Background: Observational studies have suggested an association between thyroid volume changes and thyroid disease, but the causal relationship and direction of these effects remain unclear. This study employs a two-sample Mendelian randomization (MR) approach to assess the effect of thyroid volume on clinically common benign and malignant thyroid diseases.

Methods: Summary data from genome-wide association studies (GWAS) were utilized for secondary data analysis to investigate the link between thyroid volume and disease.

December 2024

Otolaryngology-Head and Neck Surgery, King Saud Bin Abdulaziz University for Health Sciences, King Abdullah International Medical Research Center, Ministry of the National Guard Health Affairs, Jeddah, Kingdom of Saudi Arabia.

Unlabelled: Thyroid follicular nodular disease (non-toxic multinodular goiter), the commonest thyroid disorder, can be managed with different surgical methods, including total thyroidectomy (TT), near-total thyroidectomy (NTT), subtotal thyroidectomy (STT), and Dunhill operation (DO). This systematic review and meta-analysis aimed to provide an update on the role of TT versus NTT, STT, or DO in MNG concerning goiter recurrence, thyroid cancer incidence, and reported complications. We conducted a systematic literature search in Medline, EMBASE, and CENTRAL.

Article Synopsis
  • KEAP1 is a protein that usually promotes the degradation of NRF2, a factor that helps cells respond to oxidative stress, but mutations in KEAP1 can lead to diseases like familial multinodular goiter.
  • This study analyzed 39 patients with this goiter, identifying five with specific KEAP1 mutations that did not correlate with any other mutations related to goiter.
  • Results showed that these mutations led to increased activity of NRF2, causing thyroid hyperplasia and a protective response against oxidative stress, indicated by the upregulation of antioxidant and detoxification genes.

  • The study investigates the rising rate of incidental thyroid malignancy (ITM) found in patients undergoing surgery for benign thyroid disease (BTD) without preoperative concerns about cancer.
  • An analysis of 2,528 patients revealed that 22.9% had ITM, with the majority being women and having a mean BMI of 29.6.
  • The findings suggest a significant association between higher BMI and the occurrence of ITM, indicating that patients with higher BMI and BTD may need closer monitoring.

  • Multinodular goiter (MNG) is a common thyroid condition with multiple nodules, often benign but sometimes harboring latent cancer, necessitating thorough investigation and management.* -
  • A study of 412 patients who underwent total thyroidectomy revealed that 14.02% had concealed thyroid carcinoma within MNG, predominantly of the papillary type.* -
  • The findings highlight the need for careful evaluation of patients with MNG due to the potential for malignancy, particularly in cases that may also involve other thyroid disorders like Hashimoto's thyroiditis.*

Introduction: rhTSH-assisted radioiodine therapy of multinodular goiter is not fully known and only a few studies, with a limited number of patients have evaluated the effect of rhTSH assisted radioiodine therapy beyond 1 year. Though there is an effective and safe management of benign non-toxic MNG available, it is not applicable to all patient categories in Kuwait covering the impact of the past environmental events (Gulf War) and the genetic relation. The proposed project aims to address those points raised, that is exclusive to the Kuwait population.

Tracheomalacia following a total thyroidectomy in a patient with a large non-toxic goiter: A case report.

Int J Surg Case Rep

March 2024

Surgery Department, Medical Faculty of Universitas Sebelas Maret, Dr. Moewardi Hospital, Surakarta, Indonesia.

Introduction: Nontoxic nodular goiter is one of the most prevalent thyroid conditions worldwide. Thyroidectomy for large goiters has a relatively high risk of postoperative airway obstruction, with tracheomalacia being one of the potential complications.

Case Report: A 61-year-old woman complained of a lump in her neck for 45 years.

Article Synopsis
  • Unsuspected thyroid cancer can be found in multinodular goiter (MNG) with a malignancy risk of 7-9%.
  • Fine needle aspiration (FNAc) is used if there are suspicious ultrasound findings, and benign results usually don't require surgery unless there are pressure symptoms or cosmetic issues.
  • The case presented involves a patient who unexpectedly had papillary thyroid cancer detected during a thyroid scan.

Phospholipase C-γ as a Potential Therapeutic Target for Graves' Orbitopathy.

Endocrinol Metab (Seoul)

December 2023

Department of Ophthalmology, Severance Hospital, Institute of Vision Research, Yonsei University College of Medicine, Seoul, Korea.

Article Synopsis
  • Phospholipase C-γ (PLC-γ) is important for immune responses and is linked to inflammatory disorders; this study focused on its role in Graves' orbitopathy (GO) and tested a specific inhibitor, U73122.
  • The researchers found that PLCG1 and PLCG2 were expressed more in GO tissues compared to normal ones, and that U73122 effectively reduced inflammation markers in GO fibroblasts and normal fibroblasts.
  • The study concludes that inhibiting PLC-γ with U73122 may lower inflammatory responses and suggests PLC-γ as a potential therapeutic target for treating Graves' orbitopathy.

Borderline personality disorder and thyroid diseases: a Mendelian randomization study.

Front Endocrinol (Lausanne)

October 2023

Department of Thyropathy, Sunsimiao Hospital, Beijing University of Chinese Medicine, Tongchuan, Shanxi, China.

Article Synopsis
  • - The study investigated the potential causal relationship between borderline personality disorder (BPD) and four thyroid diseases using a method called Mendelian randomization analysis.
  • - Results indicated that individuals with BPD are 1.13 times more likely to develop non-toxic single thyroid nodules, but no significant link was found between BPD and other thyroid conditions like hyperthyroidism or hypothyroidism.
  • - The findings suggest that monitoring thyroid health is important for patients with BPD, but more research is needed to explore connections between other mental illnesses and thyroid diseases.

Article Synopsis
  • - A recent meta-analysis examined the outcomes of total thyroidectomy (TT) versus less than total thyroidectomy (LTT) in treating benign multinodular non-toxic goiter, utilizing data from five randomized controlled trials (RCTs).
  • - TT showed a lower recurrence rate for goiter compared to LTT, but adverse events like nerve damage and permanent parathyroid issues were similar in both groups, except for temporary hypoparathyroidism being more common with TT.
  • - The analysis highlighted concerns about bias in the studies reviewed and concluded there was no definitive advantage or disadvantage of TT over LTT concerning recurrence and re-operation rates, with overall evidence quality rated as low to moderate.
