Empyema Caused by Mixed Infection with and in a Patient with Previous Surgery for Oral Carcinoma: A Case Report.

Infect Drug Resist

October 2024

Department of Pulmonary and Critical Care Medicine, Sir Run Run Shaw Hospital, Zhejiang University School of Medicine, Hangzhou, People's Republic of China.

Background: The incidence of community-acquired empyema caused by the group (SAG) has been on the rise in the 2020s. To the best of our knowledge, while empyema caused individually by either strain has been reported, there are no reports on empyema caused by concurrent infection with these two strains. Here, we report for the first time empyema caused by concurrent infection with and (both SAG species) in a postoperative patient who had been treated for floor of the mouth carcinoma.

The platelet human leucocyte antigen (HLA) gene bank contains the genetic information of HLA loci in a large number of blood donors. Currently, the most effective treatment for platelet transfusion refractoriness (PTR) is to evaluate the probability of antigenic mismatch by HLA genotyping of patients and to select HLA-matched donors in the gene bank through an information system. This case report describes the treatment of PTR in patients with malignant tumors using the platelet HLA gene bank digital matching technique.

Article Synopsis
  • - The study created a detailed 3D model of the lower lumbar spine (L3-S1) to analyze how different fixation methods affect the stability and stress during oblique lumbar interbody fusion (OLIF).
  • - Four surgical models were tested: Stand-alone (SA), unilateral (UCBT), bilateral cortical bone trajectory (BCBT), and bilateral pedicle screws (BPS), with BCBT showing the least range of motion, indicating better stability, while SA had the highest ROM and cage stress.
  • - Overall, using OLIF with internal fixation increased segment stability and reduced stress on cages, with BPS being particularly good for preventing damage and reducing risks of complications, while BCBT effectively maintained stability in

Article Synopsis
  • The study investigates the long-term survival outcomes of patients with solitary hepatocellular carcinoma (HCC) less than or equal to 5 cm who underwent radiofrequency ablation (RFA) compared to other treatment options like hepatectomy, radiotherapy, and chemotherapy.
  • Using the SEER database from 2004 to 2017, researchers analyzed overall survival (OS) and cancer-specific survival (CSS) rates, finding that RFA had better survival outcomes than radiotherapy, chemotherapy, and control groups but lower than hepatectomy.
  • The study also developed prognostic models using factors such as age and cirrhosis status to predict survival rates over 1, 3, and 5 years, demonstrating the

Cancer-related anemia (CRA) is a common comorbidity in cancer patients, and it can lead to a worse prognosis. The aim of this cross-sectional study is to investigate the clinical value of the red cell size factor (Rsf) in the early diagnosis of nondigestive tract CRA. A total of 231 patients with nondigestive tract solid cancer were included, and they were divided into anemic and nonanemic subjects according to the hemoglobin (Hb) levels.

Article Synopsis
  • A study was conducted on 409 MDS patients receiving hypomethylating agents (HMA) at 45 hospitals in Zhejiang province to assess their efficacy and safety, focusing on response rates and survival outcomes.
  • The findings revealed a complete remission (CR) rate of 33.98% and an overall response rate (ORR) of 77.02%, with factors like complex karyotype and TP53 mutations impacting the prognosis.
  • No significant differences were observed between dosages of HMA, but patients treated with decitabine had a longer median overall survival of 29.53 months compared to 20.17 months with azacitidine, though the incidence of side effects varied between the two treatments.

  • The study compares the clinical and radiological outcomes of modified mini-open TLIF (mMO-TLIF) via a midline incision versus minimal invasive TLIF (MIS-TLIF) using the Wiltse approach in patients with lumbar degenerative diseases.
  • A total of 60 patients were analyzed, with both groups showing significant improvements in disability and pain scores post-operation, though no significant differences were found between the two surgical techniques.
  • At the 2-year follow-up, both methods demonstrated high fusion rates over 90%, indicating that mMO-TLIF can achieve results comparable to MIS-TLIF for treating lumbar degenerative conditions.

A portent of catastrophic carbon dioxide embolism in laparoscopic hepatectomy: A case report.

Medicine (Baltimore)

June 2024

Department of Anesthesiology, the First Affiliated Hospital, Zhejiang University School of Medicine, Hangzhou, Zhejiang Province, China.

Introduction: Laparoscopic hepatectomy (LH) poses a high risk of carbon dioxide embolism due to extensive hepatic transection, long surgery duration, and dissection of the large hepatic veins or vena cava.

Patient Concerns: A 65-year-old man was scheduled to undergo LH. Following intraperitoneal carbon dioxide (CO2) insufflation and hepatic portal occlusion, the patient developed severe hemodynamic collapse accompanied by a decrease in the pulse oxygen saturation (SpO2).

Background: Mechanical thrombectomy has emerged as the primary endovascular treatment for acute ischemic stroke. Numerous studies have investigated the relationship between thrombus composition and factors such as pharmacological thrombolysis, stroke etiology, mechanical thrombectomy, and radiological imaging. However, limited research has explored the association between thrombus composition and clinical outcomes.

Patients with sepsis are often complicated by acute kidney injury (AKI), which greatly increases mortality. In this study, our purpose was to explore the expression and function of CDGSH iron sulfur domain 2 (CISD2) in septic AKI, and the underlying molecular mechanism. Western blot and quantitative real-time polymerase chain reaction (RT-PCR) were employed to detect protein and mRNA levels in cells.

This study utilizes mechanochemistry to prepare retinol acetate (RA) solid dispersion (RA-sodium starch octenyl succinate (SSOS)), resulting in improved solubility, stability, and bioavailability compared with raw RA and commercial RA microcapsules. RA, poloxamer 188, SSOS, and milling beads (8 mm) were mixed in a ratio of 2:1:8:220 (w/w) and ball-milled at 100 rpm for 3 h. RA-SSOS exhibited a solubility of 1020.
