Home blood pressure (BP) measurement is widely used, and home BP plays a central role in hypertension management in clinical practice in Japan. We updated the systematic review to determine whether home BP-based antihypertensive treatment is superior to office BP-based treatment in improving clinical outcomes, including BP levels, in adult patients with essential hypertension. A literature search identified 14 randomized controlled trials (RCTs).

Article Synopsis
  • This study investigated the link between serum uric acid (UA) levels and new-onset hypertension (HTN) in university students aged 18 to 20 in Gifu, Japan, over 12 years.
  • Out of nearly 2,900 normotensive students, 75 (2.6%) developed HTN after four years, showing higher UA levels at both initial and follow-up visits compared to those who remained HTN-free.
  • The findings indicate that elevated UA levels are a significant independent risk factor for developing HTN in young adults, with increases correlating to a higher likelihood of new-onset hypertension.

Article Synopsis
  • Epidemiological studies show that the urine sodium-to-potassium (Na/K) ratio is a stronger indicator of high blood pressure and cardiovascular disease risk compared to measuring sodium or potassium alone.
  • For accurate individual estimates, it's recommended to average the Na/K ratio from casual urine samples collected on at least four different days a week, due to high variability in measurements.
  • The ideal target for the Na/K ratio is an average of 2 for healthy individuals, aligning with dietary recommendations for sodium and potassium in Japan, while an average of 4 can be a reasonable temporary goal based on population averages.

Atypical femoral fracture (AFF) is generally a rare complication of long-term use of bisphosphonate (BP); glucocorticoid (GC) use and Asian race are also risk factors. Femoral localized periosteal thickening (LPT, also termed "beaking") of the lateral cortex often precedes AFF. This cohort study investigated the incidence of LPT and AFF and their clinical courses over 10 yr in patients with autoimmune inflammatory rheumatic diseases (AIRDs) treated with BP and GC.

Nomenclature for psychosis risk in Japan: Survey results from high-risk individuals, caregivers, and mental health professionals.

Schizophr Res

May 2024

Centre for Youth Mental Health, The University of Melbourne, Melbourne, Victoria, Australia; Orygen Specialist Program, Parkville, Victoria, Australia.

Background: Labeling terms for high-risk state for psychosis, such as 'ultra-high risk' (UHR), 'attenuated psychosis syndrome' (APS), and 'at-risk mental state' (ARMS), have been criticized for their potential to lead to stigma. Hence, mental health service users in Melbourne recently proposed new terms illustrating the at-risk concept ['pre-diagnosis stage' (PDS), 'potential of developing a mental illness' (PDMI), and 'disposition for developing a mental illness' (DDMI)]. We aimed at testing the suitability of these existing and new terms in the clinical settings of early psychiatric intervention in Japan.
