Article Synopsis
  • A comprehensive scale was developed to measure the burdens of three types of treatment: expected, unexpected complications, and ancillary procedure needs.
  • Expert panels assessed the burden of treatments in urology cases, calculating a burden score (BS) for different treatment options including partial nephrectomy, percutaneous ablation, radical cystectomy, and others.
  • Results indicated that less invasive treatments like percutaneous ablation and trimodal therapy had similar success rates as more invasive options, but with significantly lower burden scores, aiding in patient consultations on treatment choices.

The use of SGLT2 inhibitors (SGLT2is) has been found in large clinical studies to slow the progression of chronic kidney disease (CKD) and to lower the risk of acute kidney injury (AKI). Recent reports suggest that SGLT2is may also reduce the likelihood of developing radiocontrast-associated nephropathy (CAN) following contrast-enhanced imaging and intravascular interventions. This review underscores potential pitfalls and confounders in these studies and calls for caution in adopting their conclusions regarding the safety and renoprotective potency of SGLT2is, in particular in patients at high risk, with advanced CKD and hemodynamic instability undergoing coronary intervention.

Article Synopsis
  • Clonal hematopoiesis of indeterminate potential (CHIP) is linked to a higher risk of cardiovascular (CV) disease in general populations, but its impact on patients with existing CV conditions in clinical trials was unclear.
  • A study of 63,700 patients from five randomized CV therapy trials found that while CHIP+ patients had a 30% higher risk of a first myocardial infarction (heart attack), their risk for recurrent heart attacks showed no significant increase.
  • The research also indicated that CHIP does not significantly affect the effectiveness of standard CV treatments, suggesting that both CHIP+ and CHIP- patients benefit similarly from these therapies.

The central role of natriuretic peptides (NPs) in the complex cardio-renal integrated physiology and organ failure has been revealed over the last four decades. Atrial natriuretic peptide (ANP), the oldest representative of the NPs family, is produced through conversion of proANP to the mature peptide by corin, a trans-membrane protease localized to the cardiac myocyte membrane. Similarly, brain natriuretic peptide (BNP) is generated by furin, which cleaves proBNP to BNP in myocytes.

Department of Medicine, University Medical Centre Hamburg-Eppendorf, 20246 Hamburg, Germany; European Reference Network for Hepatological Diseases (ERN RARE-LIVER), 20246 Hamburg, Germany; Hamburg Centre for Translational Immunology, University Medical Centre Hamburg-Eppendorf, 20246 Hamburg, Germany; Martin Zeitz Centre for Rare Diseases, University Medical Centre Hamburg-Eppendorf, 20246 Hamburg, Germany. Electronic address:

Primary sclerosing cholangitis (PSC) is an immune-mediated liver disease of unknown pathogenesis, with a high risk to develop cirrhosis and malignancies. Functional dysregulation of T cells and association with genetic polymorphisms in T cell-related genes were previously reported for PSC. Here, we genotyped a representative PSC cohort for several disease-associated risk loci and identified rs56258221 (BACH2/MIR4464) to correlate with not only the peripheral blood T cell immunophenotype but also the functional capacities of naive CD4 T (CD4 T) cells in people with PSC.

Cancer is a consequence of stochastic (mutations, genetic, and epigenetic instabilities) and deterministic (evolutionary bottlenecks) events. Stochastic events are less amenable to prediction, whereas deterministic events yield more predictable results. The relative contribution of these opposing forces determines cancer predictability, which affects the accuracy of our prognostic predictions and is critical for treatment planning.
