[Predictive value of the proportion of hibernating myocardium in total perfusion defect on reverse remodeling in patients with HFrEF underwent coronary artery bypass graft].

To evaluate the predictive value of the proportion of hibernating myocardium (HM) in total perfusion defect (TPD) on reverse left ventricle remodeling (RR) after coronary artery bypass graft (CABG) in patients with heart failure with reduced ejection fraction (HFrEF) by Tc-methoxyisobutylisonitrile (MIBI) single photon emission computed tomography (SPECT) myocardial perfusion imaging (MPI) combined with F-flurodeoxyglucose (FDG) gated myocardial imaging positron emission computed tomography (PET). Inpatients diagnosed with HFrEF at the Cardiac Surgery Center, Anzhen Hospital of Capital Medical University from January 2016 to January 2022 were prospectively recruited. MPI combined with F-FDG gated PET was performed before surgery for viability assessment and the patients received follow-up MPI and F-FDG gated PET at different stages (3-12 months) after surgery.

Concordance of the new ESPEN criteria with low phase angle in defining early stages of malnutrition in cardiac surgery.

Background & Aims: The aim of this study was to evaluate the incidence of malnutrition (MN) in cardiac surgery patients by implementing the new ESPEN diagnostic criteria and to determine whether these criteria are concordant with the bioelectrical impedance analysis (BIA) provided phase angle (PA) in predicting early stages of malnutrition.

Methods: A prospective study was conducted in a tertiary hospital. The nutritional state of the cardiac surgery patients was evaluated one day prior to cardiac surgery using the malnutrition screening tools NRS-2002, MUST and SF-MNA, and bioelectrical impedance analysis.

FBN1 polymorphisms in patients with the dilatative pathology of the ascending thoracic aorta.

Objectives: To investigate polymorphisms of the fibrillin-1 (FBN1) gene (namely, rs2118181, rs1036477, rs10519177, rs755251 and rs4774517) in a case-control study for dilatative pathology of the ascending thoracic aorta (DPATA) from Lithuanians.

Methods: We studied 312 patients who had undergone aortic reconstructive surgery for DPATA. These patients were sub-divided according to the phenotypes of their DPATA into (i) ascending aortic aneurysm (n = 160), (ii) post-stenotic dilatation of the ascending aorta due to aortic valve stenosis (n = 79) and (iii) Stanford A dissection (n = 73).

Low-volume, single-shot crystalloid cardioplegia is safe for isolated aortic valve replacement.

We report our experience with a low-volume (100 mL), single-shot crystalloid cardioplegia (Cardioplexol) in 61 consecutive patients undergoing isolated aortic valve replacement (AVR). Cardioplexol resulted in immediate cardiac arrest. Intraoperative courses were uneventful.

Plicated patch repair for acquired Gerbode defect involving the tricuspid valve.

Gerbode's defect, a left ventricular-to-right atrial communication, with involvement of the tricuspid valve acquired after bacterial endocarditis can be challenging to repair. We report a modified technique for a shunt closure and reconstruction of the tricuspid valve using a plicated bovine pericardial patch. Combining such a repair with a left ventricular patch resulted in a complete defect closure and competent tricuspid valve without regurgitation.

Cryopreservation of vascular tissues.

Cryopreservation of human blood vessels may become an important tool in bypass surgery and peripheral vascular reconstruction. Ideally cryopreservation of a blood vessel should preserve functional characteristics comparable to those of fresh controls. The key advantage of cryopreservation is the fact that storage at deep subzero temperatures allows storage of structurally intact living vascular tissues for virtually infinite time.

Reactive oxygen species mediate functional differences in human radial and internal thoracic arteries from smokers.

Objective: Smoking not only increases the risk that coronary heart disease will develop but also morbidity and mortality in patients with known coronary atherosclerosis and after coronary artery bypass grafting. Excessive generation of reactive oxygen species (ROS) has been implicated as the final common pathway for the development of endothelial dysfunction in various cardiovascular risk factors. This study assessed the influence of smoking on two different human arteries routinely used as coronary artery bypass graft conduits.

