December 2024

From the Department of Emergency Medicine, University of Health Sciences, Haseki Research and Training Hospital, Istanbul, Türkiye.

Objectives: The BIG score (base deficit + [2.5 × international normalized ratio] + [15 - Glasgow Coma Score]) was compared with the Pediatric Trauma Score (PTS) for predicting mortality in pediatric patients with multiple trauma.

Methods: This retrospective, single-center study included 318 consecutive pediatric patients (aged 1-18 years) with multiple trauma who were admitted to the emergency department between January 1, 2021, and December 31, 2023.

Background/aim: The most common and often first metastatic site of colorectal cancer (CRC) is the liver, and radiological modalities have a critical role in the diagnosis of colorectal liver metastasis (CRLM). In this study, the possible relationship between portal vein diameter, number of metastases, and metastasis diameter was evaluated in CRLM patients who underwent computed tomography (CT) examination with intravenous contrast (IV).

Patients And Methods: Cases diagnosed with CRLM who underwent abdominal CT examination with IV contrast between December 2020 and January 2024 were retrospectively scanned.

Trocar-site Herniation after Laparoscopic Myomectomy via Culdotomy Extraction.

Gynecol Minim Invasive Ther

July 2024

Department of Obstetrics and Gynecology, Diyarbakir Gazi Yasargil Research and Training Hospital, University of Health Sciences, Diyarbakir, Turkey.

Trocar-site hernia is a rare and life-threatening complication of laparoscopic abdominal surgery; 5-mm trocar-site bowel herniation is rarer than ≥ 10-mm trocar-site herniation. We present a 41-year-old female patient on the postoperative 6 day with laparoscopic myomectomy admitted to our emergency department with severe vomiting and nausea. At the end of the evaluations, she was diagnosed with 5-mm trocar-site small bowel herniation.

Article Synopsis
  • * An analysis of 829 bladder cancer cases revealed that 80.5% were conventional urothelial carcinoma, while 19.5% had variant histology, with squamous differentiation being the most common subtype in the latter.
  • * Key factors like tumor grade and invasion levels differed significantly between the variant and conventional groups, affecting diagnostic outcomes, but demographic factors like age and gender showed no significant differences.

Article Synopsis
  • - The study investigates the link between altered sense of smell and protein-energy wasting (PEW) in patients with chronic kidney disease (CKD), particularly focusing on those with moderate-to-advanced stages and those undergoing maintenance hemodialysis (MHD).
  • - Researchers tested the smell abilities of 139 individuals using an odor identification test and assessed their nutritional status with a malnutrition inflammation score (MIS); results showed CKD patients had poorer odor scores and higher inflammation indicators compared to healthy controls.
  • - Although advanced CKD patients had significantly impaired olfactory function, the study found no strong statistical connection between their smell abilities and the severity of PEW, indicating that while both issues are prevalent, they may not directly impact each other

Purpose: This study had two main goals: to determine which rhabdomyolysis patients need haemodialysis; and to highlight the significance of blood gas parameters, particularly base excess, as predictors of the need for haemodialysis.

Method: A total of 270 patients were included in this multicentre, retrospective study. Among the patients who were transferred in from the earthquake region and developed rhabdomyolysis, those with creatine kinase (CK) values >1000 U/L were included in our study.

  • The study aimed to assess the status of point-of-care ultrasound (POCUS) usage, perceptions, and educational barriers in pediatric emergency departments (PEDs) and pediatric intensive care units (PICUs) in Turkey.
  • The survey revealed that a significant majority of PEDs (84.2%) and all PICUs (100%) utilize POCUS, mainly for thoracic and cardiovascular assessments, but face challenges like lack of equipment, manpower, and training.
  • Participants emphasized the importance of mandatory POCUS training during fellowship programs to enhance usage and improve patient outcomes in pediatric care.

Developmental and epileptic encephalopathies (DEEs) feature altered brain development, developmental delay and seizures, with seizures exacerbating developmental delay. Here we identify a cohort with biallelic variants in DENND5A, encoding a membrane trafficking protein, and develop animal models with phenotypes like the human syndrome. We demonstrate that DENND5A interacts with Pals1/MUPP1, components of the Crumbs apical polarity complex required for symmetrical division of neural progenitor cells.

Introduction: Contribution of T helper 1 and 2 cells-related cytokines to pathogenesis of myasthenia gravis (MG) is well known. Recently, the contribution of follicular T helper (Tfh) and T helper 17 cells-related molecules to the pathogenesis has gained importance. In this study, we aimed to evaluate the changes in Tfh- and Th17-related molecules before and after rescue therapy in patients with myasthenic crisis (cMG) and to reveal the molecular differences between stable MG and cMG patients.

Background: The main complications seen in patients who have undergone modified radical mastectomy (MRM) are seroma, surgical site infection, hematoma, wound dehiscence, flap necrosis, and nerve damage. While these complications lead to some problems the most feared effect in the early period is that they cause a delay in adjuvant treatment. Incisional Negative Pressure Wound Therapy (iNPWT) decreases wound dehiscence by reducing oedema and tension, especially in the incision line.

Article Synopsis
  • The study aims to evaluate the effectiveness of complete blood count (CBC) parameters and various ratios in distinguishing pulmonary tuberculosis (TB) from community-acquired pneumonia (CAP) in children.
  • It analyzed data from 163 patients and found that certain CBC values and serum electrolyte levels differed significantly between TB and CAP, with CAP showing higher counts for neutrophils, monocytes, and other markers.
  • The findings indicate that specific CBC parameters and ratios, along with C-reactive protein (CRP), can serve as useful and cost-effective diagnostic tools for early differentiation between these two infectious diseases.

  • Abacavir is a commonly used initial HIV treatment that can cause hypersensitivity reactions (HSR) linked to the HLA-B*57:01 allele, which can be serious and limit treatment options.
  • A study in Istanbul involved 867 treatment-naïve HIV patients to assess the prevalence of HLA-B*57:01 and found that 1.6% were positive, with four of those patients switched from abacavir-based therapy.
  • This prevalence data is lower than previous reports (3-3.6%) and suggests that HLA-B*57:01 screening is important for effective and safe HIV treatment planning in Turkey.
