Background: Complete endoscopic resection of superficial non-ampullary duodenal epithelial tumors (SNADETs) is technically difficult, especially with an extremely high risk of adverse event (AE), although various endoscopic resection methods including endoscopic mucosal resection (EMR), underwater EMR (UEMR), and endoscopic submucosal dissection (ESD) have been tried for SNADETs. Accordingly, a novel simple resection method that can completely resect tumors with a low risk of AEs should be developed.

Aims: A resection method of Noninjecting Resection using Bipolar Soft coagulation mode (NIRBS) which has been reported to be effective and safe for colorectal lesions is adapted for SNADETs.

Background: Liver resection offers substantial advantages over open liver resection (OLR) for patients with hepatocellular carcinoma (HCC) in terms of reduced intraoperative blood loss and morbidity. However, there is limited evidence comparing the indications and perioperative outcomes with the open versus laparoscopic approach for resection. This study aimed to compare postoperative outcomes between patients undergoing laparoscopic liver resection (LLR) and OLR for HCC with clinically significant portal hypertension (CSPH).

The fifth version of the Clinical Practice Guidelines for Hepatocellular Carcinoma was revised by the Japan Society of Hepatology, according to the methodology of evidence-based medicine and partly to the Grading of Recommendations Assessment, Development and Evaluation system, which was published in October 2021 in Japanese. In addition to surveillance-diagnostic and treatment algorithms, a new algorithm for systemic therapy has been created, as multiple drugs for hepatocellular carcinoma can be currently selected. Here, new or revised algorithms and evidence on which the recommendations are based are described.

Backgrounds: Liver resection for hepatocellular carcinoma (HCC) in patients with Child-Pugh class (CPC) B increases the incidence of postoperative complication and in-hospital death and decreases the disease-free survival (DFS) and overall survival (OS) compared with those with CPC A. Conversely, some selected patients possibly gained benefits for liver resection.

Methods: Clinical records of 114 patients with CPC B who underwent liver resection for HCC were retrospectively reviewed.

Article Synopsis
  • The study investigated how repetitive transcranial magnetic stimulation (rTMS) of the cerebellum affects motor performance during targeting tasks in 22 healthy young adults.
  • Participants completed a wrist manipulation task where they aimed to move a bar to a target as quickly as possible; they underwent either active or sham rTMS after the initial repetitions.
  • Results showed that while the sham rTMS group improved in reaction time and accuracy, the active rTMS group experienced inhibited improvements, suggesting that low-frequency rTMS may disrupt motor learning and highlight the cerebellum's role in motor skill development and error correction.

Article Synopsis
  • The study investigated risk factors for severe complications in patients undergoing laparoscopic liver surgery for tumors in specific liver segments from 2009 to 2019.
  • Key findings indicated that a tumor size of 3 cm or more, segmentectomy, and liver cirrhosis significantly increased the risk of complications after surgery.
  • Patients without these risk factors experienced lower complication rates, less blood loss, shorter surgery times, and shorter hospital stays, suggesting they are better candidates for this type of surgery.

Background/purpose Of The Study: Tumor heterogeneity based on copy number variations is associated with the evolution of cancer and its clinical grade. Clonal composition (CC) represents the number of clones based on the distribution of B-allele frequency (BAF) obtained from a genome-wide single nucleotide polymorphism (SNP) array. A higher CC number represents a high degree of heterogeneity.

This retrospective study recorded pertinent baseline geriatric assessment variables to identify risk factors for recurrence-free survival (RFS) and overall survival (OS) after hepatectomy in 100 consecutive patients aged ≥70 years with hepatocellular carcinoma. Patients had geriatric assessments of cognition, nutritional and functional statuses, and comorbidity burden, both preoperatively and at six months postoperatively. The rate of change in each score between preoperative and postoperative assessments was calculated by subtracting the preoperative score from the score at six months postoperatively, then dividing by the score at six months postoperatively.
