We present a unique case of mixed ovarian carcinoma, including both clear cell carcinoma (CCC) and grade 1 endometrioid carcinoma (EC) in a 49-year-old woman. The patient had no complaints of pain but presented with an enlarged abdomen, necessitating a thorough diagnostic assessment. Histopathological evaluation revealed an unusual histological juxtaposition of CCC coexisting with grade 1 EC found in an ovarian mass, resulting in diagnostic and therapeutic challenges.

Renal angiomyolipomas, benign tumors composed of blood vessels, adipose tissue, and smooth muscle, affect approximately 70% to 80% of patients with tuberous sclerosis. Angiomyolipomas smaller than 4 cm are usually asymptomatic, whereas larger ones can cause lumbar pain, anemia, and hematuria. Contrary to its sporadic counterparts, tuberous sclerosis-associated angiomyolipomas often present at a young age, are multicentric and large, and carry a higher risk of life-threatening hemorrhage.

Background: Neovaginal ileitis is an understudied complication of intestinal vaginoplasty for which no evidence-based treatments exist. Mesalamine has been successfully reported to treat neovaginal colitis and has mixed evidence of efficacy in treating ileal inflammatory syndromes.

Case: We report a 29-year-old female with a history of ileal neovaginoplasty at two years of age for cloacal variant and treatment-refractory neovaginal ileitis with pain and bleeding whose symptoms resolved with treatment with topical mesalamine.

Patients excluded were those undergoing repeat laparoscopy, with previously histologically diagnosed endometriosis, asymptomatic endometriosis, an unknown first gynecologic encounter, or an unknown initial BMI.
