Improving real-world evaluation of patient- and physician-reported tolerability: niraparib for recurrent ovarian cancer (NiQoLe).

JNCI Cancer Spectr

December 2024

Department of Medical and Surgical Oncology & Hematology, Institut of Cancer Strasbourg (ICANS), Strasbourg, France.

Article Synopsis
  • The study focused on individualized dosing of niraparib for patients with platinum-sensitive recurrent ovarian cancer (PSROC), revealing that most patients required treatment modifications due to adverse events (AEs) during the first three months.
  • A significant proportion of patients (62%) experienced AEs, with common issues including fatigue, insomnia, and thrombocytopenia, highlighting the physical and emotional burden of the therapy.
  • The findings indicated that physicians often underestimated these symptoms, emphasizing the importance of patient self-reporting for a comprehensive understanding of treatment-related challenges.

Objective: Ampullary neoplastic lesions can be resected by endoscopic papillectomy (EP) or transduodenal surgical ampullectomy (TSA) while pancreaticoduodenectomy is reserved for more advanced lesions. We present the largest retrospective comparative study analysing EP and TSA.

Design: Of all patients in the database, lesions with prior interventions, benign histology advanced malignancy (T2 and more), patients with hereditary syndromes and those undergoing pancreatoduodenectomy were excluded.

Article Synopsis
  • The study investigates the relationship between hip mobility and spinopelvic tilt, finding that higher hip movement (∆PFA ≥ 95°) significantly increases the risk of adverse spinopelvic mobility when transitioning from standing to seated positions.
  • 337 patients undergoing total hip arthroplasty (THA) were analyzed using x-rays to measure spinopelvic tilt (∆SPT) and pelvic femoral angle (∆PFA), demonstrating distinct differences in these measurements based on patient mobility levels.
  • Results indicate that patients with high hip mobility are at a much greater risk of experiencing significant spinopelvic mobility issues, suggesting the importance of assessing hip function before surgery to manage potential complications.

Background: The PEXIVAS (Plasma exchange and glucocorticoids in severe antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis) trial showed that a reduced-dose glucocorticoid regimen (redGC) was non-inferior to a standard-dose regimen (standGC) with respect to death or end-stage kidney disease (ESKD) in patients with ANCA-associated vasculitis (AAV). However, the primary endpoint did not include disease progression or relapse, cyclophosphamide was the main induction therapy and rituximab (RTX)-treated patients tended to have a higher risk of death or ESKD with redGC. We aimed to evaluate the real-world use of redGC.

Introduction: Dupuytren's disease is a benign disorder leading to flexion contracture of the fingers and functional disability. Many treatments have been described. Open fasciectomy is the gold standard; however percutaneous needle fasciotomy (PNF) is a reliable option for uncomplicated primary contracture but it has a high rate of recurrence.

Low-dose methadone added to another opioid for cancer pain: a multicentre prospective study.

Support Care Cancer

October 2024

AP-HP, Groupe Hospitalier Universitaire APHP-Sorbonne Université, Service Soins Palliatifs, Accompagnement Et Soins de Support, 47-83 Bd de L'Hôpital, 75013, Paris, France.

Article Synopsis
  • * Conducted at 19 pain and palliative care centers, the study found significant reductions in pain ratings from baseline to day 14, indicating that patients experienced relief.
  • * The results showed that half of the participants responded positively to the treatment, with no serious adverse effects reported, supporting the idea of using low-dose methadone as a helpful addition to pain management strategies for cancer patients.

[Metastatic castration-resistant prostate cancer and PARP inhibitors: From tumor genomics to new therapeutic combinations].

Bull Cancer

January 2025

IUCT Oncopôle Toulouse, service d'urologie, Toulouse, France; Clinique La Croix du Sud, UROSUD, Toulouse, France.

Castration-resistant metastatic prostate cancer remains lethal and a therapeutic challenge. Current strategies are geared towards the personalization of treatments based on the identification of relevant molecular targets, including genomic alterations involved in tumoral processes. Among these novel targeted therapies, poly-ADP-ribose polymerase inhibitors (PARPi), by blocking the action of enzymes involved in deoxyribonucleic acid (DNA) repair, induce the destruction of cells carrying defects in homologous recombination repair, often associated with alterations in genes involved in this mechanism.

Article Synopsis
  • - Ovarian cancer is a major cause of death among gynecological cancers, and platinum-based chemotherapy is the primary treatment method, along with maintenance strategies to extend chemotherapy intervals.
  • - Current maintenance treatments include bevacizumab and poly ADP-ribose polymerase inhibitors, but these can only be used once during the disease's progression, leaving a gap in treatment for patients experiencing relapses.
  • - A new international Phase II trial is exploring a combination of a therapeutic cancer vaccine (OSE2101) and anti-PD1 (pembrolizumab) as a maintenance therapy for patients with platinum-sensitive recurrences, regardless of prior treatments.

We assessed cumulative detection and determinants of anal high-grade squamous intraepithelial lesions (HSILs) in men who have sex with men living with human immunodeficiency virus and who underwent 3 visits over 2 years, with cytology and high-resolution anoscopy, within the ANRS-EP57-APACHES study. The cumulative HSIL detection rate was 33% (134 of 410), of which 48% HSILs were detected at baseline. HSIL detection varied considerably by center (from 13% to 51%).

Introduction: The position of the pelvis in the sagittal plane can vary considerably between different functional positions. Adapting the position of the acetabular cup in relation to the alignment between the spine and the hip of each individual, prior to prosthesis placement, can prevent the risk of prosthetic impingement. Taken individually, risk factors for unfavorable spinopelvic kinematics can be difficult to interpret when trying to precisely predict which patients are at risk.
