Objectives: This scoping review was undertaken to identify risk predictions models and preoperative predictors of surgical site infection (SSI) in adult cardiac surgery. A particular focus was on the identification of novel predictors that could underpin the future development of a risk prediction model to identify individuals at high-risk of SSI, and therefore guide a national SSI prevention strategy.

Methods: A scoping review to systematically identify and map out existing research evidence on preoperative predictors of SSI was conducted in two stages.

Labor Status at Delivery and Lung Function in Extremely Prematurely Born Young Adults.

Pediatr Pulmonol

December 2024

Women and Children's Health, School of Life Course Sciences, Faculty of Life Sciences and Medicine, King's College London, London, UK.

Background: There has been conflicting evidence regarding the impact of mode of delivery on respiratory outcomes in later childhood and adulthood. It is possible labor status, rather than mode of delivery, influences later respiratory morbidity. We hypothesized that extremely premature infants born to mothers in labor would have better lung function at follow-up than those born to mothers not in labor.

Background: The rare incidence of small cell carcinoma of the esophagus (SCCE) makes prospective studies difficult to conduct, the efficacy of existing standard treatment regimens for SCCE is therefore highly controversial. This study aimed to explore differences in the efficacy of three different treatment regimens [upfront surgery, neoadjuvant chemotherapy (NCT), and chemoradiotherapy (CRT)] in patients with limited-stage SCCE (LS-SCCE).

Methods: In total, 483 patients with LS-SCCE were screened from five centers from June 2001 to June 2020, and 128 patients with LS-SCCE were screened from the Surveillance, Epidemiology, and End Results (SEER) database.

Implementing and evaluating shared decision-making before transcatheter aortic valve implantation with a dedicated pathway and questionnaire.

Eur Heart J Open

November 2024

Guy's and St Thomas' NHS Foundation Trust, Harefield Hospital, Department of Cardiology, Hill End Rd, Harefield, Uxbridge UB9 6JH, UK.

Aims: Transcatheter aortic valve implantation (TAVI) is an alternative to surgical aortic valve replacement for patients with aortic valve stenosis. The choice between TAVI, surgery, or a conservative approach should be based upon multiple factors including clinical considerations, technical feasibility, and informed patient preference. In this context, engaging patients in a shared decision-making (SDM) process becomes essential, but this practice is generally underused.

The Bronchodilator and Anti-Inflammatory Effect of Long-Acting Muscarinic Antagonists in Asthma: An EAACI Position Paper.

Allergy

December 2024

Allergy Unit, Hospital Regional Universitario de Malaga, IBIMA-Plataforma BIONAND, RICORS Inflammatory Diseases, Department of Medicine and Dermatology, Universidad de Malaga, Malaga, Spain.

As cholinergic innervation is a major contributor to increased vagal tone and mucus secretion, inhaled long-acting muscarinic antagonists (LAMA) are a pillar for the treatment of chronic obstructive pulmonary disease and asthma. By blocking the muscarinic receptors expressed in the lung, LAMA improve lung function and reduce exacerbations in asthma patients who remained poorly controlled despite treatment with inhaled corticosteroids and long-acting β2 agonists. Asthma guidelines recommend LAMA as a third controller to be added on before the initiation of biologicals.

Article Synopsis
  • The study surveyed pathologists and thoracic surgeons from the IASLC to evaluate their views on the R classification for lung cancer, with a response rate of 72% from 447 professionals.
  • Findings revealed geographic differences in R classification usage, with Europe and Asia often reporting R status, while 70% of North American pathologists rarely do, and varied practices regarding who assigns R categories between regions.
  • The "uncertain resection" category (R(un)) is largely unused globally, highlighting the need for more evidence and standardization in reporting surgical margins and R categories in pathology.

Department of Oral Surgery, Guy's and St Thomas' NHS Foundation Trust, UK.

Myofascial pain represents the largest subgroup of temporomandibular disorders (TMD) that account for a common cause of non-dental orofacial pain. The management of TMD is complex due to the chronic nature of the condition, alongside acute episodes presenting to the clinician. A fundamental part of TMD management is consideration of the biopsychosocial element in its aetiology.
