J Endocrinol Invest

December 2024

Department of Urology, Xiangya Hospital, Central South University, 87 Xiangya Road, Changsha, 410008, Hunan, China.

Article Synopsis
  • The study explores the genetic characteristics of urinary bladder paragangliomas (UBPGLs) in a cohort of 70 Chinese patients, aiming to better understand their heritability and genetic variations.
  • Results show that 38 patients had pathogenic variants (PVs), mainly in cluster 1A genes like SDHB and HRAS, with UBPGLs showing higher PV rates in cluster 1A compared to non-head and neck PGLs.
  • The findings indicate a diverse genetic profile among Chinese patients with UBPGLs, emphasizing the necessity for genetic testing to identify significant pathogenic variants.

Article Synopsis
  • The study investigates the factors leading to missed presurgical diagnoses of paragangliomas of the urinary bladder (UBPGLs), rare neuroendocrine tumors that pose diagnostic challenges.
  • Out of 124 patients analyzed from multiple regions, only about 47.6% were diagnosed before surgery, with those diagnosed earlier being younger and having larger tumors.
  • Key symptoms like micturition spells, hypertension, and sweating were found to significantly increase the likelihood of early biochemical testing, suggesting they could help establish timely diagnoses.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis belong to a severe dermatopathic spectrum that includes frequently fatal mucocutaneous manifestations consisting of whole epidermal necrosis and sloughing with bullous transformation, blistering, and further skin detachment. Notably, cancer patients are at higher risk of developing SJS than the general population as a consequence of both the nature of neoplastic disease and frequent exposure to anticancer drugs. Ribociclib is a newly approved cycline-dependent kinase inhibitor that has been recently associated with a single case of SJS.

Primary lower-limb lymphoedema is a chronic, progressive and debilitating condition with a difficult management, especially in advanced cases (elephantiasis). Recently, autologous lymph node transplantation (ALNT) appears to be a promising treatment for extremity lymphoedema. A case of a double ALNT for an advanced primary lower-limb lymphoedema is here reported: a contralateral inguinal lymph node flap was transferred to the knee and, in a second surgery, a thoracic lymph node flap was transplanted to the inguinal region.
