Background: Adjuvant therapy with immune-checkpoint inhibitors (CPI) or BRAF/MEK-directed targeted therapy (TT) improves recurrence-free survival (RFS) for patients with advanced, V600-mutant (mut) resected melanoma. However, 40% of these patients will develop distant metastases (DM) within 5 years, which require systemic therapy. Little data exist to guide the choice of upfront adjuvant therapy or treatment management upon DM.

Intraoperative MRI-Guided Resection Is Not Superior to 5-Aminolevulinic Acid Guidance in Newly Diagnosed Glioblastoma: A Prospective Controlled Multicenter Clinical Trial.

J Clin Oncol

December 2023

Department of Neurosurgery, University Hospital Tübingen, Center for Neuro-Oncology, Comprehensive Cancer Center Tübingen-Stuttgart, Eberhard-Karls-University, Tübingen, Germany.

Purpose: Prospective data suggested a superiority of intraoperative MRI (iMRI) over 5-aminolevulinic acid (5-ALA) for achieving complete resections of contrast enhancement in glioblastoma surgery. We investigated this hypothesis in a prospective clinical trial and correlated residual disease volumes with clinical outcome in newly diagnosed glioblastoma.

Methods: This is a prospective controlled multicenter parallel-group trial with two center-specific treatment arms (5-ALA and iMRI) and blinded evaluation.

Perfusion CT is established to aid selection of patients with proximal intracranial vessel occlusion for thrombectomy in the extended time window. Selection is mostly based on simple thresholding of perfusion parameter maps, which, however, does not exploit the full information hidden in the high-dimensional perfusion data. We implemented a multiparametric mass-univariate logistic model to predict tissue outcome based on data from 405 stroke patients with acute proximal vessel occlusion in the anterior circulation who underwent mechanical thrombectomy.

Despite enormous research efforts, the disease mechanisms causing multiple sclerosis (MS) are still incompletely understood, suggesting a complex and multifaceted pathogenesis. Here, we report the clinical observation that in a designated German center for Neurofibromatosis type 2 (NF2), the number of MS cases among NF2 patients is higher than in the general population. Epidemiological studies investigating a connection between NF2 and MS are difficult to perform due to the rarity of NF2 disease.

  • Patients with myasthenia gravis (MG) may experience more severe outcomes from COVID-19, and this study looked at how their immunosuppressive therapy (IST) affects both the risk of infection and the severity of the disease.
  • Analysis of data from the German myasthenia gravis registry revealed that out of 1379 patients, 95 contracted COVID-19, with a significant portion (76%) already on IST.
  • The results indicated that while IST did not increase the risk of contracting COVID-19, it was linked to a higher likelihood of hospitalization or death among those infected, highlighting the need for prevention strategies in this vulnerable population.*
