Klinisk institutt 2, Universitetet i Bergen, og, Barne- og ungdomsklinikken, Haukeland universitetssjukehus.

This clinical review will give doctors who work with children and neonates an introduction to the diagnosis and treatment of congenital hyperinsulinism, the most common cause of persistent neonatal hypoglycaemia. The condition is a rare monogenic disorder characterised by elevated insulin secretion and is a result of mutations in genes that regulate insulin secretion from pancreatic beta cells. The anabolic effect of insulin induces systemic glucose uptake and inhibits gluconeogenesis, glycogenolysis, ketogenesis and lipolysis.

Article Synopsis
  • - The case highlights the need for more invasive testing to diagnose rare conditions in patients with severe anaemia, as standard blood tests may not be sufficient.
  • - A middle-aged man experienced severe anaemia leading to a transfusion, and after ruling out other common causes, a diagnosis of pure red cell aplasia was confirmed through a repeat bone marrow biopsy.
  • - Treatment with ciclosporin was effective, but the exact cause of the man's condition remains uncertain – whether it was idiopathic or related to previous dicloxacillin use.

The disease not only affects the musculoskeletal system, but is multisystemic, and ocular involvement with cataract formation is a frequent additional finding. To avoid recurrence of secondary opacification that is difficult to treat, the cataract should not be treated with traditional lens replacement.

A dermatologist recognised clinical features associated with a rare diagnosis, which was eventually confirmed by diagnostic workup. Known atopic eczema increased the risk of a severe course of what was initially a mild disease, and a multidisciplinary approach proved necessary.

Chronic graft-versus-host disease is a late complication of allogeneic stem cell transplantation and leads to chronic inflammation and fibrosis in various organs due to dysregulation of donor immune cells. The disease can occur in all organs, but is most frequently seen in the skin, eyes, oral cavity, gastrointestinal tract, genitalia, lungs, muscles, fascia and joints. Chronic graft-versus-host disease is associated with considerable morbidity and mortality, and treatment requires close collaboration between different parts of the specialist health services.
