: This retrospective, observational, multicenter study included patients, aged ≥ 18 years, diagnosed with COVID-19 and treated with HFNC therapy between January 16, 2020, and March 31, 2022.

Article Synopsis
  • The study aimed to assess the effectiveness of pathological criteria from the JSCCR guidelines for determining the need for additional treatment in patients with pT1 colorectal carcinoma who had undergone local resection.
  • Researchers analyzed data from 4,667 patients and found that those with risk factors for lymph node metastasis had a significantly higher incidence of metastasis and recurrence, while those without risk factors had excellent survival rates.
  • Overall, the findings support the JSCCR criteria, highlighting their importance in identifying low-risk patients who can expect favorable outcomes post-resection.

Background: Catheter ablation of premature ventricular complexes (PVCs) and ventricular tachycardia (VT) from the left ventricular summit (LVS) may require advanced ablation techniques. Bipolar ablation from the coronary veins and adjacent endocardial structures can be effective for refractory LVS arrhythmias.

Objective: The aim of this study was to investigate the outcomes of bipolar ablation performed between the coronary venous system and adjacent endocardial left ventricular outflow tract (LVOT) or right ventricular outflow tract (RVOT).

In cases of unresectable, locally advanced esophageal cancer, conversion surgery may be considered if chemotherapy produces favorable results and surgical resection is indicated. The use of immune checkpoint inhibitors in chemotherapy for esophageal cancer has expanded, and has increased the number of cases in which conversion surgery becomes possible. The patient in the present report had received a diagnosis of Stage Ⅳa esophageal carcinoma, and a prior nephroureterectomy discouraged the administration of platinum-based agents.

Article Synopsis
  • The study investigates how new antirheumatic drugs affect the characteristics of lymphoproliferative disorder (LPD) in rheumatoid arthritis (RA) patients, using data from 53 hospitals in Japan between 1999 and 2021.
  • A total of 752 patients with RA-associated LPD were compared to 770 with sporadic LPD, revealing notable differences in their clinical features and the impact of various drug combinations.
  • The findings suggest that medications taken before LPD onset may alter its characteristics, and recommend tocilizumab (TCZ) as a better treatment option after LPD has developed.

Article Synopsis
  • - The study discusses a challenging case of peri-mitral atrial flutter in a 66-year-old male patient who had multiple unsuccessful radiofrequency ablation attempts due to complex conduction pathways. - The patient underwent a sixth catheter ablation using cryoballoon ablation (CBA), which had not been previously reported for this type of flutter, successfully achieving a bidirectional block in the problematic mitral isthmus area. - The authors suggest that using CBA, despite being off-label, can be effective and safe for peri-mitral atrial flutter, especially with advanced mapping techniques like CARTOUNIVU™ for creating a more effective ablation site.

Background: Bow Hunter's syndrome is vertebral basilar artery insufficiency caused by mechanical occlusion of the vertebral artery during head rotation. This is often due to the formation of osteophytes, herniated discs, cervical spondylosis, or tumours. However, whether the contralateral vessel is organically stenotic is not well known.

Introduction: We investigated the clinical characteristics and outcomes of stroke in SARS-CoV-2 infected patients in Japan.

Methods: This prospective, multicenter observational study of stroke in patients with SARS-CoV-2 infection involving 563 primary stroke centers across Japan was conducted between July 2020, and May 2022. We included 159 stroke cases (131 ischemic stroke, 2 transient ischemic attack (TIA), 21 intracranial hemorrhage, and 5 subarachnoid hemorrhage) and collected their clinical characteristics.

Aim: C-reactive protein (CRP) is both an inflammatory and prognostic marker in various cancers. This study aimed to elucidate the characteristics of CRP and the prognostic factors in patients who were administered with atezolizumab plus bevacizumab (ATZ + BEV) for unresectable hepatocellular carcinoma (HCC).

Methods: A total of 213 patients who received ATZ + BEV for HCC from November 2020 to March 2023 at 15 hospitals were enrolled in this retrospective study.

  • The WHO recommends focusing on specific outcomes like oxygen use and mechanical ventilation to assess COVID-19 vaccine effectiveness (VE), especially post-Omicron, due to variability in hospitalization data.
  • A study in Japan examined VE among 2125 individuals during the Delta and early Omicron periods, finding that 2 doses provided strong protection during Delta, while 3 doses were effective during Omicron.
  • Results showed high VE for both variants, particularly for severe outcomes, highlighting the importance of booster shots in maintaining protection against severe COVID-19 disease.

Thoracic outlet syndrome (TOS) caused by a primary brachial plexus tumour is very rare. A male politician in his 40s presented with numbness, left limb pain and positive Wright and Roos test results. Magnetic resonance imaging (MRI) revealed a tumour located just below the clavicle, compressing the subclavian artery during left arm elevation.

Article Synopsis
  • Ischemic heart disease is a major global health issue, with coronary artery bypass grafting (CABG) being a common treatment, and landiolol is a beta-antagonist believed to reduce postoperative complications like atrial fibrillation.
  • This study analyzed data from a large cohort of CABG patients between 2010 and 2020, comparing those given landiolol during surgery to those who weren't, focusing on their in-hospital mortality rates.
  • Results indicated that patients who received intraoperative landiolol had a significantly lower in-hospital mortality rate (3.7%) compared to those in the control group (4.3%), suggesting that landiolol may improve outcomes for CABG patients, although further research

Background: Although previous studies have shown the isolated areas after pulmonary vein isolation (PVI) using cryoballoons (CB) (CB-PVI), no studies have investigated the association between the isolated area and recurrence of atrial fibrillation (AF) and atrial tachycardia (AT). This single-center observational study investigated the association between the ablated area and recurrence rate after durable CB-PVI for paroxysmal AF.

Methods: This study included 76 patients with paroxysmal AF who underwent CB-PVI and established durable PVI with a second procedure, regardless of AF/AT recurrence, 6 months after the first procedure.
