Background: Covering a significant part of a woman's life, the postmenopausal phase is often associated with the onset of obesity, metabolic dysfunction, osteoporosis, and their most disabling complications. In this context, scant evidence from both preclinical and clinical studies suggests that single nucleotide polymorphisms (SNPs) of the follicle-stimulating hormone receptor (FSHR) gene might be involved in the etiopathogenesis of these conditions, posing them as possible molecular predictors of their development. Therefore, this study aimed to evaluate the role of the FSHR gene SNPs c.

J Med Cases

October 2024

Department of Medicine, Division of Nephrology and Hypertension, The Mehandru Center for Innovation in Nephrology, Jersey Shore University Medical Center, Hackensack Meridian School of Medicine, Neptune, NJ, USA.

Article Synopsis
  • - Insertion of a peritoneal dialysis (PD) catheter in patients with end-stage renal disease (ESRD) and liver cirrhosis is difficult, often leading to complications like ascitic fluid leaks that disqualify them from PD.
  • - A case study involving three ESRD patients with tense ascites who underwent large volume fluid drainage alongside hemodialysis found that they had a challenging lifestyle due to multiple medical procedures each week.
  • - To prevent leaks after PD catheter insertion, the study suggests using a continuous drainage system for about two weeks post-surgery before beginning PD training, which resulted in successful healing without complications.

  • Urinary tract infections (UTIs) are common bacterial infections, but varying definitions in research complicate study comparisons, especially in light of rising antibiotic resistance.
  • A recent Delphi consensus study established a new standard definition for UTI that includes symptoms, pyuria, and urine culture results, moving away from the complicated vs. uncomplicated categorization.
  • Implementing this reference standard in research will enhance study design and interpretability, despite challenges in establishing sensitivity and specificity for different settings and potential issues with including "probable" and "possible" UTI cases in research outcomes.*

Spermatozoa have been shown to carry key RNAs which, according to animal evidence, seem to play a role in early embryo development. In this context, a potential key growth regulator is insulin-like growth factor 2 (IGF2), a highly conserved paternally expressed imprinted gene involved in cell growth and proliferation which, recent observations indicate, is expressed in human spermatozoa. We herein hypothesized that sperm IGF2 gene expression and transmission at fertilization is required to support early embryo development.

Gene-environment interaction in functional hypothalamic amenorrhea.

Front Endocrinol (Lausanne)

September 2024

Department of Clinical, Surgical, Diagnostic and Pediatric Sciences, University of Pavia, Pavia, Italy.

Article Synopsis
  • - Functional hypothalamic amenorrhea (FHA) is a condition causing missed periods and chronic anovulation in young women, often due to issues like calorie restriction, excessive exercise, and stress, which disrupt hormone secretion.
  • - Recent research suggests a genetic component to FHA, indicating some women may be more susceptible to stress-related triggers, with rare gene variants linked to this condition.
  • - The review proposes that FHA is influenced by a mix of genetics, environmental factors, and epigenetic changes, and hints at the possibility of a similar condition in men, highlighting the need for more research in this area.

MRI and active surveillance: thoughts from across the pond.

Eur Radiol

September 2024

Glickman Urological and Kidney Institute, Cleveland Clinic, Cleveland, OH, USA.

In the United States (US), urological guidelines recommend active surveillance (AS) for patients with low-risk prostate cancer (PCa) and endorse it as an option for those with favorable intermediate-risk PCa with a > 10-year life expectancy. Multiparametric magnetic resonance imaging (mpMRI) is being increasingly used in the screening, monitoring, and staging of PCa and involves the combination of T2-weighted, diffusion-weighted, and dynamic contrast-enhanced T1-weighted imaging. The American Urological Association (AUA) guidelines provide recommendations about the use of mpMRI in the confirmatory setting for AS patients but do not discuss the timing of follow-up mpMRI in AS.
