Purpose: Oral corticosteroids (OCS) are recommended for the treatment of exacerbations in people with COPD; however, high cumulative lifetime doses (≥1000mg prednisolone-equivalent) are associated with adverse health effects. This issue is well defined in asthma but is less well understood in COPD. The aim of this study was to examine cumulative OCS dispensed to people with COPD over 12 months.

The Association Between the CALLY Index and All-Cause Mortality in Patients with COPD: Results from the Cohort Study of NHANES 2007-2010.

Purpose: The C-reactive protein (CRP)-albumin-lymphocyte (CALLY) index is a newly developed biomarker that combines measurements of CRP, serum albumin, and lymphocyte count. This index provides a thorough assessment of a patient's inflammation level, nutritional condition, and immunological function. The objective of this study is to examine the correlation between the CALLY index and all-cause mortality in COPD patients.

Association Between Diurnal Temperature Range and Risk of Cardiomyopathy-Induced Hospitalisation in Henan, China: A Time-Series Study.

Purpose: The effect of the diurnal temperature range (DTR) on human health in diverse geographic areas and the potential confounding factors are not fully understood. Additionally, while a robust association has been reported between temperature and cardiomyopathy (CM), evidence of the impact of DTR is relatively limited. Here, we determined whether an association exists between DTR and CM hospitalisations in vulnerable populations.

This paper aims to examine and compare the impact of DT on the risk of falling (ROF) among Saudi female students.

Methods: The study that utilised the Lot Quality Assurance Sampling (LQAS) approach was conducted in eight districts of Central Uganda; Kiboga, Kyankwanzi, Mubende, Kasanda, Mityana, Luwero, Nakaseke, and Nakasongola districts. Each district was divided into five supervision areas (SAs).

Mesonephric-like adenocarcinoma (MLA) is a rare and newly recognized subtype of ovarian and endometrial carcinomas, introduced in the 2020 World Health Organization Classification. This tumor likely originates from Müllerian-derived tissues and often mimics more common ovarian cancers, leading to frequent misdiagnosis. This case study details a 36-year-old woman who presented with urinary symptoms following a hysterectomy.

A union of two rare pathologies: small bowel diverticula perforation secondary to impacted gallstone ileus.

An 84-year-old lady presented with 1 day history of sudden onset generalized abdominal pain, fevers, and peritonism. Computed tomography was suggestive of a mid-small bowel perforation associated with a distal ovoid soft tissue density structure without pneumobilia. An urgent laparotomy demonstrated two areas of jejunal diverticula necrosis and perforation associated with a 3 cm luminal mass in the proximal ileum, and proximal small bowel dilatation.

However, the increasing use of electronic devices has led to a significant increase in exposure to visible light, especially blue light. We measured the irradiance (mW/cm) and estimated dose (J/cm) of visible light and blue light emitted from various electronic devices including smartphones, tablets and computers.

Although acne typically presents during adolescence, it is a chronic condition that also affects many adults. Despite the spectrum of treatments available for acne, limitations in tolerability and safety concerns can present challenges for the use of conventional medications in clinical practice.

Roads to remission: evolving treatment concepts in type 2 inflammatory diseases.

Unlabelled: Non-communicable diseases (NCDs) characterised by type 2 inflammation, including asthma, allergic rhinitis, chronic rhinosinusitis with nasal polyps, atopic dermatitis, food allergies and eosinophilic esophagitis, are increasing in prevalence worldwide. Currently, there is a major paradigm shift in the management of these diseases, towards the concept of disease modification and the treatment goal remission, regardless of severity and age. Remission as a treatment goal in chronic inflammatory NCDs was first introduced in rheumatoid arthritis, and then adopted in other non-type 2 inflammatory diseases.

Alcohol use and HIV suppression after completion of financial incentives for alcohol abstinence and isoniazid adherence: a randomized controlled trial.

Background: In a recent randomized trial, six months of financial incentives contingent for recent alcohol abstinence led to lower levels of hazardous drinking, while incentives for recent isoniazid (INH) ingestion had no impact on INH adherence, during TB preventive therapy among persons with HIV (PWH). Whether the short-term incentives influence long-term alcohol use and HIV viral suppression post-intervention is unknown.

