Article Synopsis
  • The study investigated the reliability of the sperm chromatin dispersion assay in predicting outcomes of assisted reproductive technology, focusing on its correlation with sperm DNA fragmentation levels.
  • Elevated sperm DNA fragmentation was linked to reduced fertilization and embryo cleavage rates, but did not significantly impact overall clinical pregnancy, implantation, miscarriage, or live birth rates.
  • A specific analysis of the intracytoplasmic sperm injection method indicated poorer reproductive outcomes compared to other methods, highlighting potential limitations of sperm DNA integrity in assisted reproductive contexts.

Article Synopsis
  • A systematic review and meta-analysis were conducted to investigate trends in sperm concentration among fertile men and those not selected based on fertility in the U.S. from 1970 to 2023, amidst concerns of declining male fertility.* -
  • The analysis screened 874 articles, with 58 studies included that collectively involved 11,787 men, showing no overall decline in sperm concentration from 1970 to 2018 when adjusted for region, but a modest annual decline was noted when considering both region and fertility status.* -
  • The findings suggest that while previous concerns about declining sperm counts exist, the data presents a more complex picture that varies depending on fertility status and geographic region.*

Article Synopsis
  • Many kidney transplant patients are classified as having no rejection after biopsy, but some may actually have undetected rejection activities.
  • A study of over 5,000 biopsies found that more than half were marked as no rejection, yet many showed signs of subthreshold T cell-mediated rejection (TCMR) and antibody-mediated rejection (ABMR).
  • Patients with these hidden rejection activities are at increased risk of future graft issues, as higher levels of TCMR and ABMR correlate with worse kidney function and higher chances of rejection in subsequent biopsies.

Urinary Microbiome Research Has Not Established a Conclusive Influence: Revisiting Koch's Postulates.

Eur Urol Focus

September 2024

Jack Birch Unit, Department of Biology, York Biomedical Research Institute, University of York, York, UK.

In order to establish a causal relationship between urinary microbiota and a urological disease or condition, a series of rigorous scientific criteria must be met. Demonstration of an association between microbiota and a condition does not necessarily imply causality, importance, or clinical relevance.

Background: Lower urinary tract symptoms (LUTS) contribute to erectile dysfunction (ED) and premature ejaculation (PE). However, only a few studies have been conducted with representative groups of men that had well-balanced demographic characteristics. Thus, we aimed to confirm the effect of LUTS on ED and PE and to analyze the association between LUTS and men's sexual activity in a large representative cohort.

Male infertility is a major public health concern globally with unknown etiology in approximately half of cases. The decline in total sperm count over the past four decades and the parallel increase in childhood obesity may suggest an association between these two conditions. Here, we review the molecular mechanisms through which obesity during childhood and adolescence may impair future testicular function.

Vascular access for hemodialysis is the lifeline for patients with end-stage renal disease (ESRD); therefore, maintenance of the vascular access is of the utmost importance. The dialysis circuit can be complicated by stenosis or thrombosis. In particular, central venous stenosis is frequently encountered in the vascular access of patients with ESRD, and this complication may require endovascular management.

Background: Most patients starting chronic in-center hemodialysis (HD) receive conventional hemodialysis (CHD) with three sessions per week targeting specific biochemical clearance. Observational studies suggest that patients with residual kidney function can safely be treated with incremental prescriptions of HD, starting with less frequent sessions and later adjusting to thrice-weekly HD. This trial aims to show objectively that clinically matched incremental HD (CMIHD) is non-inferior to CHD in eligible patients.
