Article Synopsis
  • Diagnosing acute heart failure (AHF) in elderly patients with acute dyspnea is challenging, prompting the study of various biomarkers for improved accuracy.
  • A multicenter study in France assessed the effectiveness of five biomarkers (NT-proBNP, hs-cTnI, ST2, galectin-3, CD146) in diagnosing AHF among 238 elderly patients over a four-year period.
  • While individual biomarker accuracies varied, NT-proBNP alone provided the best diagnostic accuracy (0.72), and combining the biomarkers did not significantly improve upon this.

Article Synopsis
  • Acute heart failure (AHF) is a complex condition influenced by both sudden triggers and existing heart issues, often linked to valvular heart disease (VHD).
  • Patients with AHF may show varying symptoms, ranging from mild heart failure to severe cases like cardiogenic shock, making it tough to assess their condition accurately.
  • Current treatment guidelines for AHF patients with severe VHD are unclear due to limited evidence from clinical trials, prompting a need for more research on this specific group.

Aims: To evaluate the prevalence and associations of non-cardiac comorbidities (NCCs) with in-hospital and post-discharge outcomes in acute heart failure (AHF) across the ejection fraction (EF) spectrum.

Methods And Results: The 9326 AHF patients from European Society of Cardiology (ESC)-Heart Failure Association (HFA)-EURObservational Research Programme Heart Failure Long-Term Registry had complete information for the following 12 NCCs: anaemia, chronic obstructive pulmonary disease (COPD), diabetes, depression, hepatic dysfunction, renal dysfunction, malignancy, Parkinson's disease, peripheral vascular disease (PVD), rheumatoid arthritis, sleep apnoea, and stroke/transient ischaemic attack (TIA). Patients were classified by number of NCCs (0, 1, 2, 3, and ≥4).

Electronic address:

Objectives: Ceftolozane-tazobactam (C/T) proved its efficacy for the treatment of infections caused by non-carbapenemase producing Pseudomonas aeruginosa and Enterobacterales. Here, we aimed to provide susceptibility data on a large series of Enterobacterales since the revision of EUCAST categorization breakpoints in 2020.

Methods: First, C/T susceptibility was determined on characterized Enterobacterales resistant to third generation cephalosporins (3GCs) (extended spectrum β-lactamase [ESBL] production or different levels of AmpC overexpression) (n = 213) and carbapenem-resistant Enterobacterales (CRE) (n = 259), including 170 carbapenemase producers (CPE).

Purpose: Adrecizumab, a non-neutralizing antibody of adrenomedullin (ADM) was recently investigated regarding its potential to restore endothelial barrier function in septic shock patients with high plasma ADM levels. Circulating dipeptidyl peptidase 3 (cDPP3), a protease involved in the degradation of several cardiovascular mediators, represents another biological pathway strongly associated with outcome in septic shock, although unrelated to ADM. Therefore, the prognosis of patients with elevated cDPP3 may not be influenced by Adrecizumab.

Detection of SARS-CoV-2 in Saliva and Nasopharyngeal Swabs According to Viral Variants.

Microbiol Spectr

December 2022

Laboratoire de Virologie, AP-HP, Hôpital Saint Louis, Paris, France.

Article Synopsis
  • The Omicron variant of SARS-CoV-2 has over 50 mutations that may increase transmission and evade immune responses from vaccines or prior infections.
  • A study compared viral loads in saliva and nasopharyngeal swabs (NPS) during the Alpha, Delta, and Omicron waves, finding lower Ct values in NPS, but higher viral loads in saliva for Omicron.
  • The results suggest that current diagnostic methods may need updates to better detect new variants like Omicron, highlighting the need for refined screening strategies.*

A universal predictive and mechanistic urinary peptide signature in acute kidney injury.

Crit Care

November 2022

Department of Nephrology and Organ Transplantation, University Hospital of Toulouse, and French Intensive Care Renal Network, 31000, Toulouse, France.

Background: The delayed diagnosis of acute kidney injury (AKI) episodes and the lack of specificity of current single AKI biomarkers hamper its management. Urinary peptidome analysis may help to identify early molecular changes in AKI and grasp its complexity to identify potential targetable molecular pathways.

Methods: In derivation and validation cohorts totalizing 1170 major cardiac bypass surgery patients and in an external cohort of 1569 intensive care unit (ICU) patients, a peptide-based score predictive of AKI (7-day KDIGO classification) was developed, validated, and compared to the reference biomarker urinary NGAL and NephroCheck and clinical scores.

Université de Lorraine, CHRU de Nancy, Intensive Care Medicine Babois, INSERM U1116, FCRIN INI-CRCT, Nancy, France.

Background: Impact of in-ICU transfusion on long-term outcomes remains unknown. The purpose of this study was to assess in critical-care survivors the association between in-ICU red blood cells transfusion and 1-year mortality.

Methods: FROG-ICU, a multicenter European study enrolling all-comers critical care patients was analyzed (n = 1551).

Background: Myelodysplastic syndromes (MDS) are clonal hematopoietic diseases of the elderly characterized by chronic cytopenias, ineffective and dysplastic haematopoiesis, recurrent genetic abnormalities and increased risk of progression to acute myeloid leukemia. A challenge of routine laboratory Complete Blood Counts (CBC) is to correctly identify MDS patients while simultaneously avoiding excess smear reviews. To optimize smear review, the latest generations of hematology analyzers provide new cell population data (CPD) parameters with an increased ability to screen MDS, among which the previously described MDS-CBC Score, based on Absolute Neutrophil Count (ANC), structural neutrophil dispersion (Ne-WX) and mean corpuscular volume (MCV).

The overall patient population in contemporary cardiac intensive care units (CICUs) has only increased with respect to patient acuity, complexity, and illness severity. The current population has more cardiac and noncardiac comorbidities, a higher prevalence of multiorgan injury, and consumes more critical care resources than previously. Patients with heart failure (HF) now occupy a large portion of contemporary tertiary or quaternary care CICU beds around the world.

Acute Heart Failure in the 2021 ESC Heart Failure Guidelines: a scientific statement from the Association for Acute CardioVascular Care (ACVC) of the European Society of Cardiology.

Eur Heart J Acute Cardiovasc Care

February 2022

Emergency Department, Hospital Clínic, "Processes and Pathologies, Emergencies Research Group" IDIBAPS, University of Barcelona, Barcelona, Catalonia, Spain.

The current European Society of Cardiology (ESC) Heart Failure Guidelines are the most comprehensive ESC document covering heart failure to date; however, the section focused on acute heart failure remains relatively too concise. Although several topics are more extensively covered than in previous versions, including some specific therapies, monitoring and disposition in the hospital, and the management of cardiogenic shock, the lack of high-quality evidence in acute, emergency, and critical care scenarios, poses a challenge for providing evidence-based recommendations, in particular when by comparison the data for chronic heart failure is so extensive. The paucity of evidence and specific recommendations for the general approach and management of acute heart failure in the emergency department is particularly relevant, because this is the setting where most acute heart failure patients are initially diagnosed and stabilized.

Background: Cardiogenic shock has a high mortality on optimal therapy. Adrenomedullin is released during cardiogenic shock and is involved in its pathophysiological processes. This study assessed treatment with the humanised, monoclonal, non-neutralising, adrenomedullin antibody adrecizumab, increasing circulating concentrations of adrenomedullin in cardiogenic shock.
