Comparison of long-term clinical outcome after endovascular versus neurosurgical treatment of ruptured intracranial anterior circulation aneurysms: A single-centre experience.

Brain Spine

July 2024

Department of Neurosurgery, University Neurosurgical Centre Holland, Leiden University Medical Centre, Haaglanden Medical Centre, and Haga Teaching Hospital, Leiden University, The Hague, the Netherlands.

Introduction: In patients with anterior circulation aneurysmal Subarachnoid Haemorrhage (aSAH), endovascular coiling is currently practiced more frequently than neurosurgical clipping. However, despite multiple previous studies, it is still uncertain whether coiling is favourable in terms of long-term clinical outcome.

Research Question: What is the effect of clipping versus coiling on long-term functional outcome of patients with an aSAH?

Material And Methods: All anterior circulation aSAH patients (2012-2015) treated with clipping or coiling in two hospitals in the Netherlands were studied up to five years after treatment.

Article Synopsis
  • Medication non-adherence in COPD patients is a pressing issue, prompting interest in simpler treatment options like single-inhaler triple therapy (SITT) over multi-inhaler triple therapy (MITT) and the use of smart inhalers.
  • A literature review examining adherence levels between SITT and MITT found that most observational trials favored SITT for better adherence and clinical outcomes, while smart inhalers generally showed a positive but varied impact.
  • The TRICOLON trial is a planned study to further investigate the effectiveness of SITT and smart inhalers on improving adherence among COPD patients, involving 300 participants across ten hospitals.

Lymphovascular space invasion (LVSI) is an important prognostic parameter in endometrial carcinoma (EC) and has gained increasing interest in recent years due to an expanding body of evidence of its independent prognostic value, especially when the presence of LVSI is quantified. A key strength of LVSI as a prognostic factor is that it can be detected on routine microscopic examination, without ancillary tests, and thus can be used in low-resource settings. A weakness, however, is the lack of uniformly applied criteria for assessment and quantification of LVSI, resulting in interobserver variation in diagnosis.

Background And Objectives: The results of the ULTRA trial showed that ultra-early and short-term treatment with tranexamic acid (TXA) does not improve clinical outcome after aneurysmal subarachnoid hemorrhage (aSAH). Possibly, the lack of a beneficial effect in all patients with aSAH is masked by antagonistic effects of TXA in certain subgroups. In this post hoc subgroup analysis, we investigated the effect of TXA on clinical outcome in patients with good-grade and poor-grade aSAH.