[Survival rate of patients with ascending aorta aneurysm and aortic valve regurgitation in the late postoperative period].

[Extra-anatomic thoracic aortic bypass operations].

Objective: When the patient condition contraindicates major surgery for descending thoracic aneurysms, the surgeon should consider using an ascending aorta to abdominal aorta bypass graft, leaving the diseased segment undisturbed. Our experience with eight patients is presented.

Material And Methods: Between 1988 and 2008, eight patients were treated for the following indications: reoperation for coarctation (two patients), complicated descending aortic aneurysms (five patients), and posttraumatic descending aorta dissection (one patient).

Impella Recover 100: successful perioperative support for off pump coronary artery bypass grafting surgery in a patient with end-stage ischemic cardiomyopathy.

The authors present the successful use of the Impella recover 100 intracardiac left ventricular assist device in a 55-year old man suffering from end-stage ischemic cardiomyopathy. The pump was implanted preoff pump coronary artery bypass grafting (OPCAB) and it was successfully weaned 5 days postoperatively. In addition, an intra-aortic balloon pump (IABP) was implanted to convert the non-pulsatile flow of the Impella into a pulsatile flow.

[Immunological changes after the Ross operation].

The aim of present paper is to analyze immunological changes due to Ross operation. Cellular immune response as well as incidence of antibodies directed against human leukocyte antigens were studied in patients who received cardiac valve allografts. Standard microlymphocytotoxicity test was used to determine the percentage of panel reactive antibodies.

[Closure of atrial septal defect via small lateral thoracotomies, partial and full length sternotomies].

Surgical closure of atrial septal defect is considered a low risk procedure. Various minimally invasive surgical techniques spread over the conventional full-length sternotomy. But the risk profile and complication rates of the minimally invasive closure of atrial septal defect are not revealed sufficiently.

[Revision surgery after closure of isolated ventricular septal defects].

During the period from 1964 10 15 to 2002 06 01 428 patients underwent defect closure for isolated ventricular septal defect at Vilnius University Heart Surgery Clinic. Seventeen (4.2%) patients necessitated revision surgery in the postoperative period to repair haemodinamically significant postoperative complications.

[Influence of reconstruction of right ventricle outflow tract to the results of radical correction of tetralogy of Fallot].

[Congenital coronary artery anomalies in patients with tetralogy of Fallot].

Objective: The aim of the study was to establish frequency of congenital coronary arteries anomalies in patients with tetralogy of Fallot; to evaluate importance of early diagnostics in planning of surgical tactics and impact on early surgical results.

Material And Methods: Consecutive retrospective analysis of angiography data of 278 patients with tetralogy of Fallot and 219 surgical protocols of total corrections of tetralogy of Fallot was performed.

Results: Twenty coronary arteries anomalies were diagnosed by means of angiography and 2 during surgery.

[Congenital heart surgery in neonates].

From January 1974 trough August 2002 184 neonates with congenital heart disease underwent surgical repair in Heart Surgery Clinic of Vilnius University. It came to 3.8% of all 4813 procedures of congenital heart diseases performed.

[Interventional paediatric cardiology in Vilnius].

[Risk of second and third reoperation on prosthetic heart valves].

As the volume of heart valve replacements increases, more patients are undergoing repeated operations. There are many reasons for reoperations that include incomplete or imperfectly executed primary operation, infection on valve prostheses, valve related complications. Repeated operations have their own specific technical problems, and from them depends mortality and complications rate.

[Early and late postoperative results of mitral and tricuspid valve insufficiency surgical treatment using edge-to-edge central coaptation procedure].

Edge-to-edge approximation of the mitral leaflets and creation of a double-orifice mitral valve (Jatene-Alfieri procedure), for mitral regurgitation surgical treatment was effective in most cases, especially for patients who had rheumatic and degenerative valve disease, traumatic or ischemic mitral valve insufficiency. Positive results of double-orifice technique encourages to use this method to correct some forms of tricuspid insufficiency. Triple-orifice repair procedure is a lot more effective to correct central prolapses in all of the three leaflets (extraordinary elongated chordaes) and/or degenerative tricuspid annular dilation than the ordinary routine procedures.
