Introduction: Heart failure with preserved ejection fraction (HFpEF) is characterised by severe exercise intolerance, particularly in those living with obesity. Low-energy meal-replacement plans (MRPs) have shown significant weight loss and potential cardiac remodelling benefits. This pragmatic randomised trial aims to evaluate the efficacy of MRP-directed weight loss on exercise intolerance, symptoms, quality of life and cardiovascular remodelling in a multiethnic cohort with obesity and HFpEF.

Objectives: To examine the provision of cardiovascular magnetic resonance (CMR) using gadolinium-based contrast agents (GBCA) in patients with chronic kidney disease (CKD).
Methods: An electronic survey was sent to the service leads of all CMR units in the UK in October 2022 requesting information on current departmental protocols and practice.
Results: A response rate of 55% was achieved from the 82 UK CMR units surveyed.
Pulmonary embolism (PE) is a life-threatening condition with varied presentations, occasionally mimicking ST-segment elevation myocardial infarction (STEMI). This case highlights a 52-year-old male patient with a history of venous thromboembolism (VTE) who presented with progressive shortness of breath over a month, culminating in dyspnea at rest, and anterior ST-segment elevation on electrocardiography (ECG). The initial evaluation suggested STEMI.

Background: Specialist cardiac care has been shown to reduce inpatient mortality following non-ST segment myocardial infarction (NSTEMI), but whether this benefit extends beyond index admission is unclear.
Methods: Using the linked Myocardial Ischaemia National Audit Project (MINAP) registry, and Office for National Statistics mortality recording, we included 425,205 NSTEMI patients admitted to UK hospitals, between January 2005 and March 2019 that survived to discharge. 217,964 (52 %) were admitted to a specialty cardiac ward.

Sustained pathologic myocardial hypertrophy can result in heart failure(HF); a significant health issue affecting a large section of the population worldwide. In HF there is a marked elevation in circulating levels of the peptide urotensin II(UII) but it is unclear whether this is a result of hypertrophy or whether the high levels contribute to the development of hypertrophy. Methods: Utilising the Clinical Practice Research Datalink, we matched 16-50-year-old individuals with newly diagnosed type 2 diabetes to ∼10 individuals without using sex, age and primary care practice. GLOB-cAGE aims to harmonize non-identifiable data from longitudinal cohorts examining cardiovascular health and cardiovascular disease diagnosis and management in older individuals to perform meta-regression analyses using combined repositories of standardized subject-level data points.
