Background: Cerebral aneurysms (CAs) affect 3-5% of the general population, with saccular aneurysms being the most common type. Despite advances in treatment, patient understanding of CAs and associated procedures remains limited, impacting informed consent and treatment outcomes.

Objectives: This pilot study aims to evaluate the effectiveness of mixed reality (MR) technology in enhancing patient education and understanding of cerebral angiograms and aneurysm treatment, thereby improving the patient-surgeon communication process.

Gastroschisis is one of the most common congenital gastrointestinal disorders, occurring in about one in 1,953 infants born each year in the United States. Infants with gastroschisis rely on total parenteral nutrition (TPN) preoperatively, and due to intestinal function and dysmotility issues, continue to face feeding challenges postclosure, including feeding intolerance and increased risk of necrotizing enterocolitis (NEC). Postclosure, human milk-feeding is preferred over infant formula because of its associated reduced risk of feeding intolerance and NEC.

Introduction: The utility of pancreaticoduodenectomy (PD) for high-grade traumatic injuries remains unclear and data surrounding its use are limited. We hypothesized that PD does not result in improved outcomes when compared with non-PD surgical management of grade IV-V pancreaticoduodenal injuries.

Methods: This is a retrospective, multicenter analysis from 35 level 1 trauma centers from January 2010 to December 2020.

Article Synopsis
  • * A study compared clinical outcomes between critically ill patients receiving meropenem via IVP and EI, finding no significant overall difference in clinical stabilization rates (48% vs. 44%).
  • * However, patients on the EI regimen reached clinical stability faster (20.4 hours vs. 66.2 hours) and had shorter hospital stays (13 days vs. 17 days) and ICU stays (6 days vs. 9 days), indicating potential advantages of EI over IVP.

Standardization and Reproducibility of Dynamic Stress Testing for Occult Pelvic Ring Instability.

J Am Acad Orthop Surg

July 2024

From the University of Chicago Medical Center, Chicago, IL (Lee), Cedars Sinai Medical Center, Los Angeles, CA (Marecek), and Texas Health Harris Methodist Hospital Fort Worth, Fort Worth, TX (Collinge).

Introduction: Examination under anesthesia (EUA) (stress fluoroscopy) is commonly done after pelvic ring injury to identify occult instability because unstable disruptions may displace causing morbidity. The force applied during EUA for these injuries has not been standardized. The purpose of this study was to examine the forces used during the EUA by experienced orthopaedic trauma pelvic surgeons.

Objectives: To analyze patients, injury patterns, and treatment of femoral neck fractures (FNFs) in young patients with FNFs associated with shaft fractures (assocFNFs) to improve clinical outcomes. The secondary goal was to compare this injury pattern to that of young patients with isolated FNFs (isolFNFs).

Design: Retrospective multicenter cohort series.

Article Synopsis
  • Ballistic embolism (BE) is a rare but serious complication from firearm injuries, often leading to confusion and delays in treatment if not recognized early.
  • A study analyzed 136 cases of BE, finding that arterial emboli were more likely to cause symptoms and required more invasive management compared to venous emboli.
  • Patients with arterial emboli had better outcomes, with most remaining stable or improving after discharge, underscoring the importance of monitoring for BE within 48 hours of injury.

Assessment of Force Applied During Examination Under Anesthesia Used to Determine Stability in Posterior Wall Acetabular Fractures.

J Am Acad Orthop Surg

November 2024

From the Dignity Health Medical Group - St. Joseph's Hospital and Medical Center, Phoenix, AZ (Lee), Texas Health Harris Methodist Hospital Fort Worth, Fort Worth, TX (Collinge).

Objectives: Determination of hip instability associated with posterior wall acetabular fractures may be difficult. Thus, dynamic stress examination under anesthesia (EUA) was developed as a tool for guiding treatment. EUA uses positioning of the hip and application of force across the hip to detect instability.

Guidelines provide varying recommendations for the prophylactic antimicrobial treatment of open fractures. This single-center, retrospective cohort study was conducted to determine how well an institutional prophylactic antibiotic protocol covered pathogens associated with open fractures. The authors included adult trauma patients with one or more open fractures and a positive culture from the site of the open fracture, and compared outcomes between patients who were covered by prophylactic antibiotics with patients not covered by prophylactic antibiotics.

Key Clinical Message: Austrian Syndrome classically consists of meningitis, endocarditis, and pneumonia due to bacteremia. A literature review, however, does not show variants of this triad. Our case highlights a unique variant of Austrian Syndrome with mastoiditis, meningitis, and endocarditis which requires immediate recognition and treatment to prevent devastating patient outcomes.

Objective: Mild traumatic brain injuries (MTBI) associated with intracranial haemorrhage are commonly transferred to tertiary care centres. Recent studies have shown that transfers for low-severity traumatic brain injuries may be unnecessary. Trauma systems can be overwhelmed by low acuity patients justifying standardisation of MTBI transfers.

Changing Osteoporosis Knowledge and Behaviors Through Structured Education.

Orthop Nurs

January 2023

Kindra McWilliam Ross, DNP, APRN, ACNS-BC, Orthopaedic Clinical Nurse Specialist, Texas Health Harris Methodist Hospital Fort Worth, Fort Worth; Clinical Assistant Professor-AO, College of Nursing and Health Innovation, The University of Texas, Arlington.

Osteoporosis is a silent disease that is associated with enormous cost and can lead to disability and death. We identified that individuals who have sustained a fracture from a fall are often unaware of osteoporosis risk factors and have a knowledge deficit regarding osteoporosis. Therefore, they do not receive the proper treatment.

Objective: Collection of bone graft with the Reamer-Irrigator-Aspirator (RIA) system has become common practice across the field of orthopaedic surgery. While RIA bone graft is typically obtained from native long bones, grafting material can likewise be harvested from long bones that have previously undergone the placement and removal of an intramedullary nail, a process termed re-reamed RIA (RRR). The purpose of this study was to evaluate the total protein and growth factor concentrations present in native-RIA (NR) compared with RRR samples.

Background: Implant-based breast reconstruction accounts for the vast majority of breast reconstruction procedures and is commonly performed with human acellular dermal matrix. There is no consensus as to the optimal human acellular dermal matrix preparation, and high-quality evidence concerning comparative effectiveness is lacking. This study is the first prospective, multicenter, randomized controlled clinical trial to compare human acellular dermal matrix-related complications of the two most commonly used human acellular dermal matrices in implant-based breast reconstruction.
