BMC Psychiatry

December 2024

Department of Psychiatry, Psychotherapy and Psychosomatics, Faculty of Medicine, RWTH Aachen University, Pauwelsstraße 30, Aachen, 52074, Germany.

Background: Smoking remains one of the major public health threats, necessitating substantial scientific and societal interest in further developing and implementing systematic, smoking cessation trials. This review examines ongoing randomized controlled trials (RCTs) on smoking cessation and harm reduction, focusing on adherence to German S3 guidelines for tobacco dependence and identifying areas needing further research and neglected aspects in the implementation of treatment guidelines for tobacco dependence.

Methods: A systematic search was conducted on the International Clinical Trials Registry Platform, comprising multiple trial registries worldwide, to identify ongoing RCTs focusing on smoking cessation and harm reduction.

Interventional oncology (IO) employs various techniques to enable minimally invasive, image-guided treatment of tumor diseases with both curative and palliative goals. Additionally, it significantly contributes to managing tumor-related and perioperative complications, offering diverse supportive procedures for patients at all stages of their diseases. The execution of IO procedures places unique demands on the equipment, personnel, and structural organization of radiological clinics, necessitating specific expertise from interventional radiologists.

Article Synopsis
  • Stevens-Johnson syndrome and toxic epidermal necrolysis (SJS/TEN) are severe, drug-induced skin conditions that can be life-threatening and are now viewed as different levels of the same disease known as epidermal necrolysis (EN).
  • A new guideline has been created based on scientific literature and expert consensus to help medical professionals in diagnosing and treating EN.
  • This guideline targets various specialists like dermatologists and intensive care doctors, as well as informing patients, families, insurers, and policymakers about EN and includes recommendations for acute care and follow-up treatment.

Article Synopsis
  • Primary atopic disorders (PAD) are rare genetic conditions caused by specific gene variants that affect skin and immune function, making diagnosis challenging among common allergic disease cases.
  • Identifying PAD requires recognizing clinical red flags like family history and unusual infections, as conventional lab tests are inadequate for definitive diagnosis.
  • Whole-genome sequencing (WGS) enhances diagnostic efficiency and accuracy, but requires careful interpretation and collaboration among specialists to effectively manage PAD cases.

Article Synopsis
  • - Patients with advanced mycosis fungoides and Sézary syndrome generally have a poor prognosis, with survival rates between 4.7 and 1.4 years, highlighting the need for better treatments that enhance both life quality and length.
  • - Mogamulizumab and low-dose total skin electron beam therapy are established treatments but their combined effects had not been studied until now.
  • - This report examines the concurrent use of low-dose TSEBT with mogamulizumab in two advanced-stage patients, suggesting that the combination may be well tolerated and could improve response times, especially in skin and blood areas, though further research is necessary.

The "International Society for Heart and Lung Transplantation Guidelines for the Evaluation and Care of Cardiac Transplant Candidates-2024" updates and replaces the "Listing Criteria for Heart Transplantation: International Society for Heart and Lung Transplantation Guidelines for the Care of Cardiac Transplant Candidates-2006" and the "2016 International Society for Heart Lung Transplantation Listing Criteria for Heart Transplantation: A 10-year Update." The document aims to provide tools to help integrate the numerous variables involved in evaluating patients for transplantation, emphasizing updating the collaborative treatment while waiting for a transplant. There have been significant practice-changing developments in the care of heart transplant recipients since the publication of the International Society for Heart and Lung Transplantation (ISHLT) guidelines in 2006 and the 10-year update in 2016.

The development of aortic insufficiency (AI) during HeartMate 3 durable left ventricular assist device (dLVAD) support can lead to ineffective pump output and recurrent heart failure symptoms. Progression of AI often comingles with the occurrence of other hemodynamic-related events encountered during LVAD support, including right heart failure, arrhythmias, and cardiorenal syndrome. While data on AI burdens and clinical impact are still insufficient in patients on HeartMate 3 support, moderate or worse AI occurs in approximately 8% of patients by 1 year and studies suggest AI continues to progress over time and is associated with increased frequency of right heart failure.

Article Synopsis
  • During the COVID-19 pandemic, the percentage of patients with high frailty in spine care increased significantly compared to pre-pandemic levels.
  • A decrease in low frailty patients was noted, coinciding with changes in surgery rates; low frailty patients experienced an increase in spine surgeries, while high frailty patients saw a decrease.
  • High frail patients showed a larger increase in comorbidities during the pandemic, but changes in in-hospital mortality rates were not linked to frailty levels.

Recognizing cisplatin as a potential radiation recall trigger: case report and focused systematic review.

Strahlenther Onkol

July 2023

Department of Radiation Oncology, Medical Faculty and University Hospital Düsseldorf, Heinrich Heine University, Moorenstr. 5, 40225, Düsseldorf, Germany.

We present a case of mild radiation recall dermatitis triggered by cisplatin chemotherapy given simultaneously to re-irradiation. The dermatitis area correlated to skin exposure of the previous radiation therapy, characterizing the reaction clearly as a recall. Cisplatin has not yet been recognized as a potential trigger for recall reactions.
