Sepsis is characterized by a dysregulated and excessive systemic inflammatory response to infection, associated with vascular and metabolic abnormalities that ultimately lead to organ dysfunction. In immune cells, both non-oxidative and oxidative metabolic rates are closely linked to inflammatory responses. Mitochondria play a central role in supporting these cellular processes by utilizing metabolic substrates and synthesizing ATP through oxygen consumption.

In recent years, the diagnostic accuracy of Alzheimer's disease has been enhanced by the development of different types of biomarkers that indicate the presence of neuropathological processes. In addition to improving patient selection for clinical trials, biomarkers can assess the effects of new treatments on pathological processes. However, there is concern about the indiscriminate and poorly supported use of biomarkers, especially in asymptomatic individuals or those with subjective cognitive decline.

Novel therapies for Alzheimer's disease, particularly anti-amyloid drugs like lecanemab and donanemab, have shown modest clinical benefits but also significant risks. The present paper highlights the challenges of access to diagnosis, cost-effectiveness, safety, and the need for more representation of diverse populations in clinical trials. Recommendations include careful patient selection, risk-benefit analysis, and the importance of proven amyloid pathology for treatment.

Hemolytic disease of the fetus and newborn-a perspective of immunohematology.

Hematol Transfus Cell Ther

November 2024

Departamento de Ciências Básicas da Saúde, Universidade Federal de Ciências da Saúde de Porto Alegre (UFCSPA), Porto Alegre, RS, Brazil. Electronic address:

Article Synopsis
  • Hemolytic disease of the fetus and newborn is a serious health issue due to incompatibility between maternal and fetal blood, with no preventive measures for most problematic antibodies.
  • A systematic review of 75 studies revealed that 44 case reports linked the disease to 11 infant deaths, with only a 0.17% alloimmunization rate noted, alongside many intrauterine transfusions.
  • The most common antibodies involved were from the Rh, Kell, and MNS blood systems; further research is needed globally to improve diagnosis and treatment management for at-risk pregnancies.

Radiographic Evaluation of the Surgical Treatment of Pediatric Supracondylar Humeral Fractures.

Rev Bras Ortop (Sao Paulo)

August 2024

Cirurgia de Ombro e Cotovelo, Santa Casa de Misericórdia de Porto Alegre, RS, Brasil.

To perform a radiographic assessment of the quality of supracondylar fracture fixation by identifying the factors that have contributed to inadequate reduction and increased the chance of reduction loss during outpatient follow-up. The variables analyzed were as follows: fracture line, initial displacement, time of day the surgery was performed, and chosen fixation technique.  Review of electronic medical records and radiographic evaluation of supracondylar fractures operated from January 2017 to December 2022.

Department of Physical Therapy, Universidade Federal de Ciências da Saúde de Porto Alegre, Porto Alegre, RS, Brazil.

To evaluate the muscle thickness and prevalence of muscle atrophy of the biceps brachii/brachialis (BB) and quadriceps femoris (QF) in critically ill children using ultrasound (US). The prospective longitudinal study was conducted in the pediatric intensive care unit (PICU) of a tertiary hospital in southern Brazil with children and adolescents of both sexes, aged 1 month to 12 years, on invasive mechanical ventilation for 24 h. US measurements were taken up to 24 h after admission, 72 h after, and weekly until discharge from the PICU.

Precise annotations for large medical image datasets can be time-consuming. Additionally, when dealing with volumetric regions of interest, it is typical to apply segmentation techniques on 2D slices, compromising important information for accurately segmenting 3D structures. This study presents a deep learning pipeline that simultaneously tackles both challenges.

Temperature and humidity are studied in the context of seasonal infections in temperate and tropical zones, but the relationship between viral trends and climate variables in temperate subtropical zones remains underexplored. Our retrospective study analyzes respiratory pathogen incidence and its correlation with climate data in a subtropical zone. Retrospective observational study at Moinhos de Vento Hospital, South Brazil, aiming to assess seasonal trends in respiratory pathogens, correlating them with climate data.

Background: Head elevation is recommended as a tier zero measure to decrease high intracranial pressure (ICP) in neurocritical patients. However, its quantitative effects on cerebral perfusion pressure (CPP), jugular bulb oxygen saturation (SjvO), brain tissue partial pressure of oxygen (PbtO), and arteriovenous difference of oxygen (AVDO) are uncertain. Our objective was to evaluate the effects of head elevation on ICP, CPP, SjvO, PbtO, and AVDO among patients with acute brain injury.

Dopamine agonists are the first line of treatment for patients with symptomatic hyperprolactinemia due to prolactinomas and in those with idiopathic hyperprolactinemia. Treatment with these agents is effective in 80%-90% of the cases. Infertility treatment of patients with hyperprolactinemia is also carried out with dopamine agonists, aiming for the normalization of prolactin levels.

Article Synopsis
  • Hyperprolactinemia can lead to issues like menstrual irregularities, infertility, and other health problems, caused by various factors categorized as physiological, pharmacological, and pathological.
  • It’s crucial to differentiate prolactinomas (tumors producing prolactin) from other conditions that could cause elevated prolactin levels, especially due to pituitary stalk disconnection.
  • The Brazilian Federation of Gynecology and Obstetrics Associations and the Brazilian Society of Endocrinology and Metabolism have outlined guidelines for measuring serum prolactin and investigating hyperprolactinemia, whether it presents symptoms or arises from medications.

Calorie restriction protects against acute systemic LPS-induced inflammation.

Nutr Neurosci

November 2024

Universidade Federal do Rio Grande do Sul (UFRGS), Instituto de Ciências Básicas da Saúde (ICBS), Graduate Program in Biochemistry, Porto Alegre, Brazil.

Caloric restriction (CR) has been proposed as a nutritional strategy to combat chronic diseases, including neurodegenerative diseases, as well as to delay aging. However, despite the benefits of CR, questions remain about its underlying mechanisms and cellular and molecular targets. As inflammatory processes are the basis or accompany chronic diseases and aging, we investigated the protective role of CR in the event of an acute inflammatory stimulus.

Background And Aims: Alagille syndrome (ALGS) is characterized by chronic cholestasis with associated pruritus and extrahepatic anomalies. Maralixibat, an ileal bile acid transporter inhibitor, is an approved pharmacologic therapy for cholestatic pruritus in ALGS. Since long-term placebo-controlled studies are not feasible or ethical in children with rare diseases, a novel approach was taken comparing 6-year outcomes from maralixibat trials with an aligned and harmonized natural history cohort from the G lobal AL agille A lliance (GALA) study.
