The breakthrough effects of tyrosine kinase inhibitors (TKIs) have lessened indications for allogeneic hematopoietic stem cell transplantation (HSCT) in chronic myeloid leukemia (CML). However, HSCT is still attractive for children and adolescents/young adults (AYAs) requiring lifelong TKI therapy. Nevertheless, little has been reported on the outcomes of large clinical studies of HSCT targeting these age groups.

Posterior communicating artery (PComA) aneurysms are common cerebral aneurysms. In cases of PComA aneurysms accompanied by anatomical variations of PComA, such as hypoplasia, hyperplasia, or infundibular dilatation, it is challenging to completely determine the relationship between the parent artery, daughter arteries, and the aneurysmal neck preoperatively. We present a case of a ruptured PComA aneurysm originating from the PComA, accompanied by infundibular dilatation, which was preoperatively diagnosed as a double-hump PComA aneurysm with hypoplastic or absent PComA, based on computed tomography (CT) angiography images.

The introduction of tyrosine kinase inhibitors (TKIs) in chronic myeloid leukemia (CML) treatment has dramatically improved the prognosis of CML patients and reduced the number of patients receiving allogeneic stem cell transplantation (allo-SCT). However, the impact of the newest-generation TKIs on the overall survival (OS) after allo-SCT has not been well described. To investigate the beneficial effects of TKIs on the prognosis after allo-SCT, we conducted a retrospective observational study using the Transplant Registry Unified Management Program database in Japan.

Acute myeloid leukemia (AML) predominantly affects elderly adults, and its prognosis worsens with age. Treatment options for patients in Japan ineligible for intensive chemotherapy include cytarabine/aclarubicin ± granulocyte colony-stimulating factor (CA ± G), azacitidine (AZA), low-dose cytarabine (LDAC), targeted therapy, and best supportive care (BSC). The country's aging population and the evolving treatment landscape are contributing to a need to understand treatment pathways and associated outcomes.

Calcineurin inhibitors (CNIs), used as immunosuppressants, have revolutionized transplantation medicine with their strong suppressive activity on alloreactive T lymphocytes; however, they may also cause various adverse effects, including an increased risk for infection and nephrotoxicity. Regulatory T (Treg) cells can complement the deleterious side effects of CNIs with their effective Ag-specific suppressive activities. However, several studies have shown that CNIs suppress Treg cell differentiation.

Impact of HLA Epitope Matching on Outcomes After Unrelated Bone Marrow Transplantation.

Front Immunol

May 2022

Division of Endocrinology, Diabetes and Metabolism, Hematology, Rheumatology (Second Department of Internal Medicine), Graduate School of Medicine, University of the Ryukyus, Nishihara, Japan.

Article Synopsis
  • The study investigates how antibody-identified epitopes impact humoral alloimmunity in non-permissive HLA mismatching during hematopoietic stem cell transplantation (HSCT), using a large cohort of nearly 10,000 patients.
  • It quantifies HLA eplet mismatches in donor-recipient pairs, finding varying degrees of mismatches in HLA class I and II, which correlate to patient outcomes.
  • Specifically, HLA-C eplet mismatches in the graft-versus-host direction significantly increase the risk of severe acute graft-versus-host disease (aGVHD), non-relapse mortality, and lower overall survival rates, underscoring the potential of HLA epitope matching to predict complications post-trans

Even in the era of high-intensity chemotherapy, disease recurrence remains a major cause of treatment failure in adult patients with Philadelphia chromosome-negative B-cell acute lymphoblastic leukemia (Ph-negative B-ALL). For patients who achieved second complete remission (CR2) with salvage chemotherapy, allogeneic hematopoietic stem cell transplantation (allo-HSCT) could be the best curative treatment. However, limited data are available on the outcomes of allo-HSCT for adult Ph-negative B-ALL in CR2 in the high-intensity chemotherapy era.

Article Synopsis
  • - A study evaluated the effectiveness of PD-1 inhibitor therapy in 36 patients with recurrent or metastatic salivary gland carcinoma, revealing an objective response rate of only 11.1%.
  • - Only patients with specific histopathological subtypes, particularly salivary duct carcinoma, showed positive responses, all of whom also had high PD-L1 expression.
  • - The results suggest that while PD-1 therapy has limited overall efficacy for this cancer type, it may benefit certain patients, especially those with positive PD-L1 in salivary duct carcinoma.

Risk and Predictive Factors for Candidemia After Allogeneic Hematopoietic Cell Transplantation: JSTCT Transplant Complications Working Group.

Transplant Cell Ther

April 2022

Division of Hematology, Jichi Medical University Saitama Medical Center, Saitama, Japan; Transplant Complications Working Group of the Japan Society for Transplantation and Cellular Therapy (JSTCT).

Although antifungal prophylaxis that covers Candida species is a standard of care in allogeneic hematopoietic cell transplantation (HCT), candidemia mainly caused by non-albicans Candida species still occurs and is associated with a high mortality rate. This study aimed to evaluate the risk factors for candidemia after allogeneic HCT. Particularly, we evaluated the impact of patient factors such as hematopoietic cell transplantation-specific comorbidity index (HCT-CI) and performance status (PS) in addition to well-recognized risk factors including donor type, delayed engraftment, and graft-versus-host disease (GVHD).
