Background: Eosinophilic granulomatosis with polyangiitis (EGPA) is an extremely rare type of vasculitis characterized by inflammation within small blood vessels or tissues that may cause damage to the lungs, heart, kidneys, and other organs. Here, we present a rare case of EGPA with cardiac involvement that presented with acute heart failure.

Clinical Findings: A 44-year-old woman with a history of bronchial asthma and sinusitis presented with fever, shortness of breath, fatigue, unintentional weight loss, and polyarthritis.

Nonsurgical Treatment of Splenic Abscesses in Patient With Simultaneous Diagnosis of Endocarditis and Crohn's Disease: A Case Report and Literature Review.

Background: The incidence of multiple pregnancies has increased worldwide recently and women with a twin pregnancy are at higher risk of adverse outcomes compared with women with a singleton pregnancy. It is important to understand the risk factors for adverse fetal outcomes in twin pregnancy in order to guide clinical management.

Aim: To identify the independent risk factors, including maternal personal and family medical histories and first trimester ultrasound screening findings, for adverse fetal outcomes of twin pregnancy before 28 weeks of gestation.

Elevated serum CA 19-9 level mimicking pancreaticobiliary carcinoma from a hepatic abscess: case report and literature review.

Serum levels of the tumor marker CA 19-9 are widely utilized in the diagnosis and monitoring pancreatic and biliary malignancies. However, serum levels of CA 19-9 have also been reportedly elevated in non-malignant conditions. Here, we present the rare case of a 65-year-old woman with a history of gallbladder malignancy who was found to have a new hepatic lesion on surveillance CT with an associated elevation in CA 19-9 to 5,866 U/mL.

Ocular inoculation of toad venom: toxic cataract and proteomic profiling.

Purpose: To report a singular case of cataract caused by toad venom inoculation and to scrutinize the pathological mechanisms through proteomic sequencing of the lens specimen.

Methods: A young Chinese male presented with progressively deteriorating vision in his right eye subsequent to a history of toad venom inoculation. He was diagnosed with a toxic cataract, and underwent phacoemulsification cataract surgery.

Long-Term Opioid Use After Free Flap Breast Reconstruction: Incidence and Associated Factors.

Opioid dependence can occur in 6% to 10% of patients undergoing breast reconstruction. With the expansion of interdisciplinary initiatives to decrease opioid use after surgery, an updated look at the incidence of and risk factors for prolonged opioid dependence after free flap breast reconstruction is essential. We retrospectively identified all cases of free flap breast reconstruction completed at our institution from 2017 to 2020.

Traditional and Non-Traditional Risk Factors of Acute Coronary Syndrome in Young Women: Evidence from the ANCORS-YW Study.

Maternal History of Child Maltreatment Predicts Dysregulated Offspring Stress Response System Functioning.

Identifying proximal and multigenerational distal risk mechanisms through which adversity exposure may shape neuroendocrine dysregulation among children is critical to advancing effective preventive interventions for adversity-exposed individuals. Utilizing longitudinal data ( = 247), the current study examined maternal and offspring history of childhood maltreatment (CM) as predictors of offspring cortisol/DHEA ratios, and, in exploratory analyses, extended this longitudinally to offspring depressive symptoms in young adulthood. Youth (ages 8-13 years) initially attended a research camp, then were followed up approximately eight years later (ages 18-22 years).

Pollination ecotypes and the origin of plant species.

Ecological niche shifts are a key driver of phenotypic divergence and contribute to isolating barriers among lineages. For many groups of organisms, the history of these shifts and associated trait-environment correlations are well-documented at the macroevolutionary level. Globally, many species are threatened by population decline because of anthropogenic changes leading to population fragmentation, genetic isolation and inbreeding depression. Genetic rescue, the controlled introduction of genetic variation, is a method used to relieve such effects in small populations. However, without understanding how the characteristics of rescuers impact rescue attempts interventions run the risk of being sub-optimal, or even counterproductive.

Forecasting sea otter recolonization: insights from isotopic analysis of modern and zooarchaeological populations.

Retrospective datasets offer essential context for conservation by revealing species' ecological roles before industrial-era human impacts. We analysed isotopic compositions of pre-industrial and modern sea otters () to reconstruct pre-extirpation ecology and offer insights for management. Our study focuses on southeast Alaska (SEAK), where sea otters are recolonizing, and northern Oregon, where translocations are being considered.

Germline BRCA testing in Denmark following invasive breast cancer: Progress since 2000.

Background And Purpose: Despite advancements in genetic testing and expanded eligibility criteria, underutilisation of germline testing for pathogenic variants in BRCA1 and BRCA2 (BRCA) remains evident among breast cancer (BC) patients. This observational cohort study presents real-world data on BRCA testing within the context of clinical practice challenges, including incomplete family history and under-referral.

Material And Methods: From the Danish Breast Cancer Group (DBCG) clinical database, we included 65,117 females with unilateral stage I-III BC diagnosed in 2000-2017, of whom 9,125 (14%) were BRCA tested.

Paton Prize Lecture Winner 2024: Gordon Holmes and the Irish spirit of adventure: Lessons for modern thinkers.

This lecture is given in honour of Sir William Paton (1917-1993), physiologist, pharmacologist and Fellow of the Royal Society. His passion for the history of medicine led to generous donations to the Society, who consequently founded the Paton Prize Fund for historical research. After his death, this eponymous Prize Lecture was debuted in 1994.

Increased sodium consumption is associated with psoriasis: A population-based cohort study.