Methods: We analyzed twelve-month HIV viral suppression and alcohol use in the Drinkers' Intervention to Prevent Tuberculosis study, a randomized controlled trial among PWH with latent TB and unhealthy alcohol use in south-western Uganda.

A reproducible framework for monitoring the impact of randomized clinical trials on clinical practice using large-scale real-world data: application to gynaecological surgical trials using the French national healthcare database.

Background: Randomized clinical trials (RCTs) are fundamental to evidence-based medicine, but their real-world impact on clinical practice often remains unmonitored. Leveraging large-scale real-world data can enable systematic monitoring of RCT effects. We aimed to develop a reproducible framework using real-world data to assess how major RCTs influence medical practice, using two pivotal surgical RCTs in gynaecologic oncology as an example-the LACC (Laparoscopic Approach to Cervical Cancer) and LION (Lymphadenectomy in Ovarian Neoplasms) trials.

Over the past few decades, the global landscape of chronic liver diseases, including hepatitis, metabolic dysfunction-associated steatotic liver disease (MASLD), liver fibrosis, and HCC, has undergone substantial changes. Copper, a vital trace element for human health, is predominantly regulated by the liver.

Long-term mental health crisis among municipal public employees caused by the Fukushima nuclear accident and subsequent disasters: Questionnaire survey 10 years postdisaster.

Aim: After the Fukushima nuclear accident in 2011, several municipal offices were forced to evacuate, and municipal public employees (MPEs) had to perform many administrative tasks related to the disaster. Typhoons and the COVID-19 pandemic also affected the area afterwards. We conducted a survey for MPEs to investigate the mental health impacts and related factors.

Launching a child and adolescent psychiatry training program in Mongolia inspired by Japanese models.

This manuscript details the development and implementation of Mongolia's first official training program for child and adolescent psychiatry (CAP) specialists. This initiative, inspired by and developed in collaboration with Japanese CAP training models, addresses the substantial gap in specialized mental health services for children and adolescents in Mongolia. Our discussion elaborates on the collaborative efforts between the Mongolian National University of Medical Sciences, Nagoya University, and other partnering institutions, reflecting on the initial outcomes and the strategic importance of this program.

Comparison of good review practices of seven countries participating in the ECOWAS medicines regulatory harmonisation initiative: identifying opportunities for improvement.

Introduction: When implemented by national and regional regulatory agencies good review practices (GRevPs) support the timely high-quality review of medicines for enhanced patients' availability to safe, quality and efficacious innovative and generic products. It is important that all aspects of GRevPs are continuously evaluated and updated to promote the continuous improvement of regulatory systems at national and regional levels. The aim of this study was to assess and compare the GRevPs of the national medicines regulatory agencies (NMRAs) of Burkina Faso, Cote d'Ivoire, Ghana, Nigeria, Senegal, Sierra Leone and Togo, who are active participants of the ECOWASMRH initiative to identify opportunities for improvement.

Generative artificial intelligence (GenAI) is rapidly transforming various sectors, including healthcare and education. This paper explores the potential opportunities and risks of GenAI in graduate medical education (GME). We review the existing literature and provide commentary on how GenAI could impact GME, including five key areas of opportunity: electronic health record (EHR) workload reduction, clinical simulation, individualized education, research and analytics support, and clinical decision support.

Effect of breathing exercises to prevent pulmonary complications in patients undergoing coronary artery bypass graft surgery: a prospective randomized controlled trials study protocol.

Purpose: To study the effects of breathing exercises on preventing pulmonary complications in patients undergoing coronary artery bypass graft surgery.

Methods: Observing whether preoperative breathing exercises can reduce the incidence of postoperative pulmonary complications in patients undergoing coronary artery bypass graft surgery; observing whether these exercises can improve postoperative arterial oxygen pressure, oxygen saturation, and the distance walked in a six-minute walk test after surgery; as well as reduce hospital stay duration, lower treatment costs, and improve the quality of life as measured by the Short Form-36 Health Survey (SF-36).
