Introduction: Polypropylene material is commonly used for posterior wall reconstruction in hernia repair, in contrast with the classically described stainless-steel wire used at Shouldice Hospital. This study was conducted to evaluate possible differences in Shouldice Repair outcomes using polypropylene or stainless-steel wire sutures.

Methods: A prospective follow-up of consecutive patients who underwent elective unilateral Shouldice primary inguinal hernia repair at Shouldice Hospital between December 6, 2021, and September 1, 2022, was conducted.

Melatonin, a pineal gland hormone closely associated with the circadian rhythm, has been trending over the past years as an over-the-counter medication to aid with sleep disturbances. Although generally believed to be safe, recent studies show negative inotropic and chronotropic effects on the heart rate and blood pressure in humans. Several studies suggested that melatonin induces cardiac vagal tone and affects heart rate and mean arterial pressure.

Department of Otolaryngology - Head and Neck Surgery, Hamad Medical Corporation, Doha, QAT.

Background Stapes surgery success depends on several factors, including the length of the prosthesis used. Whether to use a standard-size prosthesis or measure the length of the stapes prosthesis has been debated in the literature. This study aims to assess the surgical outcomes of a stapedotomy using the standard 4.

Background: The use of cuffed endotracheal tubes (ETTs) has become the standard of care in pediatric practice. The rationale for the use of a cuffed ETT is to minimize pressure around the cricoid while providing an effective airway seal. However, safe care requires that the cuff lie distal to the cricoid ring following endotracheal intubation.

  • The "OUTSTRIP COVID-19" study aimed to evaluate the long-term outcomes of ICU patients with COVID-19, focusing on mortality, physical and psychiatric issues, and overall recovery over a two-year period in Qatar.
  • A cohort of 100 ICU survivors was assessed for cognitive impairment and psychological conditions 7 weeks post-discharge and again after 3 months, using specific assessment tools.
  • The results showed improvement in cognitive impairment among survivors, with mild cognitive issues decreasing from 72% to 56%, while stress levels and clinical depression also showed slight improvement, but anxiety demonstrated a significant reduction over the same period.

Background: N6-methyladenosine (mA) is the most common internal RNA modification and is involved in regulation of RNA and protein expression. AlkB family member 5 (ALKBH5) is a mA demethylase. Given the important role of mA in biological mechanisms, mA and its regulators, have been implicated in many disease processes, including cancer.

This case presents a somewhat unique and different phenotype of hereditary spastic paraplegia from previously reported kinase D-interacting substrate of 220 kDa () gene mutation-related disease. We report a unique putative causative heterozygous mutation in in a pure hereditary spastic paraplegia (HSP) patient expanding the HSP group further. We also deliberate on how our case was different from prior -related pathologies including spastic paraplegia, intellectual disability, nystagmus, and obesity (SINO) syndrome, and the observation of and aquaporin-4 () downregulation in the ventricular ependymal lining of idiopathic normal pressure hydrocephalus (iNPH) patients.

The accuracy of Gemini, GPT-4, and GPT-4o in ECG analysis: A comparison with cardiologists and emergency medicine specialists.

Am J Emerg Med

October 2024

Emergency Medicine, Department of Emergency Medicine, Hamad Medical Corporation, Hamad General Hospital, Doha, Qatar. Electronic address:

Introduction: GPT-4, GPT-4o and Gemini advanced, which are among the well-known large language models (LLMs), have the capability to recognize and interpret visual data. When the literature is examined, there are a very limited number of studies examining the ECG performance of GPT-4. However, there is no study in the literature examining the success of Gemini and GPT-4o in ECG evaluation.

  • Individuals with normal blood pressure and about 30% of chronic kidney disease (CKD) patients may experience masked hypertension (MHTN), with those on medication called masked uncontrolled hypertension (MUHTN), leading to a higher risk of cardiovascular events and kidney dysfunction.
  • The review explores the diagnostic methods, prevalence, underlying mechanisms, and importance of managing masked hypertension to protect vital organs like the kidneys and heart.
  • Early detection through regular blood pressure monitoring can help prevent organ damage and progress to sustained hypertension, indicating a need for updated guidelines and further research in this area.

Article Synopsis
  • The study investigates the survival outcomes of adult COVID-19 patients on venovenous extracorporeal membrane oxygenation (VV-ECMO) across different regions and timeframes, analyzing data from 1060 patients in 351 institutions worldwide between January 2020 and December 2021.
  • It found that the in-hospital mortality rate for the overall cohort was 47.12%, with notable differences in mortality trends among regions, especially a spike in deaths in North America and Europe during mid-2020, while Latin America maintained better survival rates for longer.
  • The results suggest that various factors, including the timing of ECMO initiation and patient demographics, contribute to mortality variances, emphasizing the need for improved patient selection

Background: SGLT2 inhibitors are known for their osmotic diuretic effect, and their use by Muslim patients with type 2 diabetes during the fasting month of Ramadan may pose an increased risk of volume depletion, potentially impacting renal function.

Methods: We conducted a systematic review registered on PROSPERO (registration number CRD42020204582) of studies published between 2013 and January 2023, sourced from PubMed, EMBASE, and the Cochrane Central Register of Controlled Trials. The study selection criteria included controlled studies that reported the use of SGLT2 inhibitors (SGLT2i) by fasting adult type 2 diabetes patients and provided data on creatinine or estimated glomerular filtration rate (eGFR) as outcomes.

Introduction: This study was undertaken to investigate whether sustained rather than a single measure of corneal nerve loss was associated with the onset of diabetic peripheral neuropathy (DPN) and the progression of neuropathic symptoms and deficits in individuals with type 2 diabetes (T2D).

Methods: Participants underwent clinical, metabolic testing and assessment of neuropathic symptoms, vibration perception threshold (VPT), sudomotor function, and corneal confocal microscopy (CCM) at baseline, 1, 2, and 4-7 years. Sustained corneal nerve loss was defined as abnormal corneal nerve fiber density (CNFD, <24 fibers/mm), corneal nerve branch density (CNBD, <21 branches/mm), and corneal nerve fiber length (CNFL, <16 mm/mm) persisting for ≥50% of the study duration.

Introduction: Adolescents and young adults (AYAs) diagnosed with chronic myeloid leukemia (CML) constitute a significant demographic group, particularly in regions with youthful populations like Qatar. Despite the global median age of CML diagnosis being 65 years, Qatar's age distribution reflects a younger cohort. This study investigates whether AYAs with CML exhibit distinct clinicopathological characteristics or outcomes compared to older age groups.

  • Hepatitis A virus (HAV) is a major cause of acute viral hepatitis globally, and this study focused on its seroprevalence among Qatar's migrant craft and manual workers (CMWs), who make up about 60% of the population.
  • Out of 2,607 tested serum samples, 92% were positive for HAV antibodies, with seroprevalence varying by nationality—ranging from 70.9% for Sri Lankans to 99.8% for Pakistanis.
  • Factors such as age, nationality, and education level significantly influenced HAV infection rates, with older CMWs showing a higher risk, indicating that many individuals were likely exposed during childhood.

Purpose: The globally increasing older population raises concerns about age-related conditions, including cognitive impairment and depressive symptoms. In Latin America, nearly one-third of the population is affected by either of these conditions. However, data investigating the association between cognitive impairment and depressive symptoms, particularly in Brazil, are limited to small-scale studies that have not carefully examined the critical effects of variables such as education level and socioeconomic status on this relationship.