Background: Sodium is stored in skin and may trigger or perpetuate autoimmune diseases including psoriasis. One previous study found skin sodium was elevated in a small group of patients with severe psoriasis compared to healthy controls, but the relationship between sodium intake and psoriasis within a population has not been investigated.

Objectives: To identify whether dietary sodium intake is associated with psoriasis and whether there are subgroups of individuals more likely to have salt-sensitive psoriasis.

Behavioral Cetology in the 19th Century: Thomas Beale, Henry Cheever, Thomas Southwell and Their Role in Awakening a Cetacean Protection Consciousness.

Although naturalists have devoted attention to cetaceans since Antiquity, it was only in the 19th century that cetology underwent a true explosion. Three key cetological works of this period are The Natural History of the Sperm Whale (1839) by Thomas Beale, The Whaleman's Adventures in the Southern Ocean (1850) by Henry Cheever and The Seals and Whales of the British Seas (1881) by Thomas Southwell. Importantly, these three works did not only represent fundamental compendia of scientific knowledge of cetaceans, but also had a crucial role in awakening a cetacean protection consciousness.

The Electrochemical Iodination of Electron-Deficient Arenes.

The iodination of electron-deficient arenes and heteroarenes is a long-standing problem in organic synthesis. Herein we describe the electrochemical iodination in nitromethane with BuNI as iodine source and supporting electrolyte under Lewis acid-free conditions in the presence of small amounts of chloride anions. The electrochemically generated reagent could be applied for the iodination of halogenated arenes, aromatic aldehydes, acids, esters, ketones, as well as nitroarenes to afford the products in good to excellent yields.

Optimizing Prostate Cancer Diagnostic Work-Up Through Micro-Ultrasound: Minimizing Unnecessary Procedures and Reducing Overdiagnoses.

Introduction: We aim to critically assess Microultrasound (mUS) clinical performance in an outpatient setting, focusing on its ability to reduce unnecessary diagnostic procedures, potentially reshape prostate cancer (PCa) diagnostic protocols, and increase the ability to rule out clinically significant (Gleason Score ≥ 3 + 4) PCa (csPCa).

Materials And Methods: Between November 2018 and April 2022, we conducted a prospective study involving men who underwent mUS examination due to clinical symptoms, PSA elevation, or opportunistic early detection of PCa. Experienced urologists performed mUS assessments in an outpatient setting using the prostate risk identification using micro-ultrasound (PRI-MUS) protocol to identify lesions suspicious of csPCa (PRI-MUS score ≥ 3).

Armenia and Türkiye between conflict and cooperation: explaining (with) disaster diplomacy.

Over the past 30 years, the relationship between the neighbouring states of Armenia and Türkiye has been greatly influenced by a politicised atmosphere and ongoing debates about their shared history, alongside efforts at reconciliation. It is noteworthy that, in a global context increasingly characterised by both anthropogenic and natural calamities, the phenomenon of (shared) disasters as it pertains to the dynamics of Armenia-Türkiye relations has garnered relatively scant scholarly attention. This study investigates instances of the COVID-19 (coronavirus disease 2019) pandemic on a global scale and the Kahramanmaraş (Türkiye) earthquakes that occurred on 6 February 2023, which serve as disasters possessing diplomatic and symbolic significance that could facilitate a transition from a state of conflict to one of cooperative peace between these two nations.

Efficient X-ray dark field contrast simulations using a condensed history approach.

Due to the growing interest in X-ray phase contrast imaging (XPCI), the development of XPCI simulators has become an active field of research. Efficient computer simulations are a prime tool for optimizing the XPCI setup and acquisition process. In addition to phase contrast, modern XPCI setups provide insight into the unresolvable sample micro structure distribution through the dark field signal.

Submucosal Microbiome Profiles in Paired and Unpaired Samples From Healthy and Peri-Implantitis Dental Implants.

Background: This cross-sectional study aimed to compare the composition of the submucosal microbiome of peri-implantitis with paired and unpaired healthy implant samples.

Methods: We evaluated submucosal plaque samples obtained in 39 cases, including 13 cases of peri-implantitis, 13 cases involving healthy implants from the same patient (paired samples), and 13 cases involving healthy implants from different individuals (unpaired samples). The patients were evaluated using next-generation genomic sequencing (Illumina) based on 16S rRNA gene amplification.

Dissolved organic carbon from "white" waters in Amazon basin (Brazil) and the osmoregulatory responses of tambaqui (Colossoma macropomum) in neutral and acidic environments.

Optical characterization of dissolved organic carbon (DOC) freshly collected from the circumneutral "white water" of the Rio Solimoes revealed that it had lower aromaticity, lower molecular weight, and a greater autochthonous content than DOC from the acidic "black water" of the Rio Negro. The tambaqui (Colossoma macropomum), a characid member of the Serrasalmidae, is a model neotropical fish that migrates annually between the two rivers. We analysed ionoregulatory responses of the tambaqui over 24 h in ion-poor water at pH 7.

Risk Factors and Predictors for Persistent Dyspnea Post-COVID-19: A Systematic Review.

The most frequently reported post-coronavirus disease of 2019 (COVID-19) symptoms include shortness of breath, fatigue, and cognitive disturbances, with reports of persistent dyspnea ranging between 26% and 41%. There is an urgent need to understand the risk factors and predictors for persistent COVID-19 dyspnea in individuals at all levels of COVID-19 illness severity, to enable the implementation of targeted interventions for those likely to be most affected with persistent dyspnea. Thus, the purpose of this systematic review is to explore the risk factors and predictors that are associated with persistent dyspnea in the post-COVID-19 population.
