Background: Airway management is a critical component of the care of patients experiencing cardiac arrest, but data from randomized trials on the use of video vs direct laryngoscopy for intubation in the setting of cardiac arrest are limited. Current AHA guidelines recommend placement of an endotracheal tube either during CPR or shortly after return of spontaneous circulation but do not provide guidance around intubation methods, including the choice of laryngoscope.

Research Question: Does use of video laryngoscopy improve the incidence of successful intubation on the first attempt, compared to use of direct laryngoscopy, among adults undergoing tracheal intubation after experiencing cardiac arrest?

Study Design And Methods: This secondary analysis of the Direct versus Video Laryngoscope (DEVICE) trial compared video laryngoscopy versus direct laryngoscopy in the subgroup of patients who were intubated following cardiac arrest.

Article Synopsis
  • The study investigates the incidence and outcomes of hypothermic cardiac arrest (HCA) in an urban setting, contrasting with previous focus on wilderness cases.
  • Among 171 eligible patients, 25% suffered HCA, with risk increasing significantly as core temperature decreased; HCA was seen in 6% of moderately hypothermic, 38% of severely hypothermic, and 89% of profoundly hypothermic patients.
  • Witnessed HCA cases, particularly in prehospital or emergency department settings, showed favorable survival outcomes, with a 53% overall survival rate, and even higher (90%) for patients whose HCA occurred post-arrival in the ED.

Background: Increased cardiac troponin (cTn) concentrations occur in acute myocardial injury and chronic diseases. Characterization of cTn composition in the circulation may assist in differentiating etiologies of myocardial injury. Our goal was to study cTn composition and kinetics in patients following type 1 myocardial infraction (T1MI), cardiac procedures, and chronic heart diseases to establish the relationship between cTn composition and clinical diagnosis.

Background: Current studies suggest that cardiac troponin (cTn) forms in the circulation may vary in different clinical scenarios. Our aim was to design a combination of cTn assays specific to the main cTn forms and to evaluate their analytical performance.

Methods: We developed immunoassays specific for measuring (1) long-cTnT cTnI-cTnT-TnC (ITC) ternary complex, with cTnT in long form without cleavage at the C-terminal amino acids residue 189-223, designated "long-cTnT ITC complex assay;" (2) both the long-cTnT ITC complex plus short-cTnT ITC complex, designated "hs-total ITC complex assay;" (3) the central part of cTnT of both the long-cTnT ITC complex and free cTnT, designated "hs-cTnT assay.

Introduction: Opioids are commonly used for acute pain management in patients with rib fractures, but their use poses known risks of addiction and respiratory depression. Regional anesthesia, namely the erector spinae plane(ESP) block, has shown promise as an alternative to opioids in case series. Our aim was to evaluate the efficacy and safety of continuous ropivacaine infusion ESP catheters performed by emergency physicians (EP) for patients with multiple unilateral rib fractures.

Road Guarder (Conophis lineatus concolor: Dipsadidae): Case documentation of a clinically significant envenoming.

Toxicon

December 2024

Department of Experimental and Clinical Pharmacology, University of Minnesota, Minneapolis, MN, USA; Division of Clinical Pharmacology and Toxicology, Department of Medicine, Hennepin County Medical Center (retired), Minneapolis, MN, USA. Electronic address:

Detailed cases of envenoming by a non-front-fanged snake (NFFS) from North, Central, and South America have had limited representation in the toxicology and toxinology literature. The NFFS, Conophis lineatus, has been reported to deliver bites that resulted in moderately severe envenoming. However, most of these reported cases have been via personal communication, or self-reported and lacking in detailed medical evaluations.

Implementation of a Patient-Specific Opioid Taper Calculator for Total Hip and Knee Arthroplasty: A Pre- and Post-Implementation Study.

J Arthroplasty

November 2024

Department of Orthopedic Surgery, University of Minnesota, Minneapolis, Minnesota; Department of Orthopaedic Surgery, Hennepin County Medical Center, Minneapolis, Minnesota.

Background: Given the association between high opioid use and postoperative complications after total joint arthroplasty, it is important to prescribe opioids responsibly in the postoperative period. While many pain regimen protocols exist to try and limit opioid use, an optimal approach to narcotic prescription for arthroplasty patients is yet to be established. This study evaluated the effects of using an individualized opioid taper calculator for patients undergoing elective primary total hip arthroplasty (THA) and total knee arthroplasty (TKA).
